Risk of acute brain lesions in dizzy patients presenting to the emergency room: who needs imaging and who does not?

The usefulness of brain imaging studies in dizzy patients presenting to the emergency department (ED) is controversial. We aimed to assess the 'real-world' probability of ischemic stroke and other acute brain lesions (ABLs) in these patients to create an algorithm that helps decision-making on whether which and when brain imaging is needed. By reviewing medical records, we identified 610 patients presenting with dizziness, vertigo or imbalance to our university hospital's ED and receiving neurological workup. We collected timing/triggers of symptoms, ABCD2 score, focal neurological abnormaliti…

Incidence of acute cerebral infarction or space occupying lesion among patients with isolated dizziness and the role of D-dimer.

BACKGROUND: To determine the incidence of acute cerebral infarction or space occupying lesion (SOL) among patients with isolated vertigo or dizziness (IVD) and to evaluate the role of cerebellar function test (CFT) and D-dimer to discriminate ACI/SOL and non-ACI/SOL. METHODS: A retrospective study of consecutive emergency department (ED) patients with IVD during one year was conducted. ACI was based on the diffusion-weighted magnetic resonance imaging (DW-MRI), and SOL was based on the concurrent MRI sequences. A sensitivity analysis of CFT and D-dimer was also performed. RESULTS: Among the …

The effectiveness of anterior cervical decompression and fusion for the relief of dizziness in patients with cervical spondylosis: a multicentre prospective cohort study.

AIMS: Cervical spondylosis is often accompanied by dizziness. It has recently been shown that the ingrowth of Ruffini corpuscles into diseased cervical discs may be related to cervicogenic dizziness. In order to evaluate whether cervicogenic dizziness stems from the diseased cervical disc, we performed a prospective cohort study to assess the effectiveness of anterior cervical discectomy and fusion on the relief of dizziness. PATIENTS AND METHODS: Of 145 patients with cervical spondylosis and dizziness, 116 underwent anterior cervical decompression and fusion and 29 underwent conservative tre…

Impact of vertigo and dizziness on self-perceived participation and autonomy in older adults: results from the KORA-Age study.

PURPOSE: The impact of vertigo and dizziness on healthy ageing, and especially on participation, is not fully understood. The objective of this study was to investigate the association of vertigo and dizziness with self-perceived participation and autonomy in older non-institutionalised individuals, adjusted for the presence of other health conditions. Specifically, we wanted to investigate the different effects of vertigo and dizziness on specific components of participation, i.e. restrictions in indoor and outdoor autonomy, family role, social life and relationships, and work and education. …

Diagnostic yield of automatic and patient-triggered ambulatory cardiac event recording in the evaluation of patients with palpitations, dizziness, or syncope.

BACKGROUND: Recent studies have shown that patient-triggered cardiac event recorders (CER) have an increased diagnostic yield and are more cost effective than conventional 24-h-Holter electrocardiograms (ECGs) for the evaluation of sporadic, potentially arrhythmia-related symptoms. HYPOTHESIS: The aim of this study was to determine the diagnostic yield of a patient-triggered CER combined with continuous automatic arrhythmia detection in the evaluation of sporadic dizziness/syncope or palpitations and its clinical relevance in assessing the further management. METHODS: We investigated 101 con…

Intravenous dextrose during emergence from deep sedation reduces postprocedural dizziness in adults undergoing gastrointestinal endoscopy: a randomized controlled trial.

BACKGROUND AND AIMS: Dizziness is a central symptom of postoperative fatigue syndrome after gastrointestinal endoscopy under deep sedation, affecting the quality of postoperative recovery, elevating health care costs, and increasing the risk of falls. Preliminary studies suggest that dextrose supplementation may provide symptomatic relief, but evidence for optimal dosage and safety remains limited. This randomized controlled trial aimed to assess whether intravenous 5% dextrose during emergence from deep sedation could reduce postprocedural dizziness in adults undergoing gastrointestinal endos…

Postural stability and handicap of dizziness after preoperative vestibular ablation and vestibular prehabilitation in patients undergoing vestibular schwannoma resection.

BACKGROUND: Surgical treatment of vestibular schwannoma (VS) leads to acute ipsilateral vestibular loss if there is residual vestibular function before surgery. To overcome the sequelae of acute ipsilateral vestibular loss and to decrease postoperative recovery time, the concept of preemptive vestibular ablation with gentamicin and vestibular prehabilitation before surgery has been developed ("vestibular prehab"). OBJECTIVE: Studying postural stability during walking and handicap of dizziness over a 1-year follow-up period in VS patients undergoing vestibular prehab before surgical treatment …

The synergistic interaction of vitamin D deficiency and insomnia on dizziness-related handicap in patients with benign paroxysmal positional vertigo.

BACKGROUND: While insomnia and vitamin D deficiency are known risk factors for BPPV, their interactive effect on the dizziness-related handicap is unknown. Given that both may independently promote pro-inflammatory states, a synergistic interaction is biologically plausible. Therefore, this study aimed to investigate both the independent and interactive effects of insomnia and 25-OH-D levels on the dizziness-related handicap in BPPV patients. We specifically hypothesized that insomnia amplifies the detrimental impact of low vitamin D, aiming to provide an empirical basis for integrated clinica…
