[Hinged Elbow External Fixation for Severe Elbow Stiffness].

PURPOSE OF THE STUDY The elbow is a highly constrained synovial hinge joint that frequently becomes stiff after injury. Grade 4 elbow stiffness (ankylosis) is a strongly limiting factor in the patient s daily activities and is difficult to treat. Hinged external fixation of the elbow provides the advantages of static fixation with the benefits of continued motion of the joint. The goal of our study was to assess the improvement in the range of motion of the elbow after surgical arthrolysis using a posterior extensile approach to the joint with or without using external fixation. Radial Head Fractures Treated with Modular Metallic Radial Head Replacement: Outcomes at a Mean Follow-up of Eight Years.

BACKGROUND: Radial head arthroplasty is commonly used to treat acute unreconstructible radial head fractures. The purpose of this study was to report on the clinical and radiographic outcomes at a minimum follow-up of five years after radial head arthroplasty with a modular metallic implant for the treatment of acute radial head fractures. METHODS: The cases of fifty-five patients with unreconstructible radial head fractures treated acutely with a smooth-stemmed modular metallic radial head implant were retrospectively reviewed. A wide variety of injuries, which ranged from isolated radial he

Magnetic resonance imaging of lateral epicondylitis of the elbow with a 0.2-T dedicated system.

The purpose of this study was to evaluate MR imaging findings of the common extensor tendon in patients with lateral epicondylitis and asymptomatic volunteers studied on a 0.2-T dedicated system. In 23 patients (age range 29-58 years, mean age 47 years) with clinical symptoms of lateral epicondylitis MR imaging was performed using T1-, T2- and contrast-enhanced T1-weighted spin-echo sequences. In addition, the elbows of seven healthy volunteers (age range 22-29 years, mean age 25 years) and the symptom-free contralateral elbow of 11 of the 23 patients (age range 29-58 years, mean age 47 years)

Pediatric Musculoskeletal Trauma: Upper Limb.

Pediatric upper extremity injuries, acute and chronic due to overuse, are commonly encountered in the clinical setting. However, interpretation of imaging studies is often challenging in pediatric patients because of changing postnatal skeletal development and the broad spectrum of pathology that can potentially occur. This article discusses normal skeletal development and the pathomechanics of skeletal injuries in pediatric patients. It also outlines a framework for imaging as it pertains to the pediatric upper extremity, with specific emphasis on acute and chronic injuries to the shoulder, e

Acute trauma to the upper extremity: what to do and when to do it.

The management of acute trauma to the upper extremity includes the urgent treatment of injuries and the timing and choice of surgical stabilization and reconstruction. To evaluate and treat severe upper extremity trauma, the orthopaedic surgeon should understand the principles of emergency department and operating theater management of commonly seen traumatic injuries to the distal humerus, elbow, forearm, wrist, and hand. A review of the principles for treating these complex injuries, including principles of soft-tissue coverage, will aid surgeons in achieving the goal of providing optimal tr

A closed pelvic fracture from wheels-in-line skating.

Wheels-in-line skating is the fastest growing recreational sport in the United States. An estimated 105,000 in-line skating injuries received treatment in emergency departments in 1995. This article reports an unusual in-line skating injury: a closed pelvic fracture requiring hospitalization. More commonly encountered injuries are also reviewed. Although there is increasing evidence that in-line skating is associated with a significant number of injuries, protective gear remains greatly underused. It is prudent to promote the use of protective gear, which should include a helmet, wrist guards,

A survey of badminton injuries.

A Badminton Injury Questionnaire (BIQ) was developed to survey the type and frequency of injuries that are likely to occur from playing competitive badminton. Two hundred and thirty-one players, ranging from club players to international champions, completed the survey which indicated an injury incidence rate of .09 and .14 injuries per person per year for male and female badminton players respectively. Badminton participation resulted in relatively few injuries, most of which were cramps, blisters, strains and sprains of the lower extremities and a surprisingly low incidence of tennis elbow.

Postoperative Contralateral Facet Cyst after Endoscopic Unilateral Laminectomy for Bilateral Decompression: A Case Report.

INTRODUCTION: The terrible triad of the elbow (TTE) is a well-described entity characterized by a triad of elbow dislocation, fractures of the radial head, and coronoid. Historically, these fractures gave poor clinical outcomes, and hence, the name "terrible triad." There are many variants of TTE that may lead to poor prognosis if not adequately dealt with. One such variant is the association of TTE with olecranon fracture, which has not been adequately described in the literature. There have been just three papers (one case series and two case reports) on this variant of TTE, and the prognosi

Current Approach to the Management of Forearm and Elbow Dislocations in Children.

Pediatric orthopedic injuries are a common reason for presentation to the emergency department. This article sequentially discusses 2 important upper extremity injuries that require prompt management in the emergency department. Radial head subluxations are discussed with a focus on current evidence for imaging, reduction techniques, and follow-up. Elbow dislocations, although less common than radial head subluxations, are also addressed, highlighting imaging, reduction, immobilization, and follow-up recommendations.

Microscopic Magnetic Resonance Imaging Comparing Asymptomatic and Symptomatic Ulnar Collateral Ligament Injuries in Baseball Players.

BACKGROUND: The relationship between abnormalities of the ulnar collateral ligament (UCL) on magnetic resonance imaging (MRI) and elbow symptoms in baseball players remains unclear. PURPOSE/HYPOTHESIS: This study aimed to compare findings of the UCL on microscopic MRI between asymptomatic and symptomatic elbows in baseball players. We hypothesized that the MRI grade of UCL injuries would exhibit no correlation with medial elbow symptoms in baseball players. STUDY DESIGN: Cross-sectional study; Level of evidence, 3. METHODS: The study participants were skeletally mature baseball players who

Biomechanical study of protonation movement during the healing phase after elbow joint capsule and ligament trauma in rabbits.

PURPOSE: This study aimed to construct a forelimb contracture model in rabbits to mechanically quantify pronation-supination movements during the healing phase following joint capsule and ligament injuries. Additionally, a finite element model of the human elbow joint was developed to investigate the mechanical environment of the elbow joint during pronation-supination movements in the healing phase. METHODS: White rabbits were randomly assigned to either a control group (no injury) or an injury group (joint capsule and ligament injury). The injured forelimbs were immobilized for 2, 4, 6, and

Sports-related injuries of the biceps and triceps.

Biceps or triceps ruptures are rare but can cause a significant disability. Surgical repair has become the preferred method of treatment for the complete rupture, but the decision when to treat partial tears is less clear. Reconstruction of the tendon is the preferred method when patients have a delayed presentation.

Pitfalls in the radiologic evaluation of extremity trauma: Part I. The upper extremity.

Family physicians often are required to evaluate patients who present with acute skeletal trauma. The first of this two-part series discusses the features and evaluation of some commonly missed fractures and dislocations of the upper limb, excluding the hand. Dislocations of the sternoclavicular joint are infrequent and often missed. Clavicular fractures in adults usually are not hard to diagnose. Acromioclavicular joint dislocations represent about 10 percent of all dislocation injuries to the shoulder girdle. Forty percent of all dislocations occur at the glenohumeral joint. Scapular fractur

Diagnostic Musculoskeletal Ultrasound for the Evaluation of the Lateral Elbow: Implications for Rehabilitation Providers.

Musculoskeletal ultrasound (MSKUS) has emerged as a valuable diagnostic tool in the evaluation and management of lateral elbow pathologies. This imaging modality provides high-resolution, dynamic visualization of superficial soft tissue structures, making it particularly advantageous for assessing conditions such as lateral epicondylitis (tennis elbow), ligamentous injuries, radial tunnel syndrome, and other common disorders. This article reviews the utility of MSKUS for rehabilitation providers, focusing on its role in accurately identifying pathoanatomical changes, guiding treatment strategi

Suspected Radial Head Subluxation in Infants: The Need for Radiologic Evaluation.

Radial head subluxation ("pulled elbow" or "nursemaid's elbow") is a common pediatric condition in children aged 1 to 4 years, although it is an uncommon finding in nonambulatory infants. It is a clinical diagnosis, and further investigations such as x-ray are not routinely done. We present a case series of 2 nonambulatory infants with forearm fractures that were initially diagnosed as radial head subluxation. Current literature supports the use of x-ray in children with atypical or unclear history prior to an attempted pulled elbow reduction. We suggest that this recommendation should be exte

[Fixator with motion capacity and distraction arthrodiatasis in post-traumatic elbow stiffness].

Elbow stiffness may result from trauma, burns and head injuries. It is defined as a total range of motion of <100° with no relevant loss of forearm rotation. Of particular relevance is the flexion deficit. A detailed analysis regarding the development of the elbow stiffness is required together with an exact diagnosis in order to plan the surgical intervention. Closed distraction of the elbow joint as arthrodiatasis with an external fixator is described and evaluated. Adequate long-term results can be achieved with this technique, which reflects proper selection of patients as well as coordina

Pediatric posterior trans-olecranon fracture-dislocation of the elbow: a case report and review of literature.

OBJECTIVE: Dislocations of the elbow are not common in skeletally immature patients. Herein, we present a case report on a rare pediatric posterior trans-olecranon fracture dislocation of the elbow, a type of dislocation that has never been reported in children. We aim to discuss the injury mechanism and introduce our treatment approach for this specific condition. PATIENT: A ten-year-old girl presented with pain, swelling and deformity of the left elbow following a fall from a rocking chair. DIAGNOSES AND INTERVENTIONS: The complete injury history and detailed physical examination were reco

Design of a Smart Elbow Brace as a Home-Based Rehabilitation Device.

Poststroke and traumatic elbow injuries are the most common cause of elbow stiffness, which results in loss of functional range of motion (ROM). Various studies support early mobilization of the elbow joint after injury or after surgery to reduce risks of elbow stiffness development. After hospitalization, patients are required to follow a long-term rehabilitation program during home recovery. Still, most patients do not adhere to their clinical therapy schedule due to either rehabilitation cost, social obligations, negligence, or lack of inspiration. Moreover, the numbers of therapists and as…
