A biomechanical analysis of applied pinch force during periodontal scaling.

One of the factors associated with the high prevalence of upper extremity musculoskeletal disorders, such as carpal tunnel syndrome, among dental practitioners is the repeated high pinch force applied during periodontal scaling. The goal of this study was to determine the relationship between the pinch force applied during periodontal scaling and the forces generated at the tip of the tool. A linear biomechanical model that incorporated tool reaction forces and a calculated safety margin was created to predict the pinch force applied by experienced and inexperienced dentists during periodontal…

Care network for individuals with temporomandibular disorders in the Brazilian Unified Health System: Perspective of managers and dental surgeons.

OBJECTIVE: To analyze the care network for individuals with orofacial pain (OFP) and temporomandibular disorders (TMD) in the Brazilian Unified Health System (SUS). METHODS: Oral health managers and primary health care (PHC) dentists completed electronic questionnaires addressing service availability, clinical practices, knowledge-related items, and referral services for TMD and OFP. Data were statistically analyzed at a significance level of 5% (p < .05). RESULTS: Most managers reported that neither PHC (n = 82; 85.4%) nor secondary care (n = 68; 70.8%) treats patients with TMD. Most dentis…

Review of Guidelines for Dental Antibiotic Prophylaxis for Prevention of Endocarditis and Prosthetic Joint Infections and Need for Dental Stewardship.

Dentists prescribe 10% of all outpatient antibiotic prescriptions, writing more than 25.7 million prescriptions per year. Many are for prophylaxis in patients with prosthetic joint replacements; the American Dental Society states that "in general," prophylactic antibiotics are not recommended to prevent prosthetic joint infections. Orthopedic surgeons are concerned with the risk of implant infections following a dental procedure and, therefore, see high value and low risk in recommending prophylaxis. Patients are "stuck in the middle," with conflicting recommendations from orthopedic surgeons …

Health and wellness for women in the profession.

Regardless of gender, dentists can and do experience illness and other problems that may disrupt or impair dental practice, including substance abuse involving alcohol and/or other drugs. As part of the ethical obligation to maintain public confidence through responsible and effective self-governance, the American Dental Association (ADA) and state dental components offer health and wellness resources to members through programs such as CDAD-Dentist Well-Being Committee. Though the percentage of women entering the dental profession has consistently increased, utilization of CDAD resources by f…

Dental considerations for the treatment of patients with diabetes mellitus.

BACKGROUND: Dentists play a major role as part of an allied health team in providing oral care to patients with diabetes. As such, they may detect undiagnosed cases of diabetes and refer patients to physicians for further evaluation. METHODS: The author describes new concepts in metabolic control for diabetes and the relationship of oral complications to diabetes mellitus. The treatment of acute oral infections and the dentist's role in supporting patients in smoking-cessation programs are approaches that may reduce morbidity from diabetes mellitus. In consultation with the patient's physicia…

Antibiotics and oral contraceptives: new considerations for dental practice.

The aim of this paper is to highlight a change in guidance relating to possible interactions between antibiotics and oral contraceptives. Until recently, dentists have been advised to warn women taking the combined oral contraceptive pill of the routine need to use additional contraceptive measures while taking courses of broad spectrum antibiotics. Recent guidance relating to this issue has changed and dentists may not be aware of this. This paper reminds dentists of the previous guidelines and related evidence, reviews the pharmacokinetics of hormonal contraception and presents them with the…

Dental Practitioners' Thresholds for Restorative Intervention in Carious Lesions: A Survey-Based Systematic Review Update.

UNLABELLED: <p>Introduction: Despite evidence supporting the clinical and cost-effectiveness of minimally invasive dentistry (MID), its adoption by the dental profession has been slow. A systematic review in 2016 found the majority of dentists intervene invasively earlier than necessary. The aim was to update this review of the assessment of dental practitioners' thresholds for providing restorative treatment for carious lesions given changes in evidence, teaching, and guidelines since 2016. The primary outcome was dental practitioners' restorative thresholds (the extent of the lesion when the…

Control of blood-transmitted infections in dentistry.

As knowledge has accumulated on the blood-transmitted pathogenic agents, the contact with biological fluids (blood, plasma, saliva, etc.) from apparently healthy individuals has started to be regarded as a real professional risk for dentists. Theoretically, exposure to a contaminated biological specimen may have as a consequence transmission of infection from patient to dentist, from dentist to patient and from patient to patient via inadequately decontaminated and sterilized dental equipment. The present study is concerned with the analysis of the specific conditions that favor the occurrence…

Factors influencing treatment decisions in permanent mature teeth with irreversible pulpitis: a questionnaire-based study.

BACKGROUND: Decision-making in dentistry is a complex process, and this study evaluated factors that influence dentists' approaches in permanent mature teeth with irreversible pulpitis. METHODS: An online questionnaire was distributed to a group of dental practitioners. The questionnaire surveyed dentists' opinions about the management of vital permanent teeth with irreversible pulpitis including a case scenario. RESULTS: Data from 262 respondents were analysed. Barriers to perform vital pulp therapy (VPT) included presuming it an inappropriate long-term treatment (29.7%), lack of knowledge,…

Finger osteoarthritis and differences in dental work tasks.

Mechanical load has been proposed as a risk factor for hand osteoarthritis. Dentists produce high manual forces in their work tasks. We studied whether the pattern of dental work tasks was associated with finger osteoarthritis. Radiographs of both hands of 291 middle-aged female dentists were examined for the presence of osteoarthritis. Patterns of dental work tasks during work history were empirically defined by cluster analysis. Three patterns emerged reflecting high, moderate, and mild task variation. Age, specialization, years in clinical work, various activities requiring hand use, family…

Occupational allergic contact dermatitis due to acrylates in Lodz.

Among 1619 patients suspected of occupational contact dermatitis examined during the years 1990-1994, sensitivity to acrylates was diagnosed in 9 persons (4 dental technicians, 4 dentists, 1 textile printer). Ethyleneglycol dimethacrylate (5 positive patch tests), methyl methacrylate (4), 2-hydroxyethyl methacrylate (4) and triethyleneglycol dimethacrylate (4) were the most common sensitizers. Comparison of patch test results in dental technicians and dentists indicated that dentists were sensitive to a greater number of (meth)acrylate (acrylate and methacrylate) allergens and also to certain …

Dental Antibiotic Prescribing Practices and Antimicrobial Resistance Knowledge in Singapore.

INTRODUCTION AND AIMS: While dental antibiotic prescribing accounts for approximately 10% of all antibiotic prescriptions across human healthcare worldwide, up to 80% are inappropriate. These contribute to the growing global health threat of antimicrobial resistance (AMR). Effective antibiotic stewardship requires an up-to-date understanding of antibiotic prescribing practices and knowledge. This study aimed to assess the antibiotic prescribing practices, knowledge and attitudes among dentists in Singapore. METHODS: A survey was distributed in 2024 to dentists using convenience sampling throu…

Influence of body mass index on perceived lower facial asymmetry.

Facial asymmetry plays a crucial role in facial esthetics, and factors such as body mass index may influence its visibility by altering soft tissue thickness, which can lead to differences in how orthodontists, dentists, and laypeople perceive lower facial asymmetry. This discrepancy can affect aesthetic evaluations and treatment expectations. The aim of this study is to evaluate the perceptibility of lower facial asymmetry among individuals with different body mass indices as assessed by orthodontists, dentists, and laypeople. Frontal photographs were obtained from four female models represen…

The decision to repair or replace a defective restoration is affected by who placed the original restoration: findings from the National Dental PBRN.

OBJECTIVES: To evaluate how restoration characteristics are associated with the decision to repair or replace an existing restoration. The following hypotheses were studied: dentists who placed the original restoration are more likely to repair instead of replace restorations (H1) that are in molar teeth; (H2) that are in the upper arch; (H3) that have amalgam restorative material; (H4) if a fracture is not the primary reason for the defect; and (H5) when the restoration comprises more than one surface. METHODS: This cross-sectional study used a consecutive patient/restoration recruitment des…

Tobacco cessation in the dental office.

Evidence-based tobacco-cessation guidelines when used by clinicians are effective in reducing tobacco use and obtaining successful quits by patients. Dentists have been encouraged to provide instruction and intervention on tobacco cessation in the dental office. The dental provider is in the unique position to relate oral findings to the patient and to provide advice to tobacco-using patients to quit. In addition, dentists are able to assess patients' self-addiction and level of readiness to quit tobacco use. With this information, dentists can assist in helping patients to stop using tobacco …
