Synovial Cysts Confounding Access to the Dorsal S1 Neural Foramen in Transforaminal Epidural Steroid Injections.

OBJECTIVES: The S1 dorsal foramen is the route for 30% of lumbar transforaminal epidural injections; it is therefore important to identify structures impeding S1 foraminal access. The study objective was to characterize the imaging findings, prevalence, and anatomic origin of synovial cysts presenting within the S1 neural foramen. METHODS: A case series (N = 14) established imaging characteristics of S1 synovial cysts. Imaging studies of 400 patients undergoing epidural injections were reviewed for lesions compromising S1 foraminal access. Cadaveric dissections defined the relationship of the

Ellis-van Creveld Syndrome: A Rare Case Report with Emphasis on Skeletal Manifestations.

INTRODUCTION: Bakers (popliteal) cysts are cystic masses that arise from bursae swellings filled with synovial fluid. It is assumed that preceding osteoarthritic or meniscal injuries cause the synovial fluid to extrude and concentrate, forming a gel-like material within a cyst. Clinical presentation is either symptomatic or asymptomatic swelling. The following symptoms are commonly associated with this condition: Knee pain, stiffness, and limited knee range of motion. Based on symptomatology and etiology, the management plan can be divided into conservative versus surgical intervention, by eit

A Case of Total Retinal Detachment With Multiple Retinal Cysts Showing a Favorable Postoperative Course.

A 29-year-old man was aware of vision loss in his left eye that had been present for about two years. At the time of his visit to our clinic, his best-corrected visual acuity (BCVA) was 20/16 in the right eye and 20/200 in the left eye. There were no abnormal findings in the anterior or intermediate translucent areas of either eye. The fundus examination revealed a retinal dialysis in the inferior nasal retina of the left eye, followed by multiple retinal cysts on the pole side, and total retinal detachment. The fundus of the right eye showed no abnormal findings of note. A diagnosis of rhegma

Cervical spine arachnoid cyst complicated by spontaneous intracystic hemorrhage: Case report and review of the literature.

BACKGROUND: Spinal intradural extramedullary arachnoid cysts represent about 1-3% of all primary spinal space-occupying lesions often causing spinal cord and/or radicular nerve compression. Spontaneous intralesional hemorrhages are extremely rare and are typically found within intracranial arachnoid cysts. Here, a 55-year-old female presented with a spontaneous hemorrhage into a cervical spine arachnoid cyst warranting surgical intervention (i.e., fenestration/excision/occlusion). CASE DESCRIPTION: A 55-year-old female presented with 3 weeks of dull pain in the cervicothoracic region. She sub

[A Rathke's cleft cyst with a moving mass in the cyst].

We reported a case of Rathke's cleft cyst (RCC) with a moving mass in the cyst. A fifty-eight-year-old woman complaining of headache was admitted to our hospital. She suffered from hyponatremia, hypothalamic hypopituitarism, but did not show any neurological deficits, nor visual field nor visual acuity disturbance. MRI revealed an intrasellar cyst including a mass shadow and the cyst did not compress the hypothalamus. Interestingly, this mass moved gradually from the floor to the posterior wall of the sella turcica according to changes in position from standing to supine. We operated to remove

Delayed presentation and successful repair of a recurrent vesicovaginal fistula after hysterectomy and primary abdominal repair.

Urogenital fistulae are an uncommon consequence of gynecologic surgery. Vesicovaginal fistulae due to gynecologic surgery generally appear 1-6 weeks after surgery and recurrent fistulae within 3 months of their repair. The pathogenesis of vesicovaginal fistula formation remains unclear. We present the case of a 36-year-old woman with a spontaneously recurring vesicovaginal fistula 21 months after abdominal repair of a vesicovaginal fistula caused by a laparoscopic-assisted vaginal hysterectomy. During the repair of the fistula and excision of the vaginal cuff, two small fluid-filled cysts betw

Management of Neonatal Facial Paralysis due to Cerebellopontine Angle Arachnoid Cyst: A Case Report.

Arachnoid cysts are benign, cerebrospinal fluid-filled collections that can be located in the brain or spinal cord. Arachnoid cysts form approximately 1% of all intracranial lesions. They are accepted as arachnoid developmental anomaly and arise from membrane splitting or duplication. Generally, lesion growth causes symptoms because of mass effect or obstruction. Arachnoid cyst growing mechanisms are a largely controversial issue. We report the case of a neonatal female patient who presented with congenital facial paralysis. Magnetic resonance imaging showed a right cerebellopontine angle arac

Successfully treated Escherichia coli-induced emphysematous cyst infection with combination of intravenous antibiotics and intracystic antibiotics irrigation in a patient with autosomal dominant polycystic kidney disease.

A 62-yr-old woman with an autosomal dominant polycystic kidney disease (ADPKD) was admitted to our hospital for further evaluation of intermittent fever, nausea and left flank discomfort. The computed tomography (CT) scan revealed a gas-forming, infectious cyst of approximately 8.1 cm in size in left kidney lower pole. Escherichia coli was identified from the cyst fluid culture examination. Her symptoms improved only after the concomitant use of intravenous ciprofloxacin and an intracystic irrigation of ciprofloxacin through a percutaneous cystostomy drainage. Our case presents the successfull…

[Esophageal duplication cyst resected with video-assisted thorasic surgery; report of a case].

Duplication of the alimentary tract is a developmental anomaly, which may affect any part of the digestive tract from the mouth to the anus. Esophageal duplication cyst is caused by an incomplete differentiation of the foregut. We report a case of esophageal duplication cyst resected using video-assisted thoracic surgery (VATS). Chest radiography of a 23-year-old woman showed an abnormal shadow. Chest computed tomography (CT) indicated a cystic lesion adjacent to the descending aorta and the esophagus. Magnetic resonance imaging (MRI) showed that the cystic lesion was filled with protein-rich

Renal cell carcinoma presenting as a huge simple renal cyst.

Simple renal cysts are the most common renal masses, accounting for roughly 65-70% of cases. Transformation of a simple renal cyst into a renal cell carcinoma (RCC), however, is extremely rare. We reported a 48-year-old man with a right huge simple renal cyst from which about 800 ml of dark yellowish fluid with elevated protein and lactic dehydrogenase (LDH) levels was aspirated. Cytological examination disclosed negative for malignant cells. Nephrectomy was performed because of intractable flank pain 3 months after aspiration. The pathology examination confirmed RCC. While this is an extremel

A case of intracystic papillary carcinoma with a multilocular cyst of the breast in male.

Intracystic papillary carcinoma of the breast in males is a very rare disease but has an excellent prognosis. We report the case of a 68-year-old man who had a right subareolar soft mass. Imaging examinations showed a multilocular cyst with an intracystic component, and benign disease was diagnosed. Fine needle aspiration of the cyst fluid revealed many malignant cells, and modified radical mastectomy was performed under general anesthesia. Histological examination showed intracystic papillary carcinoma with a multilocular cyst. No positive lymph nodes were involved. The diagnosis of intracyst…

Panhypopituitarism due to Rathke's cleft cyst associated with pituitary oncocytoma.

A 38-year-old male with panhypopituitarism due to Rathke's cleft cyst associated with a pituitary oncocytoma is reported. The presenting signs were general myalgia and slight fatigue. Endocrine examinations revealed panhypopituitarism. Magnetic resonance imaging disclosed a suprasellar cystic lesion of the pituitary gland. Cytological examination demonstrated ciliated cells in the mucinous fluid flowing from the cyst during the pituitary operation. A pituitary oncocytoma with randomly scattered S-100 immunoreactive cells was found upon histologic examination of the nodular tissue curettaged fr

Is cystic disease related to breast cancer?

Human breast cystic disease is a common premenopausal benign breast condition. Apocrine metaplasia of normal breast epithelium is the lesion that allows cysts to develop. Apocrine metaplasia and breast cysts occur frequently in association with other proliferative changes in breast epithelium, especially breast epithelial hyperplasia. Clinical follow-up studies of women with breast cystic disease indicate an increased risk of subsequent development of breast carcinoma. This risk is enhanced when multiple cysts occur. A positive family history of breast carcinoma adds to the increased risk that

Giant Mediastinal Thymic Cyst.

The authors present a rare case of giant mediastinal cyst which arises from the thymus gland, and goes down in both pleural spaces, especially in the right chest cavity where a dominant part of the cyst was present. The cyst was full with 2.5 liters of transparent fluid, and compressed surrounding structures - heart and both lungs, especially the right one which was partially collapsed. The patient was a 52 years old woman, without any clinical symptoms. Accidentally, on the screened chest X-ray a shading in the distal third of the right chest was detected. The case was well documented with a

Pituitary cyst presenting with hyponatremia and increased secretion of brain natriuretic peptide. Case report.

In most cases of pituitary cyst there are no clinical symptoms and the lesions are found incidentally. The authors report the case of a 60-year-old man with a pituitary cyst causing visual disturbance and hyponatremia. The patient presented with appetite loss and general fatigue. On admission, blood workup showed severe hyponatremia (112 mEq/L), and bitemporal hemianopsia was observed on neurological examination. Magnetic resonance imaging revealed an intra- and suprasellar region cystic mass extending to the frontal base and hypothalamic area. The serum level of brain natriuretic peptide (BNP

Delayed diagnosis of traumatic rupture of hydatid cyst of the liver--a case report.

We present a patient with slow rupture of hydatid cyst into the peritoneal cavity, presenting as massive abdominal distension and respiratory embarrassment. On paracentesis, no fluid could be drained. A small lateral incision was made under local anaesthesia to drain the 'ascites', but daughter cysts typical of hydatid came out. On laparotomy, there was a cyst in the right lobe of liver which had ruptured into the peritoneal cavity leading to secondary echinococcosis.

Hyperreactio luteinalis associated with non-immune hydrops fetalis--the role of pituitary hormones.

The level of pituitary hormones was measured in the serum and cysts' aspirate in a case of hyperreactio luteinalis (HL) complicating non-immune hydrops fetalis. The level of follicular stimulating hormone (FSH), prolactin and growth hormone (GH) was within normal range for pregnancy in both the serum and cysts' aspirate. The importance of increased luteinizing hormone (LH) level which was demonstrated in the cysts' fluid should further be determined.
