The association of a thrombo-embolic venous disease and multiple osteonecroses occurring in the presence of biological risk factors for thrombosis is rarely described in the literature. A young woman was admitted to hospital with suspected pyelonephritis. Due to prolonged fever, further investigations were done and showed a thrombosis in her femoral vein; X-ray and lung scintigraphy revealed a pulmonary embolism. Blood tests showed lymphocytosis, and a primary cytamegalovirus (CMV) infection was confirmed by serology and PCR. The patient was a smoker and obese and was taking oral contraceptives; a factor V Leiden mutation was also found. Deep-vein thrombosis is a rare but severe complication of a primary CMV infection that is also seen in immunocompetent persons.

OBJECTIVE: To analyze a risk factor for the onset of fulminant bacterial infection.
PATIENTS AND METHODS: Nine unexpected acute death cases were clinicopathologically analysed. All cases represented the sudden onset of shock symptom, led to acute death within a few days, and later bacteremia was identified. Pathogens were Streptococcus pneumoniae (S. pneumoniae) (5 cases), group A beta Hemolytic Streptococcus pyogenes (S. pyogenes) (3 cases), and Vibrio vulnificus (V. vulnificus) (1 case).
