Postnatal development of palatal and laryngeal taste buds in the hamster.

Mammalian taste buds are distributed within several distinct subpopulations, innervated by branches of three cranial nerves. These taste bud populations originate and mature at different times in various mammalian species and are thought to play differential roles in the control of taste-mediated behaviors. The hamster is a common animal for the electrophysiological study of the gustatory system, and it has been shown that taste buds innervated by the IXth nerve develop postnatally in this species. To delineate further the development of the gustatory system of hamsters, we quantified the numb

Croup: Diagnosis and Management.

Croup is a common respiratory illness affecting 3% of children six months to three years of age. It accounts for 7% of hospitalizations annually for fever and/or acute respiratory illness in children younger than five years. Croup is a manifestation of upper airway obstruction resulting from swelling of the larynx, trachea, and bronchi, leading to inspiratory stridor and a barking cough. Many patients experience low-grade fevers, but fever is not necessary for diagnosis. Less commonly, stridor can be associated with acute epiglottitis, bacterial tracheitis, and foreign body airway obstruction.…

[Diseases and procedures apt to conflict with patients: an analysis of medical malpractice litigation cases].

The aim of this study was to explore effective ways to prevent conflicts between patients and healthcare professionals by analyzing 836 malpractice cases. The analysis revealed two points that especially influence court decisions: disease prognosis and inadequate informed consent. Regarding prognosis, decisions are more in favor of the defendant (medical institution) in diseases with poor prognoses, such as sepsis and anaphylaxis, than in diseases with typically good prognoses, such as acute epiglottitis and strangulation ileus. Regarding insufficient informed consent, the cases fell into two …

We report a case of a 28-year-old male who died suddenly on the way from his house to the hospital. He was said to have a gurgling sound in … ered only 1-2% of the total body surface area. The internal examination disclosed severe glossitis, epiglottitis and inflammation of the upper respiratory tract with prominent laryngeal edema. The microscopic sections of some skin lesions confirmed the previous diagnosis, and the cause of sudden death was believed to be acute upper airway obstruction which com …

[Haemophilus influenzae type B. Disease and prevention].

Haemophilus influenzae is a gram-negative rod, causing severe infections in childhood, including meningitis, sepsis, epiglottits, pneumonia and otitis. Most of the invasive infections are due to serotype b. Since ampicillin-resistance is increasing, modern cephalosporines like cefotaxime and ceftriaxone are the antibiotics of choice in severe disease. Bacterial meningitis due to Haemophilus influenzae and epiglottitis are both still life-threatening diseases with a lethality of 5% to 25%, and there are severe sequelae in 35% of meningitis cases. Efforts have been made to develop efficacious va…

Bacteremic disease due to Haemophilus influenzae capsular type f in adults: report of five cases and review.

Five cases of bacteremic infections due to Haemophilus influenzae type f in adults are described, and previous reports of type f disease in nonpediatric patients are reviewed. Respiratory tract infections were most common in our series (two cases of pneumonia, one of epiglottitis, and one of nosocomial septicemia probably resulting from aspiration pneumonitis). All of these patients had factors predisposing them to respiratory tract infections, e.g., neurologic disease, congestive heart failure, or cigarette smoking. A fifth patient, who was bacteremic without an apparent primary focus, had dy…

Antibiotics: their true place in the treatment of viral disease.

The use of antibiotics in viral diseases of childhood is discussed. If bacterial infection is likely, either as superinfection or as part of the differential diagnosis, then antibiotics should be given. The antibiotic of choice for each illness is considered. Respiratory infections are common. The diagnosis and treatment of streptococcal pharyngitis is compared with viral pharyngitis. Penicillin is indicated if the bacterial infection is possible. If there is difficulty in distinguishing between croup and epiglottitis, then chloramphenicol or ampicillin should be given. Otitis media and pneumo…

Infections of the Neck.

Infection of the neck is a relatively common emergency department complaint. If not diagnosed and managed promptly, it may quickly progress to a life-threatening infection. These infections can result in true airway emergencies that may require fiberoptic or surgical airways. This article covers common, as well as rare but emergent, presentations and uses an evidence-based approach to discuss diagnostic and treatment modalities.

Isolated laryngeal tuberculosis complex infection: expect the unexpected.

Laryngeal tuberculosis (LT), a rare but possible manifestation of extrapulmonary tuberculosis (TBC) and the most frequent granulomatous disease of the larynx, is slowly resurfacing due to the worldwide recrudescence of TBC. We present the case of a 59 y-o Caucasian woman, non-smoker, with no history of recent travels in endemic areas, affected by pulmonary sarcoidosis, that presented with a symptomatic vegetating lesion involving the left free margin of the epiglottic and a small, ulcerated lesion over the right arytenoid mucosa. While the patient's profile would not lead to a primary suspect

[Study of the reconstruction methods after vertical partial laryngectomy for glottic carcinoma of the larynx].

OBJECTIVE: To study the reconstruction methods after vertical partial laryngectomy for glottic carcinoma of the larynx. METHOD: 58 cases with glottic carcinoma of the larynx were treated with vertical partial laryngectomy or extended vertical partial laryngectomy. The clinical materials of these cases were retrospectively studied. RESULT: The total decannulation rate was 93.1% and the 3- and 5-year survival rates were 87.2% and 80.5% respectively. The voice was socially acceptable in 93.1%(54/58) of the patients. Aspiration was only 3. All resumed mouth-food-taking. CONCLUSION: The reconstr

Laryngeal cyst and sudden death.

Laryngeal cysts are relatively uncommon and account for about five per cent of benign laryngeal lesions. The commonest location is the epiglottis. Pathologically these lesions may be divided into epithelial, tonsillar or oncocytic types. The patients may be asymptomatic or present with hoarseness, dysphagia, cough or airway obstruction. Epithelial cysts of the larynx, although histologically benign, may produce sudden and unexpected death from asphyxia. A case is presented of a 32-year-old female who was sexually assaulted and later found dead. Death was most likely due to laryngeal obstructio

Cancer of the oropharynx: classification and natural history.

The development, comparative anatomy, and function of the oropharynx are briefly described. The text book descriptions of the anatomical boundaries of the oropharynx are commented on and an attempt is made to define these boundaries more rigidly and accurately. Although the advantages and limitations of the UICC classification of oropharyngeal cancer is accepted, an alternative classification is suggested based on the function of the site involved rather than its descriptive anatomy. This has resulted in a necessary rearrangement of the tumor sites and subsites. Subdivision into three sites is

Postnatal descent of the epiglottis in man. A preliminary report.

The function of the epiglottis in adult man is unclear. However, during early infancy the epiglottic cartilage appears to play an important role in separating the upper respiratory tract from the upper digestive tract. This separation is accomplished by approximation of the epiglottis to the palate, providing a continuous airway from the nose through the larynx into the trachea. This structural arrangement, however, is uniquely lost in man during postnatal development. Maturational descent of the epiglottis, found to occur between 4 and 6 months of age, is verified by cineradiography. This str

Dysphagia Phenotypes in COVID-19 Pneumonia Versus Aspiration Pneumonia: A Retrospective Quantitative Videofluoroscopic Cohort Study.

Background and Objectives: Comparisons of swallowing physiology between coronavirus disease 2019 (COVID-19) pneumonia and clinically diagnosed aspiration pneumonia (AP) have largely relied on ordinal scales, leaving etiology-specific biomechanical profiles unclear. We quantitatively compared videofluoroscopic swallowing study (VFSS) measures of pharyngeal residue and clearance mechanics to identify differential dysphagia phenotypes. Materials and Methods: This single-center retrospective cohort study included 50 adult inpatients with pneumonia (COVID-19, n = 25; AP, n = 25) who underwent VFSS

Epiglottopexy for refractory obstructive sleep apnea in children - A single-institution experience.

BACKGROUND: Refractory obstructive sleep apnea (OSA) is a common condition in children with medical comorbidities, leading to a significant impact on quality of life. Drug-induced sleep endoscopy (DISE) has become the standard of care in identifying the levels of obstruction in children with refractory OSA. Epiglottopexy has been shown to improve OSA symptoms in adults and healthy children with epiglottic prolapse in a few studies, with minimal long-term complications. The objective of our study was to evaluate the role of epiglottopexy in children with refractory OSA. METHODS: A retrospectiv

Helical computed tomography scanning of the larynx and upper trachea in rabbits.

BACKGROUND: Computed tomography (CT) is used to evaluate the human tracheobronchial tree because of its unsurpassed ability to visualize the airway and surrounding structures. To establish an ideal animal model for studying subglottic stenosis, we assessed the size and morphology of the normal rabbit's laryngotracheal airway by helical CT. We measured luminal dimensions at the levels of the arytenoid and cricoid cartilages and the first, third, and eighth tracheal rings. At all levels, the axial slices were used to calculate the maximum anteroposterior (AP) dimension, transverse dimension, and
