[Total Arch Replacement Using Bilateral Axillary Artery Perfusion].

OBJECTIVES: The selection of arterial cannulation site is an important decision to avoid cerebral complication for total arch replacement(TAR). We report the surgical outcome of TAR using bilateral axillary artery perfusion in our hospital. METHODS: Between January 2012 and June 2020, 97 patients who underwent elective TAR for atherosclerotic aneurysms were enrolled in this study. Among them, bilateral axillary artery perfusion was used for 81 patients, and frozen elephant trunk( FET) procedure were used for 34 patients. In the case of FET procedure, translocated TAR was performed with distal

The Management of the Aortic Arch in Type A Aortic Dissection: Replace, Repair with the AMDS, or Leave for Another Day?

OBJECTIVES: Acute type A aortic dissection (ATAAD) is a life-threatening condition that requires emergent surgical intervention. Numerous surgical approaches exist for ATAAD, and controversy remains regarding the optimal arch interventions for ATAAD patients. Aortic Arch Interventions: Approaches to ATAAD repair include hemiarch repair or extended arch repairs, including the hemiarch with a hybrid stent implantation, such as the AMDS hybrid Prosthesis, total arch replacement (TAR), and the use of an elephant trunk and frozen elephant trunk. While indications for each procedure exist, such as e

A new dissection-specific hybrid stent graft for patients with DeBakey type I aortic dissection.

OBJECTIVES: We present our first experience of using a new dissection-specific hybrid stent graft (SG) (the 'Soft Elephant Trunk') in the frozen elephant trunk technique, in patients with non-acute DeBakey type I aortic dissection. METHODS: Between September 2016 and February 2021, patients with non-acute DeBakey type I aortic dissection underwent the frozen elephant trunk procedure at our centre using 3 SG types: group Z used stiff, distal Z-shaped nitinol SGs (E-Vita, Medtronic); group R used a stiff, ring-shaped nitinol SG (Thoraflex); and group S used a soft, distal dissection-specific SG

Non-A non-B acute aortic dissection with entry tear in the aortic arch.

OBJECTIVES: Our aim was to describe the outcomes of the latest treatment options of acute non-A non-B aortic dissection involving an entry tear in the aortic arch. METHODS: Included were patients who presented between January 2001 and February 2020 with a non-A non-B aortic dissection involving the aortic arch but not the ascending aorta and with the most proximal entry tear located within the aortic arch between the innominate and left subclavian artery. Clinical data and operative details were retrieved from medical histories and surgical protocols. Preoperative, postoperative and follow-up

Early Results of a Novel Single-Stage Hybrid Aortic Arch Replacement Technique to Reduce Bypass and Circulatory Arrest Duration.

OBJECTIVE: Hybrid repair procedures of the aortic arch have been utilized to reduce surgical risks and apply this therapy to patients who would not traditionally be candidates for open surgical repair.  We present a variation on the frozen elephant trunk technique to further reduce cardiopulmonary bypass and circulatory arrest duration. METHODS: After initiation of cardiopulmonary bypass and during systemic cooling, a wire is advanced from the femoral artery into the aortic arch.  In the case of aortic dissection, intravascular ultrasound is used to confirm true lumen placement.  Under circul

The Calcified Barrier: A Rare Cause of a Distal Aortic Arch Stenosis.

BACKGROUND: Calcification of the aortic arch is common, but an intraluminal calcified plaque causing functional stenosis is rare. At the distal arch, it mimics aortic coarctation, with refractory upper-body hypertension and left ventricular pressure overload. CASE SUMMARY: A 61-year-old man presented with refractory hypertension despite 4 agents and recurrent hypertensive cardiac decompensation with an upper-to-lower-limb blood pressure gradient. Computed tomography angiography showed a high-grade calcified stenosis of the distal arch (peak gradient 70 mm Hg; left ventricular ejection fractio

Left axillary cannulation for acute type A aortic dissection.

The most commonly used arterial cannulation sites for type A aortic dissection are right axillary artery, femoral artery and both. Direct central aortic cannulation has also been reported. In rare cases, it is extremely difficult to choose an arterial cannulation site for type A aortic dissection due to involvement of the right axillary and both femoral arteries. Herein, we present a 39-year-old male with acute type A aortic dissection with involvement of the right axillary and both femoral arteries. Left axillary cannulation was made and selective cerebral perfusion was performed through dire

Brucella antibody seroprevalence in Antarctic seals (Arctocephalus gazella, Leptonychotes weddellii and Mirounga leonina).

Brucellosis is a worldwide infectious zoonotic disease caused by Gram-negative bacteria of the genus Brucella, and Brucella infections in marine mammals were first reported in 1994. A serosurvey investigating the presence of anti-Brucella antibodies in 3 Antarctic pinniped species was undertaken with a protein A/G indirect enzyme-linked immunosorbent assay (iELISA) and the Rose Bengal test (RBT). Serum samples from 33 Weddell seals Leptonychotes weddelli were analysed, and antibodies were detected in 8 individuals (24.2%) with the iELISA and in 21 (65.6%) with the RBT. We tested 48 southern el

Neural encoding of relative position.

Late ventral visual areas generally consist of cells having a significant degree of translation invariance. Such a "bag of features" representation is useful for the recognition of individual objects; however, it seems unable to explain our ability to parse a scene into multiple objects and to understand their spatial relationships. We review several schemes (e.g., global features and serial attention) for how to reconcile bag-of-features representation with our ability to understand relationships; we review structural description theories that, in contrast, suggest that a neural binding mecha

Appendages in zoo-housed animals: application of thermal imaging to study the thermal state of wildlife under human care.

The present review aims to discuss the application of infrared thermography as a non-invasive tool in wildlife species under human care to determine the role of uninsulated appendages such as horns, antlers, ossicones, casques, beak/bill, proboscis, and legs in animals' thermoregulation. Currently, thermal imaging in the ocular and auricular surfaces of zoo-housed species is used to evaluate the physical and mental state of animals. However, zoos house a wide diversity of animals with highly vascularized appendages, for which infrared thermography research is limited. This review will also des

Architecture of the cardiac transverse-axial tubular system across different mammalian species.

Cardiac excitation-contraction coupling relies on a pancellular network of regular cardiomyocyte surface membrane invaginations, termed the transverse-axial tubular system (TATS). The TATS is ubiquitously present in adult mammalian cardiomyocytes, enabling efficient structural and functional coupling of sarcolemma and intracellular Ca2+ stores. However TATS ultrastructural characteristics across species, and their relation to cardiomyocyte morphology and physiological parameters such as heart rate, remain largely unexplored. Here we quantified TATS and cardiomyocyte features in a confocal micr

Application of insects to wounds of self and others by chimpanzees in the wild.

Self-medication refers to the process by which a host suppresses or prevents the deleterious effects of parasitism and other causes of illness via behavioural means1. It has been observed across multiple animal taxa (e.g. bears, elephants, moths, starlings)2, with many case studies in great apes1,3. Although the majority of studies on self-medication in non-human primates concern the ingestion of plant parts or non-nutritional substances to combat or control intestinal parasites4, more recent examples also report topical applications of leaves or other materials (including arthropods) to skin …

Total aortic replacement in a patient with mega aorta syndrome--a case report.

A 64-year-old woman without manifestations of Marfan syndrome was referred for the close investigation of a pulsatile abdominal mass. Computed tomographic scans and magnetic resonance images revealed aneurysmal dilatation of the entire aorta, including the ascending aorta and extending to the bifurcation of the aorta, as well as tortuousness of the thoracoabdominal and abdominal aorta. Digital subtraction angiography also showed aneurysmal dilatation of the entire aorta and trivial aortic regurgitation. However, aortic annular dilatation was not found by echocardiography or aortography. The en

Challenging Proximal Control for Open Thoracoabdominal Repair.

Proximal control of the thoracic aorta during the open repair of thoracoabdominal aorta can be challenging. Various techniques have been developed to address these challenges, including the use of deep hypothermic circulatory arrest and staged procedures, such as the conventional as well as frozen elephant trunk procedures. This paper is a brief review of the challenges and rationale behind some approaches.
