We report a case of refractory haematuria successfully managed using intravesical PuraStat®, a synthetic haemostatic agent primarily used in gastrointestinal bleeding. A 73-year-old male presented repeatedly with haematuria following bladder neck incision, unresponsive to conventional management. Application of PuraStat® via air cystoscopy resulted in complete haemostasis with no recurrence at two months. This case supports the expanding role of PuraStat® in urological bleeding where standard techniques fail.

We describe a Schistosoma haematobium infection with asymptomatic eosinophilia, persistently negative urine microscopy, and late seroconversion (7.5 months) in a traveler returning from Mali. After initial negative parasitological tests, travel history led to diagnostic cystoscopy, allowing final diagnosis with urine microscopy after the bladder biopsy. The patient was successfully treated with praziquantel. Difficulties in making the diagnosis of schistosomiasis in asymptomatic returning travelers are discussed; we propose a trial treatment in these cases.

A 88-year-old woman presented with bladder tamponade caused by gross hematuria. She had received 2 courses of bacillus-Calmette-Guerin (BCG) intravesical instillation therapy 4 months previously because of the bladder tumor with carcinoma in situ (CIS). Imaging studies and cystoscopy showed a bladder tumor invading into perivesical fat. She underwent total cystectomy and bilateral ureterocutaneostomy on May 27, 2002. Histopathologically, the tumor consisted of squamous cell carcinoma and transitional cell carcinoma.

A case of XXY Klinefelter's syndrome associated with bilateral cryptochidism, -ultiple capillary hemangiomas and vesical telangiectasia is reported. Testicular biopsy revealed Leydig cell hyperplasia with hyalinization of the seminiferous tubular membrane. The patient presented with recurrent hematuria. Intrauterine device represents the most reversible method of contraceptive worldwide. Its insertion is a medical procedure not free from complication. We report a rare case of intravesical migration of a copper intrauterine device inserted 18 months earlier in a 28-year-old multiparous woman. The patient presented with irritative lower urinary tract symptoms, and she was managed endoscopically. This case underscores the role of cystoscopy in irritative lower urinary tract symptoms post IUD insertion.

Three cases of persistent urinary incontinence from unrecognized small vesicovaginal fistula after abdominal hysterectomy for benign gynecologic disease are presented. Visualization of urine in the posterior fornix was suggestive; diagnosis was confirmed by methylene blue test and cystoscopy. Two patients had undergone interval Marshall-Marchetti-Krantz operations for suspected stress-related urinary incontinence. An extensive literature survey confirms that the vesicovaginal fistula were an unlikely complication of the Marshall-Marchetti-Krantz procedure.

A 49-year-old female visited our hospital with a complaint of pelvic pain. She was under treatment for diabetes mellitus and dilated cardiomyopathy. Radiography revealed a radiolucent area in the bladder. We suspected vesicorectal fistula. Computed tomography showed gas within the bladder wall and the lumen. Cystoscopy revealed diffuse emphysema in the bladder wall without fistula. These findings were consistent with a diagnosis of emphysematous cystitis. The urine culture yielded Escherichia coli. After urinary drainage and antibiotic therapy, she was cured of emphysematous cystitis.

In most cases the foreign body can be removed transurethrally with a pair of grasping forceps. In some cases objects have to be cut or resected into small fragments, making it easier to extract the object or wash it out. An open surgical procedure via suprapubic transvesical approach is reserved for large-sized bodies or in cases when endoscopic methods are unsuccessful. The laparoscopic removal is one of further techniques that provide the opportunity to untie the knots (Fig. 6, Ref. 6).

OBJECTIVE: To probe into the diagnosis and treatment of urachal remnants with heterotopic sinus.
METHODS: Two cases with penis dorsal drainage for 5 approximately 6 years were diagnosed as urachal anomaly by sinus probing, methylene blue injection test, cystoscope examination, and type B ultrasonic examination, and both received surgical removal of the urachus and all the abnormal tissues associated with it.
RESULTS: Surgery and pathology proved urachal cyst with communication drainage in both of the patients. Follow-up survey showed no recurrence and malignancy.

Fifty-five cases of interstitial cystitis are reported, symptomatology in these 43 women and 12 men being of a severe nature: diurnal and nocturnal pollakiuria, supra-pubic pain. Disorders were always permanent and had been present for at least 6 months, diagnosis being confirmed by endoscopy.
