A forty-one-year-old homosexual man with acquired immunodeficiency syndrome (AIDS) and biopsy proven Kaposi's sarcoma of the oral cavity presented to our clinic with a several month history of violaceous papules and plaques on his trunk and extremities. Cryptococcosis is a potentially life-threatening opportunistic fungal infection presenting with a wide spectrum of clinical manifestations. This retrospective study (2018-2024) from North India analyzed 33 cryptococcosis cases, 66.7% HIV-positive. Fever, headache, and altered sensorium predominated. Overall mortality reached 51.5% at one year. Uncontrolled diabetes and limited amphotericin B use among Himalayan patients correlated with higher mortality. Equitable antifungal access remains crucial.

Cryptococcosis infection after transplantation is easily overlooked or misdiagnosed. We report a cluster of donor-derived cryptococcosis infection in liver and kidney transplant recipients from the same donor in China. Infections occurred within 1 month after transplantation, and were confirmed by using biopsies and blood tests.

A 73-year-old man presented with fatigue and anorexia and underwent FDG PET/CT scan to evaluate a right lung nodule revealed by a chest CT. The FDG PET/CT not only showed that the lung nodule was hypermetabolic but also revealed that there was significantly increased activity in the region of the duodenum. The findings were suspected as primary duodenal malignancy with metastases. However, an upper gastrointestinal tract endoscopic examination with further culture led to a diagnosis of duodenal cryptococcosis.
