The dilated coronary sinus: utility of coronary sinus cross-sectional area and eccentricity index in differentiating right atrial pressure overload from persistent left superior vena cava.

BACKGROUND: Pulmonary hypertension and right atrial pressure overload (RAPO) cause dilation of the coronary sinus (CS). Persistent connection of the left superior vena cava (LSVC) to the CS is another cause of CS dilation. The purpose of this study was to evaluate the usefulness of coronary sinus cross-sectional area (CSA) and eccentricity index (EI) in differentiating persistent LSVC from right heart overload and RAPO in patients with dilated CS. METHODS: We identified 15 patients with a dilated CS by echocardiography. Offline analysis was used to measure CS-CSA and CS-EI at end-diastole in

Ortner's syndrome, presentation of two cases with cardiovocal hoarseness.

We present two cases of Ortner's syndrome, a syndrome with hoarseness caused by unilateral left recurrent laryngeal nerve palsy as a complication of cardiovascular diseases. In the first case, aneurysmatic dilatation of the aorta and in the second case a dilated pulmonary trunk was thought to be the cause.

[Ventriculus terminalis dilatation in adults: A case report and review of the literature].

The ventriculus terminalis (VT) is a virtual cavity of the conus medullaris that appears during embryonic life. The occurrence of its dilatation is very rare in adulthood. Only 27 cases have been documented so far. We present the case of a 47-year-old woman who presented with hypoesthesia and dysesthesia of the lower limb, distal paresis, and urinary dysfunction evolving for the past 5 years. Electrophysiology confirmed radiculopathy. MRI revealed a cystic dilatation of the VT. Surgery consisted in laminectomy with myelotomy and fenestration. Histology showed a simple cubic ependymal epitheliu

Recurrent acute pancreatitis due to a santorinicele in a young patient.

A cystic dilatation of the terminal portion of the minor pancreatic duct (duct of Santorini) is referred to as a santorinicele. It is usually associated with pancreas divisum and has been suggested to be a cause of relative stenosis of the minor papilla, often leading to recurrent pancreatitis. While this anomaly has been reported in the paediatric population, it is more commonly found in the elderly. We present a 27-year-old woman with recurrent acute pancreatitis attributed to a santorinicele with a dorsal duct-exclusive pancreatic drainage.

Macroscopic pancreatic heterotopia on a congenital biliary dilatation.

Pancreatic heterotopia is generally asymptomatic and occurs in gut-related sites. An antenatally detected subhepatic cyst was finally diagnosed to be a congenital biliary cystic dilatation (without a caudal duodenal communication) with macroscopic Heinrich Type 2 pancreatic heterotopia. The embryogenesis and pathology of the malformation are discussed.

Surgical repair of an oesophageal stricture and prestenotic dilatation in a horse.

Prestenotic oesophageal dilatation and stricture were diagnosed in a horse with a history of trauma in the cervical area and recurrent oesophageal obstruction. The diagnosis was established on the basis of the clinical examination, endoscopy, and positive contrast oesophagography. Conservative treatment was unsuccessful and surgical intervention was necessary. Oesophageal myectomy combined with myoplasty was performed, simultaneously relieving the dilatation and the stricture. No abnormalities were detected 8 weeks after surgery. The horse was on a normal diet and oesophageal obstruction did r

Transient headache related to enlargement of the contralateral vertebral artery after vertebral artery occlusion.

BACKGROUND: Endovascular proximal parent artery occlusion has been performed for VA dissection. We describe a case that presented with headache associated with transient enlargement of contralateral VA after VA occlusion. CASE DESCRIPTION: A 54-year-old man presented with constant occipital headache on the left due to left VA dissection. Proximal parent artery occlusion of the left VA with detachable coils was performed, and the headache disappeared after coil occlusion. However, the patient presented with occipital headache on the right 1 week later. The MRI showed enlargement of the right V

Characteristics of ocular higher-order aberrations in patients with pellucid marginal corneal degeneration.

PURPOSE: To evaluate the characteristics of higher-order aberrations (HOAs) in eyes with pellucid marginal corneal degeneration (PMCD). SETTING: Department of Ophthalmology, Osaka University Medical School, Osaka, Japan. METHODS: Ocular HOAs were measured by aberrometry in 20 eyes with PMCD (PMCD group), 76 eyes with keratoconus (KC group), and 105 normal eyes (control group) with a 4.0 mm pupil. The magnitudes and axes of trefoil, coma, tetrafoil, and secondary astigmatism and the magnitude of spherical aberration were compared between the 3 groups using vector analysis of Zernike terms. R

Thoracic outlet syndrome with right subclavian artery dilatation in a child - transaxillary resection of the pediatric cervical rib.

Cervical rib is a well-documented congenital anomaly, and thoracic outlet syndrome (TOS) with vascular compression is extremely rare in childhood. The subclavian artery, subclavian vein, brachial plexus, or a combination of these can be affected in this syndrome. We present a case of TOS in an eight-year-old boy and describe the rare finding of subclavian artery compression with post-stenotic dilatation and severe brachial plexus compression symptoms. The patient underwent right transaxillary resection of the first thoracic and cervical rib. The presentation is unique and may be elusive.

Keratectasia as a presentation of surgically induced necrotizing sclerokeratitis (SINS).

PURPOSE: A case of keratectasia is reported as a severe presentation of surgically induced necrotizing sclerokeratitis (SINS). METHODS: A 72-year-old white woman had a painful, raised lesion on the superior cornea of her right eye 3 years after uncomplicated extracapsular cataract extraction with intraocular lens implantation. Examination showed a large ectatic area of the superior cornea with inflamed sclera adjacent to the surgical wound, which was diagnosed as SINS. RESULTS: No underlying systemic autoimmune condition or vasculitis was identified on investigation. Progressive painful kera

Hepatic fibrosis and cirrhosis in the Fontan circulation: a detailed morphological study.

AIMS: To describe the histological features of the liver in patients with a Fontan circulation. METHODS: Specimens from liver biopsies carried out as part of preoperative assessment prior to extracardiac cavopulmonary conversion of an older style Fontan were examined and scored semi-quantitatively for pertinent histological features. To support the use of the scoring, biopsy specimens were also ranked by eye for severity to allow correlation with assigned scores. RESULTS: Liver biopsy specimens from 18 patients with a Fontan circulation were assessed. All specimens showed sinusoidal fibrosis

A case of santorinicele without pancreas divisum: diagnosis with multi-detector row computed tomography.

A santorinicele is defined as a focal cystic dilatation of the terminal portion of the dorsal pancreatic duct at the minor papilla. Most cases reported previously were associated with pancreas divisum and a santorinicele without pancreas divisum is known to be rare. We recently experienced a typical case of a santorinicele without pancreas divisum in a 67-yr-old woman with abdominal pain and hematochezia, subsequently proven to be the result of an ischemic colitis. The santorinicele was diagnosed incidentally with multi-detector row computed tomography using a minimum intensity projection tech

A new surgical technique for anterior segment ectasia: tectonic lamellar sclerokeratoplasty.

A 41-year-old patient with severe anterior segment ectasia and a previous history of failed corneal graft was treated by placing a 14-mm tectonic corneoscleral allograft in the eye to support both the corneal and the scleral thinning. Nylon 10-0 interrupted sutures were used at the sclero-scleral junction. The host conjunctiva, including the limbal area, was carefully sutured to the donor limbal area. Three months postoperatively, the graft was stable with no progression of the ectasia. The suturing of a corneoscleral graft over a severely ectatic cornea may be an acceptable technique for prov

[Bilateral internal laryngocele with open nasality--report of a case].

Laryngocele is an abnormal saccule dilatation of the ventricle in direct communication with the laryngeal lumen. It is usually filled by air and mostly asymptomatic. This case is reported due to its rarity. A bilateral internal laryngocele with open nasality in 66-year-old man is described. Patient has been observed for 8 years with stable course of the disease and good general condition. Because the squamous cell carcinoma is known to occur in association with laryngocele necessity of control evaluation is very important. The etiology, classification, evaluation is discussed.
