Therapeutic management of traumatic facial palsy: a systematic review.

PURPOSE: Trauma is a common cause of facial nerve palsy, accounting for 3% of all cases. While many facial palsies resolve with medical treatment, some require surgical intervention. This systematic review aimed to determine the best therapeutic strategy for traumatic facial palsy. METHODS: We reviewed eligible articles for patient demographics, pre-treatment assessment, parameters of selected treatment, type of treatment, outcomes, and post-treatment assessment. RESULTS: Among 135 unique citations, 32 studies were considered eligible, reporting treatment data for 2079 patients. Most studies

Epidemiology of facial fractures: incidence, prevalence and years lived with disability estimates from the Global Burden of Disease 2017 study.

BACKGROUND: The Global Burden of Disease Study (GBD) has historically produced estimates of causes of injury such as falls but not the resulting types of injuries that occur. The objective of this study was to estimate the global incidence, prevalence and years lived with disability (YLDs) due to facial fractures and to estimate the leading injurious causes of facial fracture. METHODS: We obtained results from GBD 2017. First, the study estimated the incidence from each injury cause (eg, falls), and then the proportion of each cause that would result in facial fracture being the most disablin

A Deeper Dive into the Anatomy of the Frontal Branch of the Facial Nerve.

The frontal branch of the facial nerve innervates several muscles in the upper face and is essential for facial expression and eye closure. Injury to the frontal branch can result in functional and aesthetic issues; therefore, a thorough understanding of the anatomy and course of the frontal branch is essential for safe surgery in the lateral face and temporal region. The course of the frontal branch is variable, but certain anatomic landmarks can be used to locate the nerve. Preoperative planning, careful dissection, and intraoperative monitoring are critical to minimize nerve injury in tempo

Custom-Made Versus Non-Custom Mouthguards in the Prevention of Sports-Related Oro-Dento-Facial Trauma: A Scoping Review of the Clinical Evidence.

Sports-related oro-dento-facial trauma is a major clinical and socioeconomic burden, with oro-dento-facial injuries affecting approximately 30-40% of athletes. Mouthguards are widely recommended as primary protective devices, yet the comparative effectiveness of custom-made versus stock and boil-and-bite designs remains unclear. This scoping review aimed to map the clinical evidence on different mouthguard types for preventing sports-related oro-dento-facial injuries. The review followed the PRISMA-ScR guidelines. PubMed, Embase, Scopus, Web of Science, and the Cochrane Library were searched o…

Choice of motorcycle helmet makes a difference: a prospective observational study.

BACKGROUND: Although many states mandate that motorcyclists wear helmets, their laws do not indicate which type of helmet should be used. In addition, there are no prospective studies in the literature evaluating patterns of injuries as they relate to helmet type. The hypothesis in this study was that full-face helmets (FFHs) reduce craniofacial injuries associated with motorcycle collisions when compared with other helmet types. METHODS: A prospective observational study was conducted at a Level I trauma center to evaluate the efficacy of helmet types relative to craniofacial injuries. Data …

Review of computed tomography imaging can decrease nontherapeutic transfer after facial trauma.

BACKGROUND: Maxillofacial trauma encompasses a spectrum of injuries, from minor facial fractures to severe craniofacial defects, some requiring urgent surgical intervention while others are managed nonoperatively. Remote evaluation of patients by facial trauma specialists before transfer could provide an opportunity to reduce the number of nontherapeutic transfers. METHODS: Our institution's trauma registry was queried for adult patients transferred from a system hospital with facial injuries. A random selection of 150 patients with computed tomography (CT) imaging available for review from t…

Quality improvement in documentation for patients with suspected facial fractures: use of a structured record keeping tool.

OBJECTIVE: Patients with injuries to the midface frequently sustain ophthalmic injuries and fractures to the facial bones. Despite this, basic ophthalmic examination and assessment of important clinical signs are often missing from the records of patients attending the emergency department (ED). We implemented a structured record keeping tool to improve documentation for patients presenting to the ED with midface injuries. METHODS: At our institution, a structured record keeping tool was introduced to document important clinical features of maxillofacial injuries. This assessment tool include…

Rapid and accurate identification of cut ends of facial nerves using a nerve monitoring system during surgical exploration and anastomosis.

PURPOSE: Surgical exploration and end-to-end neurorrhaphy is the preferred management for traumatic facial nerve injury. Traditionally, finding the cut ends of facial nerves depends mainly on a surgeon's experience. In this study, a nerve monitoring system to help the surgeon quickly and accurately identify, confirm, and locate the cut ends of facial nerve branches was investigated. PATIENTS AND METHODS: Six patients with traumatic facial nerve injury were selected, and the nerve monitoring system was applied during the surgical process of facial nerve exploration and anastomosis. Dentoalveolar trauma.

Dentoalveolar injuries are an important and common component of craniomaxillofacial trauma. The dentition serves as a vertical buttress of the face and fractures to this area may result in malalignment of facial subunits. Furthermore, the dentition is succedaneous with 3 phases-primary dentition, mixed dentition, and permanent dentition-mandating different treatment protocols. This article is written for nondental providers to diagnose and treat dentoalveolar injuries.

Bilateral Facial Paralysis Caused by Bilateral Temporal Bone Fracture: A Case Report and a Literature Review.

Bilateral facial paralysis caused by bilateral temporal bone fracture is a rare clinical entity, with seven cases reported in the literature to date. In this paper, we describe a 40-year-old male patient with bilateral facial paralysis and hearing loss that developed after an occupational accident. On physical examination, House-Brackmann (HB) facial paralysis of grade 6 was observed on the right side and HB grade 5 paralysis on the left. Upon temporal bone computed tomography (CT) examination, a fracture line exhibiting transverse progression was observed in both petrous temporal bones. Our p

Using an end-to-side interposed sural nerve graft for facial nerve reinforcement after vestibular schwannoma resection. Technical note.

Increasing rates of facial and cochlear nerve preservation after vestibular schwannoma surgery have been achieved in the last 30 years. However, the management of a partially or completely damaged facial nerve remains an important issue. In such a case, several immediate or delayed repair techniques have been used. On the basis of recent studies of successful end-to-side neurorrhaphy, the authors applied this technique in a patient with an anatomically preserved but partially injured facial nerve during vestibular schwannoma surgery. The authors interposed a sural nerve graft to reinforce the

Head and neck injuries.

A survey of cranial, intracranial and neck injuries caused by blunt trauma to the head is presented. The different types of fractures to the skull (impression fractures, linear fractures and ring fractures) are discussed as well as the injury mechanisms. Brain injuries are discussed under the heading of focal brain injury (i.e. brain laceration, cortical contusion, hemorrhages of the meninges and traumatic intracerebral hemorrhages). The different types of neck injuries, including atlanto-occipital dislocation, are listed and the stability of the different fractures are discussed. Facial injur

Avoiding and correcting complications in perinasal trauma.

Injuries to the nose and perinasal region are common. Although the nasal fractures are commonly recognized and properly addressed, injuries to adjacent structures such as the orbit, medial canthus, and midface skeleton can be missed or misdiagnosed, leading to improper primary treatment and subsequent secondary deformities. In this discussion, three common injuries will be discussed, including nasomaxillary fractures, limited naso-orbital-ethmoid fractures, and severe central facial injuries with naso-orbital-ethmoid fractures. For instructional purposes, a case example of inadequate primary d…

Case Report: Facial Nerve Bifurcation Noted During Resection of Vestibular Schwannoma.

BACKGROUND AND IMPORTANCE: Resection of cerebellopontine angle tumors is challenging because the proximity of the facial nerve puts it at risk of inadvertent injury and subsequent dysfunction. It is critical to consider variations in anatomy and be aware of the potential deviations in the course of the nerve in order to avoid damage. CLINICAL PRESENTATION: We present a case of a facial nerve bifurcation identified during resection of a vestibular schwannoma. CONCLUSION: This is the only reported case of proximal facial nerve bifurcation. We review what is known about variations in proximal f
