Ultrasound assessment of congenital spinal anomalies presenting in infancy.

Sonography of the infant spine can rapidly and safely exclude or confirm the presence of a congenital malformation. This article reviews the embryology of the spinal cord, abnormal and normal sonographic findings, and findings associated with occult dysraphic lesions. Patient selection for sonographic assessment is also discussed. Illustrations of normal anatomy and the more common dysraphic lesions are provided.

Nasolacrimal abnormalities in oblique facial clefts.

A patient with a Tessier No. 4 cleft and an associated bifid nasolacrimal system is presented and considered within the Tessier classification of craniofacial clefting. Previous accounts of nasolacrimal abnormalities associated with facial clefts are reviewed together with the accepted developmental embryology. It is proposed that the bifid nasolacrimal duct system described constitutes a new addition to the literature.

[Associated abnormalities in the VACTERL syndrome--case report with autopsy findings].

We report on a male newborn with a VACTERL-association. Our patient had the characteristics of the syndrome: vertebral defects, anal atresia, cardiac anomalies, tracheal stenosis and radial dysplasia. In addition to this combination of malformations we have found a left pulmonary artery sling, cholecystaplasia and biliary dysplasia-not previously reported. Etiology, embryology, coincidence and differential diagnosis are discussed.

Orthotopic heart transplantation in patients with persistent left superior vena cava and agenesis of left brachiocephalic vein.

Persistent left superior vena cava (PLSVC) is an infrequent abnormality. Anatomical variations can be unexpectedly identified during cardiac procedures. Modifications of surgical techniques become a must during these operations, especially during orthotopic heart transplantation. We present two cases of patients with PLSVC and discuss the anatomy, embryology, diagnosis, and modifications in transplant techniques.

Gastric diverticulum mimicking cystic lesion in left adrenal gland.

Adrenal pseudotumors can have many origins. We report the case of a gastric subcardial diverticulum misdiagnosed as a left adrenal cystic lesion on magnetic resonance imaging. A retrospective study of a previous computed tomography scan detected a previously unnoticed gastric diverticulum. The embryology, clinical aspects, and explanation of the misdiagnosis are exposed.

[Acute suppurative thyroiditis secondary to fistula of the pyriform sinus].

Acute suppurative thyroiditis is very rare and often related to congenital abnormalities of the branchial pouches or immunodeficiency. A case of acute thyroiditis due to fistula of the left pyriform sinus in a young man with no relevant history is reported. We review the embryology, diagnosis, and treatment of this rare entity.

Neonatal circumcision.

Circumcision remains the most common operation performed in males in the United States. When performed by an experienced operator, circumcision is usually a safe and simple operation. The medical benefits of circumcision appear to exceed the risks of the procedure. The history, embryology, indications, techniques, and complications of neonatal circumcision are discussed.

Anterior segment dysgenesis keratolenticular adhesion and aniridia.

Three additional cases of keratolenticular adhesion and aniridia have been reported. A review of the embryology and terminology in the description of complex lesion of the anterior segment might suggest the more general phrase, anterior segment dysgenesis, to have advantage. Finally, the simultaneous occurrence of anterior segment anomalies and hemophilia is reported.

An unusual lateral neck cyst with the combined features of a bronchogenic, thyroglossal, and branchial cleft origin.

Soft tissue cystic masses arising in the neck in children are commonly of embryologic origin and often surgically excised. Depending on location and histology, they can be classified as thyroglossal duct, branchial cleft, or rarely bronchogenic in origin. We present an unusual case of an infant with a lateral neck mass that communicates with the midtrachea. The histologic appearance is that of a combination of the features seen in cysts of branchial cleft and thyroglossal duct origin. The possible embryologic development of such a lesion is discussed.

[Ivemark's syndrome (author's transl)].

Following a case report of a newborn with Ivemark's syndrome, we go into etiology, embryology, and anatomy of this congenital cardiovascular disease with asplenia and other visceral abnormalities. The cause of this complex disorder is an exogen or endogen factor active between the 28th and 32nd day of embryological development. Lethality with or without operation is high, therefore clinical diagnosis should possibly be made early by X-ray of chest and abdomen, blood smear, ultrasound, and angiocardiographic investigations.

Can the conus medullaris in normal position be tethered?

Many have equated the term tethered spinal cord to a conus that is pathologically elongated. Most authorities have declared radiologic coni located inferior to the L-2 as pathologic. However, we have described previously a small series of patients in whom symptoms of a tethered spinal cord were evident clinically yet radiologically the conus medullaris was found to lie at a widely acceptable normal anatomic site. Following a review of the extant literature prior to our publication in 1993, several authors had operated on patients for symptoms of a distally tethered spinal cord in whom a conus

Imaging spectrum of spinal dysraphism: A diagnostic challenge.

UNLABELLED: Spinal dysraphism (SD) is a collective term for congenital malformations of the spine and spinal cord. It includes a wide range of congenital anomalies resulting from aberrations in the stages of gastrulation, primary neurulation and secondary neurulation. Spinal dysraphism may lead to neurological impairment of varying severity including weakness of the extremities, incontinence of bowel and bladder, sexual dysfunction, among others. Diagnosis of SDs is quite challenging because of its wide spectrum and complex cascade of embryologic events. Knowledge of normal embryology and prop…

Original report of bilateral carotid body tumors with 2 rare concomitant anatomic findings, an ectopic parathyroid gland and cervical thymus, with literature review.

BACKGROUND: Carotid body tumors are uncommon neoplasms with unique epidemiology and management demands. Cervical embryology is complex. Developmental abnormalities can result in ectopic displacement of native tissues. METHODS: We present the case of a 21-year-old female with bilateral carotid body paragangliomas who presented to our clinic seeking excision of the symptomatic right-sided tumor. RESULTS: The patient was successfully treated with surgical excision. Two rare anatomic variants were identified in her surgical specimen: a carotid sheath parathyroid gland and ectopic thymus tissue. …
