Physical performance and physical activity in grown-up congenital heart disease.

AIM: To review exercise performance and exercise habits in patients with congenital heart disease (CHD). BACKGROUND AND METHODS: Physical e … ive anomalies (aortic stenosis, pulmonary stenosis, coarctation of the aorta). Subnormal values for exercise tolerance have also been observed in patients with successfully repaired cyanotic heart disease (tetraology of Fallot, transposition of the great arteries, Fontan operation). An important contributing factor to the impaired exercise performance is the hypoact …

Influence of Sodium Glucose Cotransporter 2 Inhibition on Physiological Adaptation to Endurance Exercise Training.

CONTEXT: The combination of two beneficial antidiabetes interventions, regular exercise and pharmaceuticals, is intuitively appealing. However, metformin, the most commonly prescribed diabetes medication, attenuates the favorable physiological adaptations to exercise; in turn, exercise may impede the action of metformin. OBJECTIVE: We sought to determine the influence of an alternative diabetes treatment, sodium glucose cotransporter 2 (SGLT2) inhibition, on the response to endurance exercise training. DESIGN, PARTICIPANTS, AND INTERVENTION: In a randomized, double-blind, repeated measures p

Frailty in Pulmonary Hypertension: A Multidimensional Comorbidity.

Pulmonary hypertension is a complex and irreversible pulmonary vascular disease with a significant impact on patients' lives. Pulmonary hypertension is categorized into five groups according to its etiology. Frailty is a multifaceted disorder that leads to physical, social, cognitive, and psychological decline. Several studies have linked a high occurrence of frailty to heart failure and chronic lung diseases, but the prevalence and impact of frailty in pulmonary arterial hypertension (PAH) remain largely unexplored. At the molecular level, inflammation, oxidative stress, cellular senescence,

Depression and anxiety in cardiac rehabilitation: differential associations with changes in exercise capacity and quality of life.

BACKGROUND: Cardiac rehabilitation (CR) has been successful in improving exercise capacity (EC) and quality of life (QoL). However, depression and anxiety are highly prevalent among cardiac patients and might represent risk factors for rehabilitation outcomes. The aim of this study was to investigate the role of depression and anxiety as possible independent risk factors for CR outcomes. METHODS: The study applied a pre-post-design. The sample comprised N = 3'434 cardiac disease patients taking part in a Swiss inpatient CR center. Variables measured at the beginning (T1) and end of rehabilita

The Effects of Cardiac Rehabilitation in Patients With Atrial Fibrillation: A Systematic Review.

BACKGROUND: Atrial fibrillation (AF), the most common sustained cardiac arrhythmia, is associated with substantial morbidity and mortality. Clinical guidelines for managing patients with AF do not include a referral to cardiac rehabilitation (CR) at present, although it is routine for most other cardiovascular conditions. The number of studies evaluating the impact of CR on the health of patients with AF is growing, but there has been no consolidation of the findings. The objective of this systematic review was to evaluate the impact of CR programs on physical and mental health outcomes in pat

Recognition, diagnosis, and treatment of mitochondrial myopathies in endurance athletes.

Endurance athletes complaining of muscle pains concomitant with fatigue and exercise intolerance provide a diagnostic challenge. When the most common causes have been ruled out, the presence of metabolic myopathies, including mitochondrial myopathies (MMs), should be considered. MMs are a group of diseases characterized by inadequate mitochondrial ATP production needed for the energetic requirement of the exercising muscles. Athletes with myalgia, fatigue, dyspnea, and muscular cramping should be questioned for history of rhabdomyolysis or myoglobinuria as well as detailed family history, give

Combined 31P and 1H MRS show that impaired muscle O2 supply limits ATP synthesis during exercise in systemic sclerosis.

Muscle weakness and fatigue feature in many diseases, but the underlying pathophysiology can be difficult to study in vivo. In patients with … cates a defect in O2 supply rather than in mitochondrial O2 use. This major contributor to impaired exercise tolerance would be difficult to establish using classical methods of biopsy and exercise physiology, which shows the power of non-invasive measurement modalities to probe the pathophysiology of complex conditions.

Noninvasive assessment of exercise cardiac function before and after pectus excavatum repair.

Surgical correction of pectus excavatum frequently results in subjective improvement of exercise tolerance. Whether or not cardiac function improves after repair remains controversial and has primarily been limited to isolated case reports. The purpose of this investigation was to assess changes in cardiac function during rest and exercise associated with the surgical correction of this deformity. First-pass radionuclide studies during upright rest and bicycle exercise were performed on 13 patients before and at least 6 months after pectus excavatum repair. Operation did not change left ventri…

Return to flight status after cardiac rehabilitation: three case histories.

A pilot's license to operate aircraft is not valid unless it is accompanied by a medical certificate. This certificate is revoked if a pilot is diagnosed with a cardiovascular illness. After a lengthy waiting period, the medical certificate may be reinstated if the pilot meets rigid standards. For many pilots, participation in a cardiac rehabilitation program is essential to achieve the minimal functional capacity in exercise testing (10.0 metabolic equivalents), document tolerance of medications, and achieve successful rehabilitation. Our staff has assisted 11 pilots in their quest to resume
