Movement of the external ear in human embryo.

INTRODUCTION: External ears, one of the major face components, show an interesting movement during craniofacial morphogenesis in human embryo. The present study was performed to see if movement of the external ears in a human embryo could be explained by differential growth. METHODS: In all, 171 samples between Carnegie stage (CS) 17 and CS 23 were selected from MR image datasets of human embryos obtained from the Kyoto Collection of Human Embryos. The three-dimensional absolute position of 13 representative anatomical landmarks, including external and internal ears, from MRI data was traced …

Pseudocyst of the auricle: an uncommon entity of the ear.

A 48-year-old white man presented with a 2-month history of a lump on his right ear following purported exposure to the cold. The patient denied any history of trauma, rubbing, insect bite, or inflammation at the site. The patient was otherwise healthy and was not taking any medications. The lesion was completely asymptomatic, as the patient denied any pain, tenderness, or change in size since the lesion appeared. Physical examination revealed a 1.1-cm x 1-cm skin-colored, slightly fluctuant, rubbery, nonmobile dermal nodule on the right scaphoid fossa (Figure 1). No erythema, warmth, tenderne…

Classification and reconstruction of posttraumatic ear deformity.

The diversity of posttraumatic auricular deformity caused by various factors often makes reconstruction exacting for the surgeon. During the past 10 years (from 2001 to 2010), a total of 60 patients (75 ears) were treated. The etiology of the deformities included burns (n = 35), traffic accidents (n = 10), cuts during fight (n = 8), and human bite injuries (n = 7). Based on previously published ones, we set a classification taking into consideration the involved tissue components, size of defects, and status of surrounding soft tissues, which categorized posttraumatic auricular deformities int…

Gotch ear in a goat: a case report.

A 1-year-old castrated male Saanen goat was observed to have drooping and edema of the left ear consistent with published accounts of gotch ear in cattle associated with a tick bite. The goat's left ear was edematous from the tip of the pinna to the base of the ear. No signs of trauma or infectious processes were observed. Three engorged Gulf Coast ticks (Amblyomma maculatum) were observed attached inside the ear. Ticks were removed and the ear biopsied at tick attachment sites. The affected ear was treated topically with betadine after removal of the ticks. No other treatment was administered…

[Functional examinations of the ear and auditory pathway].

Hearing is usually examined by means of pure tone and speech audiometry. Alternative examinations are required when the level of hearing defect needs to be more closely defined or hearing is being measured from a patient who is unable to give reliable feedback on a test sound. Neonatal hearing screening involves measuring of otoacoustic emissions generated in the cochlea of the inner ear or brain stem responses formed within the auditory pathway. Immittance measurements are used to evaluate the function of the middle ear. Most commonly used methods include tympanometry probing the mobility of …

A 2-stage ear reconstruction for microtia.

OBJECTIVE: To introduce our 2-stage reconstruction of microtia method, which results in a natural-looking contour of the reconstructed ears, one of the most demanding challenges in facial plastic surgery. METHODS: In the first stage, the 3-dimensional cartilage framework is fabricated. The skin flap and retroauricular fascial flap are elevated in the mastoid area. Then the framework is wrapped by the fascial flap from behind and covered by the skin flap from front. In the second stage the crus, the tragus, and the conchal cavity are reconstructed. So almost all of the fine structures of ear a…

A novel algorithm for autologous ear reconstruction.

Sculpting a tridimensional autologous rib cartilage framework is essential to restore a natural ear shape and becomes routine with preoperative training, but management of the skin is the key to minimizing complications. Here the authors provide a classification scheme to manage auricular skin: Type 1 is a Z-plasty with transposition of the lobule; type 2 is a transfixion incision of the microtic ear; type 3 exposes the cartilage remnants through a cutaneous incision. They also explain how to choose between the three types, depending upon the auricular skin potential. With training and method,…

The effect of noise on ears with a hole in the vestibule.

CONCLUSION: A hole in the vestibule of the inner ear leads to augmentation of the hearing loss following noise exposure. Further research is needed to ascertain the mechanism. OBJECTIVES: The possible effects of a hole in the wall of the inner ear at the vestibule on noise-induced hearing loss were assessed. METHODS: The study was conducted on a total of 14 sand rats. Of these 14 animals, 10 underwent baseline auditory nerve and brainstem evoked response (ABR) threshold measurements in each ear separately (insert earphone), opening of middle ear bulla in both ears and drilling of a hole in t…

Management of auricular hematoma and the cauliflower ear.

Acute auricular hematoma is common after blunt trauma to the side of the head. A network of vessels provides a rich blood supply to the ear, and the ear cartilage receives its nutrients from the overlying perichondrium. Prompt management of hematoma includes drainage and prevention of reaccumulation. If left untreated, an auricular hematoma can result in complications such as perichondritis, infection, and necrosis. Cauliflower ear may result from long-standing loss of blood supply to the ear cartilage and formation of neocartilage from disrupted perichondrium. Management of cauliflower ear in…

[Carcinoid of the middle ear. A case report].

The case of a 47-year-old patient is described with a carcinoid of the middle ear. Initial symptoms were hearing impairment, feeling of pressure and dizziness. Mastoidectomy was carried out for mastoid shadowing with space encroachment in the auditory canal. The histological examination initially revealed an atypical cholesteatoma and the subsequent immunohistochemical investigation revealed a carcinoid. A radical excavation with complete excision of the tumor and tympanoplasty was carried out. A carcinoid of the middle ear is definitely a rare finding and the primary treatment is complete sur…

The kidney and ear: emerging parallel functions.

The association between renal dysplasia and minor malformations of the external ear is weak. However, there is a remarkable list of syndromes that link the kidney to the inner ear. To organize these seemingly disparate syndromes, we cluster representative examples into three groups: (a) syndromes that share pathways regulating development; (b) syndromes involving dysfunction of the primary cilium, which normally provides critical information to epithelial cells about the fluid in which they are bathed; (c) syndromes arising from dysfunction of specialized proteins that transport ions and drugs…

Microsurgical replantation of a partial helix of the ear.

Microsurgical ear replantation is a significant challenge because vessel sizes are diminutive. Furthermore, as ear vessels are larger in the medial portion than in the lateral portion, microsurgical replantation of a replant including only the helix is extremely difficult. The authors replanted a partial helix of the ear using a single arterial anastomosis. As no suitable veins could be identified, medicinal leech therapy and systemic heparinization were used to achieve venous drainage. The replanted helix survived completely and the cosmetic result was excellent. Microsurgical replantation sh…

Minimizing the dead ear in otosclerosis surgery.

OBJECTIVE: Assessment of a specific surgical technique regarding the postoperative hearing results and the incidence of "dead ear" comparing local and general anesthesia. METHODS: We present a retrospective review of all stapedectomies performed by the same surgeon under general and local anesthesia during a 9-year period (1997-2006) in non-revision cases. The analysis of our data (268 operations, 160 under general and 108 under local anesthesia) included hearing results based on the guidelines from the Committee on Hearing and Equilibrium of the American Academy of Otolaryngology-Head and Ne…

[Glomus tumors of the middle ear in children].

Glomus tumor (GT) is a rare tumor of the middle ear in children. In spite of cytologically benign nature of this tumor, its course can be characterized as malignant because of GT infiltrative growth, destruction of the adjacent bone tissue, possible intracranial involvement. Because of scare symptoms, early GT diagnosis is problematic. At admission to hospital principal complaints of the children are poor hearing, discharge from the ear. These often underlie misdiagnosis -- acute otitis media. GT of the middle ear are detected with application of computed tomography which is able to show chang…
