[Affective disorders--depression diagnosis and prognosis].

Affective disorders have been described since ancient Greece. The ICD-10 classification of mania and depression mixes typology with severity. A hierarchical model for depression separating psychotic, melancholic and non-melancholic depression is outlined. Psychometric measurements of mania and depression, including scales for use in clinical practice are presented. The course of affective disorders underlines the severity of the disorders with many recurrent episodes and long duration, especially in the depressive state.

Is there a continuity between bipolar and depressive disorders?

BACKGROUND: Recent studies questioned the current categorical split of mood disorders into bipolar disorders (BP) and depressive disorders (MDD). METHODS: Medline database search of papers from the last 10 years on the categorical-dimensional classification of mood disorders. Various combinations of the following key words were used: mood disorders, bipolar, unipolar, major depressive disorder, spectrum, category/categorical, classification, continuity. Only English language clinical papers were included, review papers were excluded, similar papers selected by quality. The number of papers fo…

Understanding comorbidity with depression and anxiety disorders.

Comorbidity is the rule with anxiety and depressive disorders. Anxiety and major depressive disorder are often comorbid with each other; these disorders are commonly associated with other psychiatric disorders; and they are frequently found coexisting with long-standing chronic medical conditions such as cardiovascular disease and diabetes mellitus. The comorbidity of major depressive and anxiety disorders is associated with barriers to treatment and worse psychiatric outcomes, including treatment resistance, increased risk for suicide, greater chance for recurrence, and greater utilization of…

Agitated depression in bipolar II disorder.

STUDY AIM: To test diagnostic validity or utility of agitated depression (AD) in bipolar II disorder (BP-II). METHODS: Three hundred and twenty BP-II major depressive episode (MDE) outpatients interviewed with the Structured Clinical Interview for DSM-IV, Hypomania Interview Guide (HIG), and Family History Screen. AD defined as MDE with psychomotor agitation. Mixed depression defined as MDE with > or =4 hypomanic symptoms. AD, non-AD, mixed-AD, non-mixed-AD, and mixed-non-AD were compared versus diagnostic validators. RESULTS: AD was present in 35.0%, 75.8% of AD were mixed, while only 14.3%

Anxiety and depressive disorders in college youth.

Fear and anxiety are common experiences throughout childhood and adolescence. Anxiety disorders, along with depression and dysthymia, are characterized as internalizing disorders; they stand in distinction from the externalizing disorders representing such conditions as oppositional-defiant disorder and conduct disorder. Clinicians evaluating anxiety disorders in college youth face the task of differentiating normal, transient, developmentally appropriate expression of anxiety from pathologic states of anxiety. The developmental course of anxiety and depression, its appropriateness, and its bo

Depressive disorders in relation to neurootological complaints.

Depression is a state of depressed mood characterized by feelings of sadness, despair, and discouragement. Depression ranges from normal feelings of "the blues" through dysthymia to major depression. Endogenous depression has been identified with a specific symptom complex: psychomotor retardation, early morning awakening, weight loss, excessive guilt, and lack of reactivity to the environment. Reactive depression is precipitated by a stressful life event. In the field of depression, we found an overlapping activity between psychiatry and neurootology. Our sample comprises 134 patients (53 [39

[Anxiety and depressive disorders in tobacco dependence].

The association between tobacco dependence and various forms of anxiety and depressive disorders is well established. The presence of these disorders constitutes a key cause of failure in subjects seeking to stop smoking. In addition to the clinically manifest forms of such anomalies, systematic screening for minor forms of these disorders was performed at the Centre for Tobacco Studies of Créteil, France; as well as the standard questionnaire concerning personal and familial history and current psychological well-being, subjects were asked to complete two self-questionnaires, the HAD (Hospita…

[Treatment of depression secondary to psychotic disorders].

More than 25% patients suffering from psychotic disorders would show during their evolution some depressive disorders. More than 10% would die from suicide. Secondary depressions following psychotic disorders are heterogeneous. They have to be specified in order to assess therapeutics including biological psychological and socio-environmental approaches. Treatment of depressive disorders correlated to psychotic symptomatology, being mood congruent or not, is the same as characterized depression. However it is preferred to associate antidepressant and neuroleptic or electroconvulsivotherapy. De…

[Treatment with tianeptine for depressive disorders in the elderly].

Depression is frequent in the elderly but difficult both to diagnose and treat due to a number of distinctive features. Tianeptine is a novel antidepressant with a reverse mode of action to that of the selective serotonin reuptake inhibitors yet with proven efficacy and safety. 63 elderly patients (mean age: 68.8 years; range: 65-80 years) with depressive symptoms (major depression: 55.6%; dysthymia: 44.4%) were included in a 3-month open multicenter study with tianeptine (25 mg daily). 43 patients (68.2%) completed the study. There were no drop-outs due to side-effects. Total Montgomery and A

Women's health. Depression and related disorders.

Common mood disorders that are seen in women in primary care are major depression, dysthymic disorder, adjustment reaction with depressed mood, and premenstrual syndrome. This article reviews the diagnosis and treatment of major depression, which also are applicable in many ways to other mood disorders. Specifics about other mood disorders that are encountered frequently in women are discussed subsequently.

Dysthymic disorder: the depression that never quits.

Dysthymic disorder, an insidious and chronic depressive mood disorder that waxes and wanes in intensity over several years, is fairly prevalent in healthcare settings. Although the explicit etiology is unknown, serotonergic dysfunction may be involved. Dysthymia appears to have a high rate of comorbidity, including both psychiatric and medical disorders. The primary care physician should maintain an awareness of this mood disorder, be able to screen efficiently for signs and symptoms, and be able to differentiate major depression from dysthymia. The foundation of treatment is pharmacotherapy,

[The disordered organization of the cortical processes in depression].

The study of depressive patients with the method of the EEG mapping revealed some stable differences from the norm. Firstly, in depression there are the disturbances of the definite cortical zones' activation: the right anterior and the left posterior cortical parts are hyperactivated while the left anterior and the right posterior-- relatively inhibited, evidently causing the informational processing disturbances. The important link of these disturbances--the disconnection of the anterior and the posterior cortical regions that could be called "transversal functional blockade". The second fea

Depressive disorders and dementia: the clinical view.

The relationship between depression and dementia is complex. Transient emotional disturbances and long-lasting depressive disorders may occur as psychological reactions to the loss of mental abilities or as behavioural manifestations of brain injury. On the other hand, major depressive disorder of late onset or of a recurrent type may be superimposed on dementing illness. Depression, particularly in elderly individuals, can manifest itself as a reversible syndrome of dementia that responds favourably to antidepressant treatment. It is not known whether the dementia syndrome of depression repre…

[Depressive disorders. Risk factors of recurrence].

The prediction of recurrence in affective disorders is very difficult and in individual cases not possible at all. Socio-demographic variables as body build, positive family history, marital status and social class, do not predict recurrence. This is probably true for sex and age, too. The impact of stressors and social support on the course is relatively weak. In this context the research on expressed emotion is of interest, as it gives some evidence that the presence of a sick spouse may increase the relapse rate. On the other hand an inadequate premorbid personality, especially a high neuro

Parental characteristics in relation to depressive disorders.

Using a reliable and valid measure of reported parental care and overprotection (the Parental Bonding Instrument) patients with two types of depressive disorder were compared with a control group, and the relationships to depressive experience examined in a non-clinical group as well. Bipolar manic-depressive patients scored like controls whereas neurotic depressives reported less parental care and greater maternal overprotection. Depressive experience in the non-clinical group was negatively associated with low parental care and weakly associated with parental overprotection.

[Clinical evidence of acupuncture and moxibustion for depressive disorder: controversy, reflection and prospect].

To clarify the clinical evidence required for integrating acupuncture and moxibustion treatment for depressive disorder into the mainstream psychiatric diagnostic and therapeutic system, as well as the existing deficiencies, this paper summarizes the current state of evidence for acupuncture and moxibustion treatment for depressive disorder from three perspectives: clinical guidelines, systematic reviews, and clinical trials. The differentiation of recommendations of different guidelines and its causes are analyzed, and the key issues such as the design and implementation of clinical research,…

Longitudinal peripheral DNA methylation remodeling following ketamine-assisted psychotherapy in major depressive disorder.

Ketamine produces rapid antidepressant effects, but the molecular processes associated with their persistence remain poorly defined in humans. We examined longitudinal DNA methylation changes in 10 adults with major depressive disorder undergoing ketamine-assisted psychotherapy using two pre-treatment assessments (T1, T2) and one post-treatment assessment (T3). Participants received three sessions of intranasal racemic ketamine within a structured psychotherapy protocol. Buccal DNA methylation was measured using the Illumina MethylationEPIC v2.0 BeadChip. Depressive symptoms improved markedly …

Neurobiological mechanisms and brain network dysfunction in major depressive disorder: an integrative review.

Major depressive disorder (MDD) is a prevalent and disabling psychiatric disorder characterized by persistent mood disturbances, cognitive impairments, and significant systemic consequences. Despite its substantial clinical burden, diagnosis remains challenging because current approaches rely largely on subjective symptom-based assessments and are complicated by marked biological and clinical heterogeneity. In this review, we synthesize findings from neuroimaging, electrophysiology, genetics, and molecular studies to provide an integrated overview of the structural, functional, and neurobiolog…
