Orthopedic complications related to growth hormone therapy in a pediatric population.

Since the introduction of recombinant growth hormone, its use has diversified and multiplied. Growth hormone is now the recommended therapy for a growing indication to all forms of short stature because of its direct and indirect role on bone growth. Hereby, we discuss the orthopedic complications associated with growth hormone treatment in pediatric patients. These complications include carpal tunnel syndrome, Legg-Calve-Perthes' disease, scoliosis, and slipped capital femoral epiphysis. Their incidence rates recorded in several growth hormone therapy-related pharmacovigilance studies will be

Slipped capital femoral epiphysis: rising rates with obesity and aboriginality in South Australia.

We analysed the incidence of slipped capital femoral epiphysis (SCFE) in South Australia, investigating possible associations between an increased incidence of SCFE, the local indigenous population and the Australian obesity epidemic during the last 20 years. Data including race, age and gender were collected to obtain a profile of the South Australian SCFE patient, and were then compared with epidemiological data for South Australian adolescents. We concluded that the incidence of both obesity and SCFE is increasing. We also noted that the median weight of SCFE patients has increased and the

Accuracy of computer-assisted navigation for femoral head resurfacing decreases in hips with abnormal anatomy.

Computer-assisted navigation systems for hip resurfacing arthroplasty are designed to minimize the chance of implant malposition. However, there is little evidence computer navigation is useful in the presence of anatomical deformity. We therefore determined the accuracy of an image-free resurfacing hip arthroplasty navigation system in the presence of a pistol grip deformity of the head and femoral neck junction and of a slipped upper femoral epiphysis deformity. We constructed an artificial phantom leg from machined aluminum with a simulated hip and knee. The frontal and lateral plane implan

[Neonatal traumatic epiphyseolysis of the humeral head. A rare occurrence].

Neonatal traumatic epiphyseolysis of the humeral head is rare, and only a few cases are reported in the literature. We present a case of a 13-day-old female newborn with malposition and relaxation of the left upper limb. The clinical examination showed distinct range of motion particularly for abduction. Magnetic resonance imaging (MRI) indicated epiphyseolysis of the humeral head. Closed reposition followed by immobilisation was done. The following MRI showed correct axis with adaption of the humeral head. Later, malposition of the axis with angulation in the ventromedial position was seen. T

Patient survey of weight-bearing and physical activity after in situ pinning for slipped capital femoral epiphysis.

Patients with slipped capital femoral epiphysis(SCFE)are often instructed to use crutches and restrict their activities after surgery. In the retrospective study reported here, we determined actual duration of crutch use and activity restriction in patients with SCFE treated with in situ pinning. Forty-three patients (mean age, 14.1 years) responded to a questionnaire. Four patients (9%) never used crutches, and 29 patients (67%) used crutches for 4 weeks or less. Three patients (7%) never restricted their activity, and 29 patients (67%) had resumed full activities, including sports, by 6 mont

Complications at screw removal in slipped capital femoral epiphysis treated by cannulated titanium screws.

INTRODUCTION: Various modes of fixation are proposed for the treatment of slips of the capital femoral epiphysis (SCFE). We describe our experience with the use and removal of a new, cannulated titanium screw (Asnis III, Stryker, Howmedica). PATIENTS AND METHODS: Single cannulated titanium screws had been inserted in 101 hips of 65 patients in the 3-year period from 2001 to 2003. These pins were used for in situ fixation of minor chronic slips in 41 patients and for prophylactic fixation on the contralateral side in patients with open physis in 60 patients. RESULTS: The insertion of these sc

Seasonal variation of slipped capital femoral epiphysis.

During the 18-year period 1980-1997, 1103 patients were treated as in-patients for slipped capital femoral epiphysis (SCFE) in Scottish hospitals. This paper reports a significant seasonal variation, especially in male patients, with an excess admitted in the autumn. Examining the physiological seasonality of the monthly increment of growth in height and weight in childhood, we hypothesised that these physiological rhythms, while not pathogenetic, may be responsible for the timing of the seasonal features of SCFE. Previously published studies suggest that the average time between first symptom

Effect of femur position on the angular measurement of slipped capital femoral epiphysis.

A femur model was used to simulate a slipped capital femoral epiphysis (SCFE). Wedges were inserted into a cut in the femoral neck to create SCFEs of 25, 50, and 75 degrees. The effects of femur position on the angular measurement of SCFE were studied by taking radiographs of the SCFE at preselected positions of rotation and abduction, which were ensured by a holding device. The views selected were the cross table lateral (CTL), Billing lateral, and frog lateral (FL) projections. FL views were obtained at 22.5, 45, and 67.5 degrees of abduction. The rotational positions were 20 degrees interna

Different operative treatment of slipped capital femoral epiphysis: a comparative study of biomechanical status of the hip.

The biomechanical status of the hips subject to two different operative treatments was studied. The biomechanical status was determined by the peak stress on the weight bearing area using the 3-d mathematical model. It was found that the procedure according to Imhäuser yields considerably and statistically significantly higher peak stress than the procedure according to Dunn-Fish.

Slipped capital femoral epiphysis: a long-term follow-up study after open reduction of the femoral head combined with subcapital wedge resection.

Sixty-six hips of 65 patients with moderate to severe slipped capital femoral epiphysis were treated by open reduction of the femoral head. In 60 hips this procedure was combined with a cuneiform subcapital wedge resection of the femoral neck according to the technique described by Dunn (14). Avascular necrosis occurred in seven cases, and chondrolysis was evident in eight hips. In the 48 hips with a follow-up time of more than 10 years (M = 20.6), the results were classified as good (normal hip) in 22, moderate in 16, and poor in 10 hips. Compared with the natural disease course, the long-ter

Long-term follow-up of patients with avascular necrosis after treatment of slipped capital femoral epiphysis.

A retrospective review of 264 patients treated for slipped capital femoral epiphysis (SCFE) identified 36 who developed avascular necrosis (AVN). Twenty-two patients (24 hips) were evaluated at an average follow-up of 31 years. Nine have undergone reconstructive surgery: four during adolescence and five during adulthood. The remaining 13 patients (15 hips) have had no further operations, but all show degenerative changes on current roentgenograms. The natural history appears to be that of gradual degenerative changes for which reconstructive surgery most often can be delayed until adulthood.

Growth plate injury of the hand and wrist in renal osteodystrophy.

We present two cases of growth plate injury of the hand and wrist in young patients with renal rickets and osteodystrophy but no history of antecedent trauma. The postulated mechanism of injury is subclinical biomechanical stress on the involved extremity superimposed on the already weakened metaphysis and growth plate. Thus, the findings may be viewed as an insufficiency injury to the metaphysis and growth plate. Physicians should be aware that, in the absence of unusual athletic activity, epiphysiolysis of the upper extremity is uncommon and requires a search for underlying metabolic bone di

Articulotrochanteric distance in slipped capital femoral epiphysis.

The difference in altitude between the proximal limit of the femoral head and the tip of the greater trochanter--the articulotrochanteric distance (ATD)--was recorded in 312 patients with slipped capital femoral epiphysis and compared with that in normal hips. The ATD was markedly reduced after severe slipping, independent of the method of treatment. In mild or moderate slippings, the ATD differed with the method of treatment. The shortest ATD was seen after osteosynthesis with AO screws. Nonoperative treatment and osteosynthesis with the Johansson nail or Nyström pin caused a slight reduction

Intertrochanteric corrective osteotomy in slipped capital femoral epiphysis. A long-term follow-up study of 26 patients.

The results of intertrochanteric corrective osteotomy in a series of 26 hips with moderate to severe chronic slipped capital femoral epiphysis are reported from follow-up studies in 1976 and 1986. In hips with a slippage of less than 40 degrees (ten hips), arthrosis was present in one hip. In the remaining 16 cases in which slippage exceeded 40 degrees, osteoarthrosis was present in 15, even though correction was adequate. From these observations it can be concluded that intertrochanteric corrective osteotomy does not prevent degeneration in cases with the most severe slip. On the basis of the

[Periosteal bone resorption in the area of the metaphysis of growing bone as a precursor of epiphyseal injuries. A polarization optical and scanning electron microscopy study].

Macerated epiphyses of the growing infant bone exhibit an irregular surface which can already be observed at the macroscopic level. Polarizing microscopy demonstrates extended zones of bone resorption on the cortical surface, deductable from numerous lacunae of Howship. Ensuing experimentally induced epiphysiolyses, cortical bone fragments adhering to the inner surface of the periosteum are demonstrable by scanning electron microscopy. Having been torn out of the bone together with the periosteum, these cortical fragments leave corresponding defects on the bone surface. Due to the remodelling

The inclination of the weight bearing surface in the hip joint. The clinical significance of abnormal force.

The mechanical basis responsible for the development of secondary osteoarthritis in adults and slipped capital femoral epiphysis in adolescents is discussed, with the role of the oblique weight-bearing surface in generating abnormal shearing forces in these two conditions emphasized. The same biomechanical principles have been found to apply to the acetabular component in total hip implants. A vertical cup position will lead to abnormal stress concentration in the outer edge of the bone implant interface with an increase in shearing forces. The form of the subchondral bone sclerosis on the rad

Epiphysiolysis of the proximal femur in very young children.

In 14 hips in 11 children under 3 years of age, proximal femoral epiphysiolysis occurred from varying degrees of trauma. The vulnerability of the blood supply of the upper femoral epiphysis, and its important weight-bearing function, demands early diagnosis, reduction, and adequate fixation. The nature of the injury, however, determines the eventual outcome. Early diagnosis and investigation of the underlying pathology is necessary to avoid such complications as varus deformity, avascular necrosis, heterotopic new bone formation, non-union, and premature closure of the growth plate.
