Oxygen persufflation as adjunct in liver preservation (OPAL): study protocol for a randomized controlled trial.

BACKGROUND: Early graft dysfunction due to preservation/reperfusion injury represents a dramatic event after liver transplantation. Enhancement of donor organ criteria, in order to cope with the ever increasing donor shortage, further increases graft susceptibility to ischemic alterations. Major parts of post-preservation injury, however, occur at the time of warm reperfusion but not during ischemic storage; successful reperfusion of ischemic tissue in turn depends on an adequate redox and intracellular signal homeostasis. The latter has been shown experimentally to be favorably influenced by

Cancer therapy and risk of congenital malformations in children fathered by men treated for testicular germ-cell cancer: A nationwide register study.

BACKGROUND: Because of the potential mutagenic effects of chemo- and radiotherapy, there is concern regarding increased risk of congenital malformations (CMs) among children of fathers with cancer. Previous register studies indicate increased CM risk among children conceived after paternal cancer but lack data on oncological treatment. Increased CM risk was recently reported in children born before paternal cancer. This study aims to investigate whether anti-neoplastic treatment for testicular germ-cell cancer (TGCC) implies additional CM risk. METHODS AND FINDINGS: In this nationwide registe

Prenatal and postnatal maternal contributions to reproductive, maternal, and size-related traits of beef cattle.

Brown Swiss-Hereford (BS-H) reciprocal cross embryos were transferred to BS and H recipient cows and Red Poll-Angus (RP-A) reciprocal cross embryos were transferred to RP and A recipient cows to estimate the relative contributions of ovum cytoplasm and uterine influences to prenatal maternal effects. Calves resulting from embryo transfers (ET) were weaned early (3 to 5 d). Reciprocal cross mating also were made by natural service (NS) between BS and H and between RP and A breeds; part of the offspring were weaned at 3 to 5 d, and the remainder nursed their dams to an age of 150 to 180 d. Intracellular Crosstalk of the Gasotransmitter Trio (NO, CO, H2S) in Cardiovascular Health and Disease: From Molecular Signaling to Precision Gas Medicine.

Nitric oxide (NO), carbon monoxide (CO), and hydrogen sulfide (H2S) were once regarded solely as toxic environmental gases. However, accumulating evidence over the past several decades has established them as the three principal endogenous gasotransmitters that regulate a wide spectrum of physiological and pathological processes. Unlike conventional signaling molecules, gasotransmitters diffuse freely across biological membranes and exert potent biological effects through receptor-independent mechanisms, including redox-sensitive post-translational modifications and modulation of heme-containi

Use of artificial intelligence as an innovative donor-recipient matching model for liver transplantation: results from a multicenter Spanish study.

BACKGROUND & AIMS: There is an increasing discrepancy between the number of potential liver graft recipients and the number of organs available. Organ allocation should follow the concept of benefit of survival, avoiding human-innate subjectivity. The aim of this study is to use artificial-neural-networks (ANNs) for donor-recipient (D-R) matching in liver transplantation (LT) and to compare its accuracy with validated scores (MELD, D-MELD, DRI, P-SOFT, SOFT, and BAR) of graft survival. METHODS: 64 donor and recipient variables from a set of 1003 LTs from a multicenter study including 11 Spani

An exploratory single arm study to evaluate the role of an Ayurvedic treatment protocol as a prerequisite for in vitro fertilization in women with diminished ovarian reserve incorporating multi-omics approaches: study protocol.

INTRODUCTION: Ovarian reserve depicts the quality and quantity of oocytes remaining in the ovaries and gives an idea about ovarian function at a given time. Because of aging, ovarian reserve diminish physiologically. However, many women face a non-physiologic reduction of ovarian reserve, irrespective of age. To date, standard-of-care treatment options are not available to treat diminished ovarian reserve (DOR). Studies shows that DOR patients mostly undergo in vitro fertilization (IVF) with donor cycles. Hence, identifying effective treatment modalities for DOR is an area of great clinical re

Reproductive issues from the Islamic perspective.

The Islamic faith is regarded by its followers, Muslims, as a complete way of life. A multitude of nationalities practise Islam and also various sects, and as a result there are various interpretations of Qur'anic guidance relating to almost every matter. Only a fully qualified jurist of the highest rank can issue edicts on problems that are not already clearly addressed in the Qur'an. This applies to contemporary issues and any Muslim is at liberty to debate and dialogue with the religious leader to obtain a ruling on a specific question. Marriage is described as half the faith in Islam and t

Organ Donation in the Unrepresented Patient-Ethical Challenges and Opportunities for Improvement.

In the United States, if an individual not on the donor registry dies by neurologic criteria (DNC), has no documented advance directives, and has no identified next of kin-so called unbefriended patients-current law permits hospital administrators to authorize organ donation on behalf of these patients in a process known as administrative authorization. On the one hand, organ transplantation is a public good that improves human life, and demand for organs vastly outstrips current supply; procurement from unrepresented individuals could help to increase the pool of available organs. On the othe

Bridging the gap between basic research and commercial application at scale for in vitro bovine embryo production.

The rapid expansion of in vitro embryo production (IVP) in cattle has established this technology as a major driver of genetic progress and production efficiency. Despite widespread adoption, IVP systems remain inherently inefficient, with substantial losses occurring from oocyte to blastocyst and variable outcomes following transfer. At the same time, significant advances in the understanding of oocyte competence and embryo biology have occurred. However, this knowledge has not consistently translated into improvements in commercial embryo production. The purpose of this review is to examine

Cell-Based Therapies for Type 1 Diabetes: A Narrative Review of Stem Cell-Derived Islets, Encapsulation Strategies, and Recent Clinical Trials.

Type 1 diabetes (T1D) is an autoimmune disease characterized by the destruction of insulin-producing pancreatic β-cells, necessitating lifelong insulin therapy. While islet transplantation has demonstrated efficacy in restoring glycemic control, donor scarcity and the requirement for chronic immunosuppression limit its widespread application. Recent advances in stem cell biology, bioengineering, and immunomodulation have catalyzed a paradigm shift toward stem cell-derived β-cell replacement therapies. This narrative review presents recent clinical trials and technological innovations in cell-b

Precise CRISPR-Mediated Editing of the TGFBI R555W Mutation in Patient-Derived Peripheral Blood Mononuclear Cells.

Over 70 mutations in the transforming growth factor beta-induced (TGFBI) gene are associated with corneal dystrophies that impair vision. The R555W hotspot mutation is a major cause of granular corneal dystrophy type 1 (GCD1). Here, we evaluated the technical feasibility of CRISPR/Cas9-mediated editing of the R555W mutation in peripheral blood mononuclear cells (PBMCs) obtained from a patient with GCD1. Three single guide RNAs (sgRNA1-3) and matched single-stranded oligodeoxynucleotide donors (ssODN1-3) were designed and co-transfected into PBMCs. Transfected cells were enriched by flow cytome

Thermochemical Polarization-Driven Heterolytic Hydrogenation in an Electrochemical Palladium-Membrane Reactor.

Heterolytic hydrogenation via hydride species transfer represents a fundamental transformation in industry for chemical synthesis, but H2 or organic hydride donors are often utilized as the hydrogen source. An electrochemical palladium (Pd) membrane reactor shows the ability to catalyze heterolytic reactions by using water as the hydrogen source, but quantitative descriptions of this reactivity remain absent, and the reaction often relies on external polarization of the chemical compartment. Herein, we report thermochemical polarization-driven heterolytic hydrogenation in an electrochemical Pd

Mortality Estimation in Renal Disease (MERD Score): A Model Predicting Waitlist Mortality in Kidney Transplant Candidates.

BACKGROUND: Predicting waitlist mortality is important for prioritizing organ allocation and selecting candidates for extended criteria donors. Currently, there is no widely adopted and reliable index for predicting early mortality among kidney transplant candidates. In this study, we aim to develop an index score utilizing variables from the OPTN database to predict mortality among adult kidney transplant candidates within 3 years of being on the waitlist. METHODS: This study utilized data from 147,307 adult kidney transplant candidates listed in the OPTN database from 2018 to 2023. The coho

Clinical Translation of Mesenchymal Stem Cells to Medicinal Signaling Cells: Origins, Clinical Applications, and the Future of Regenerative Medicine-2026 Arnold I. Caplan Award for Distinguished Research in Orthobiologics.

Orthobiologics are used widely in modern orthopaedic and sports medicine practice, yet the biologic rationale for cell-based therapies is often oversimplified. Mesenchymal stem cells (MSCs) were initially understood as multipotent progenitors capable of regenerating mesenchymal tissues through engraftment and differentiation. However, continued translational research, including the isolation and characterization of muscle-derived stem cells, has refined this model. MSC-like tissues are closely associated with the perivascular microenvironment, and their clinical effect seems to be mediated lar

Recurrent Immunogenic Neoantigens and Their Cognate T-cell Receptors in Treatment-Resistant Metastatic Prostate Cancer.

UNLABELLED: New approaches that generate long-lasting therapeutic responses in patients with therapy-resistant metastatic cancer are urgently needed. To address this challenge, we developed Spot Neoantigens in Metastases (SpotNeoMet), a novel data-driven pipeline that systematically identifies recurrently presented neopeptides in treatment-resistant patients. We identified seven therapy resistance mutations predicted to produce neopeptides presented by common HLAs. Using HLA immunopeptidomics, we discovered three novel neopeptides derived from androgen receptor (AR) H875Y, a common metastatic

Strategic Acceleration of Reverse Intersystem Crossing in Multi-Resonance TADF Emitters.

Advanced multi-resonance-induced thermally activated delayed fluorescence (MR-TADF) materials offer intrinsically narrowband emissions and excellent luminescent efficiencies, making them promising emitters for next-generation organic light-emitting diodes (OLEDs). However, their development remains hindered by slow reverse intersystem crossing (RISC) rates, which cause severe efficiency roll-off at high luminance and limit their application in high-performance OLEDs. Here, we propose an effective approach for designing blue MR-TADF molecules by integrating a crumpled and asymmetric heptagonal

Nutrient-Sensitive Epigenetic Modifiers as Candidate Biomarkers of Metabolic Dysfunction in Obesity: A Nutrigenomic Review.

Obesity is a complex metabolic disorder resulting from interactions among genetic, environmental, and dietary factors. Traditional clinical markers may provide limited insight into the biochemical mechanisms that link nutrition and metabolic dysfunction; in this context, the epigenetic mechanisms through which nutrients modulate gene expression are central to understanding metabolic homeostasis. This review summarizes the published evidence on nutrient-driven epigenetic processes in obesity, focusing on DNA methyl donors, such as folate, vitamin B12, choline, betaine, serine, and methionine, a

The Tela Cutis Nasi Flap: A Technical Note on Nasal Sill Reconstruction in Secondary Cleft Rhinoplasty.

Background: Despite advances in cleft rhinoplasty, the nasal sill remains an underappreciated subunit, yet it plays a crucial role in nostril symmetry and aesthetic balance. Hypoplasia or absence of the nasal sill on the cleft side often persists after primary repair and may complicate secondary nasal base correction. Current methods for sill reconstruction are limited by donor site morbidity, variability in tissue match, or non-anatomic tissue substitution. Methods: This technical note describes a surgical protocol utilizing the Tela Cutis Nasi flap, a pedicled fibro-adipose flap harvested fr

CRISPR-Cas9-based gene editing as a proof-of-concept approach in an inborn error of immunity caused by a DCLRE1C variant.

Hypomorphic DCLRE1C variants impair T and B cell development, leading to combined immunodeficiency (CID) or leaky severe combined immunodeficiency (SCID). Current treatment options, such as allogeneic hematopoietic stem cell transplantation (aHSCT), are associated with significant risks, highlighting the need for alternative therapeutic strategies. In this study, we report the first a proof-of-concept CRISPR-Cas9–mediated correction of a hypomorphic DCLRE1C variant (c.194 C > T; p.T65I) in CD4 + helper T (Th) cells using CRISPR-Cas9 gene-editing technology.
