Preliminary characterization of the relationship between clinical assessment of prosthetic fit using plasticine and in-socket pressure measurement in transtibial amputation.

BACKGROUND: Currently, clinicians use a ball of plasticine to measure contact between the end of amputated residual limbs and prosthetic sockets to assess distal-end fit. OBJECTIVES: This study characterizes the forces required to deform plasticine and relates these to clinical decisions made about socket fit. STUDY DESIGN: Cross-sectional study. METHODS: Loading thresholds for plasticine deformation were identified. Three different sized check sockets were loaded with a phantom limb, distal-end sensors, and in-situ plasticine. Three transtibial prosthesis users were recruited and completed

Deciphering brain activation during wrist movements: comparative fMRI and fNIRS analysis of active, passive, and imagery states.

Understanding the complex activation patterns of brain regions during motor tasks is crucial. Integrated functional magnetic resonance imaging (fMRI) and functional near-infrared spectroscopy (fNIRS) offers advanced insights into how brain activity fluctuates with motor activities. This study explores neuronal activation patterns in the cerebral cortex during active, passive, and imagined wrist movements using these functional imaging techniques. Data were collected from 10 right-handed volunteers performing a motor task using fMRI and fNIRS. fMRI utilized a 3T scanner and a 20-channel head co

Role of Mechanoinsensitive Nociceptors in Painful Diabetic Peripheral Neuropathy.

The cutaneous mechanisms that trigger spontaneous neuropathic pain in diabetic peripheral neuropathy (PDPN) are far from clear. Two types of nociceptors are found within the epidermal and dermal skin layers. Small-diameter lightly myelinated Aδ and unmyelinated C cutaneous mechano and heat-sensitive (AMH and CMH) and C mechanoinsensitive (CMi) nociceptors transmit pain from the periphery to central nervous system. AMH and CMH fibers are mainly located in the epidermis, and CMi fibers are distributed in the dermis. In DPN, dying back intra-epidermal AMH and CMH fibers leads to reduced pain sens

Platelet transfusions in a murine model of neonatal polymicrobial sepsis: Divergent effects on inflammation and mortality.

BACKGROUND: Platelet transfusions (PTxs) are often given to septic preterm neonates at high platelet count thresholds in an attempt to reduce bleeding risk. However, the largest randomized controlled trial (RCT) of neonatal transfusion thresholds found higher mortality and/or major bleeding in infants transfused at higher thresholds. Using a murine model, we investigated the effects of adult PTx on neonatal sepsis-induced mortality, systemic inflammation, and platelet consumption. STUDY DESIGN AND METHODS: Polymicrobial sepsis was induced via intraperitoneal injection of cecal slurry preparat

Radiographic Assessment of Ankle Fracture Displacement: A Validation Study.

BACKGROUND: Ankle fracture displacement is an important outcome in clinical research examining the effectiveness of surgical and rehabilitation interventions. However, the assessment of displacement remains subjective without well-described or validated measurement methods. The aim of this study was to assess inter- and intrarater reliability of ankle fracture displacement radiographic measures and select measurement thresholds that differentiate displaced and acceptably reduced fractures. METHODS: Eight fellowship-trained orthopaedic surgeons evaluated a set of 26 postoperative ankle fractur

Do school-aged children with burn injuries during infancy show stress-induced activation of pain inhibitory mechanisms?

There is evidence in humans and animals that neonatal and early infant pain and stress may sensitize excitatory pain pathways. Possibly such experiences may result in long-term diminished activation of phasic endogenous pain inhibitory mechanisms. We studied stress-induced activation of endogenous pain inhibitory mechanisms in school-aged children (10-16 years) who had suffered moderate (N= =12) or severe (N=10) burn injuries in infancy (6-24 months of age) and 20 controls. Before and after the stress phase, pain threshold and pain tolerance (heat, pressure, ischemic pain) were assessed. Stres

Switching feedback mechanisms realize the dual role of MCIP in the regulation of calcineurin activity.

Calcineurin (CaN) assists T-cell activation, growth and differentiation of skeletal and cardiac myocytes, memory, and apoptosis. It also activates transcription of the nuclear factor of activated T-cells (NFAT) family including hypertrophic target genes. It has been reported that the modulatory calcineurin-interacting protein (MCIP) inhibits the CaN activity and thereby reduces the hypertrophic response. However, it has been shown that MCIP facilitates or permits the hypertrophic response under some stress conditions such as isoproterenol infusion or pressure overload by transverse aortic cons

Decision curve analysis confirms higher clinical utility of multi-domain versus single-domain prediction models in patients with open abdomen treatment for peritonitis.

BACKGROUND: Prediction modelling increasingly becomes an important risk assessment tool in perioperative systems approaches, e.g. in complex patients with open abdomen treatment for peritonitis. In this population, combining predictors from multiple medical domains (i.e. demographical, physiological and surgical variables) outperforms the prediction capabilities of single-domain prediction models. However, the benefit of these prediction models for clinical decision-making remains to be investigated. We therefore examined the clinical utility of mortality prediction models in patients sufferin

Psychophysical study of stinging pain evoked by brief freezing of superficial skin and ensuing short-lasting changes in sensations of cool and cold pain.

Psychophysical methods were used to investigate pain in human subjects elicited by controlled freezing of the skin using a novel vortex thermode. When cooling stimuli delivered with a small thermode (7 mm diameter) exceeded the normal cold pain threshold into the sub-zero temperature range (-5 to -11 degrees C), all subjects reported an intense, sharp stinging pain sensation which occurred suddenly and was readily differentiated from normal cold pain. The onset of this stinging 'freezing' pain was closely correlated with a sudden increase in skin temperature beneath the thermode of 4.77+/-0.86

Acoustic Trauma Modulates Cochlear Blood Flow and Vasoactive Factors in a Rodent Model of Noise-Induced Hearing Loss.

Noise exposure affects the organ of Corti and the lateral wall of the cochlea, including the stria vascularis and spiral ligament. Although the inner ear vasculature and spiral ligament fibrocytes in the lateral wall consist of a significant proportion of cells in the cochlea, relatively little is known regarding their functional significance. In this study, 6-week-old male C57BL/6 mice were exposed to noise trauma to induce transient hearing threshold shift (TTS) or permanent hearing threshold shift (PTS). Compared to mice with TTS, mice with PTS exhibited lower cochlear blood flow and lower

Temporal discrimination of two passive movements in writer's cramp.

Although movement abnormalities are predominant symptoms of dystonia, patients also have alterations in temporal as well as spatial discrimination of cutaneous inputs. Here, we use a recently described method to test whether deficits also exist in temporal discrimination of proprioceptive inputs. Percutaneous electrical stimulation of the motor point of the first dorsal interosseous (FDI) muscle, and of the flexor carpii radialis (FCR) muscle separately, was used to produce a nonpainful contraction of the muscles that caused index finger abduction and wrist flexion, respectively. In 10 patient

The effect of feeding fermented distillers' grains diet on the intestinal metabolic profile of Guanling crossbred cattle.

Fermented distiller's grains (FDG)-based diets are nutritious and can improve the growth and intestinal immunity in livestock. However, there is limited research examining the effect of feeding FDG-based diets on changes in intestinal metabolites and related pathways in livestock. In this study, nine Guanling crossbred cattle (Guizhou Guanling Yellow cattle × Simmental cattle) were selected and randomly divided into a basal diet (BD) group and two experimental groups fed with FDG replacing 15% and 30% of the daily ration concentrates (FDG-Case A and FDG-Case B), respectively, with three cattle

Comparison of cardiac troponin assays reveals assay-specific sensitivities in a clinical model of very acute myocardial injury.

AIMS: High-sensitivity cardiac troponin (hs-cTn) assays indicating myocardial injury are critical for the diagnosis of acute myocardial infarction (AMI), but their use to differentiate between acute and chronic myocardial injury is limited. This study aimed to assess the differential utility of three hs-cTn assays, targeting different troponin epitopes (proximal and/or central), to detect myocardial injury in a clinical model of acute myocardial injury by left-atrial ablation. METHODS AND RESULTS: First, pre- and post-procedural serum samples were prospectively collected from 158 patients und
