Eosinophilic fasciitis. A case report and review of the literature.

Eosinophilic fasciitis (EF) is a newly appreciated disease whose hallmarks are local areas of hidebound and puckered skin with peripheral eosinophilia. It looks similar to progressive systemic sclerosis and morphea but often lacks positive antinuclear antibodies (ANA) and Raynaud's phenomenon. Contractures over joints may develop; several cases of coincident aplastic anemia have been reported. Deep skin biopsy of involved skin reveals lymphocytes, plasma cells and occasionally eosinophils. Unlike scleroderma, EF improves with the administration of corticosteroids.

Eosinophilic fasciitis. A pathologic study of twenty cases.

This report presents a detailed light-microscopic evaluation of biopsies obtained from 20 patients with eosinophilic fasciitis, a newly recognized disorder characterized by inflammation and thickening of the deep fascia, hypergammaglobulinemia, and peripheral and tissue eosinophilia. Early in the course of the disease, the deep fascia and lower subcutis are edematous and infiltrated with lymphocytes, plasma cells, histiocytes, and eosinophils; these features are associated with impressive peripheral eosinophilia. As the illness progresses, these structures and eventually the dermis become coll…

Treatment of necrotizing fasciitis caused by Staphylococcus epidermidis.

Postoperative necrotizing fasciitis with septicemia caused by Staphylococcus epidermidis was documented by cultures of the blood and wound biopsy specimen. Therapy consisted of surgical debridement, topical application of mafenide acetate dressings, and parenteral administration of cefazolin sodium. The combination effectively reversed the progression of infection and necrosis.

Factitial panniculitis and necrotizing fasciitis in juvenile diabetes.

A fifteen-year-old diabetic girl developed multiple inflammatory areas on the extremities and abdominal wall at the site of insulin injection. The bizarre clinical manifestations led to great difficulties in determining both its etiology and its treatment. However, three months after her initial hospitalization, we found that the lesions were probably secondary to self-injection with a chemical agent rather than a result of insulin allergy. If idiopathic recurrent dermal infection is observed, then factitial origin should be suspected.

A Case Report of Culture-Negative Necrotizing Fasciitis With Low Laboratory Risk Indicator for Necrotizing Fasciitis (LRINEC) Score: A Diagnostic Dilemma.

The Laboratory Risk Indicator for Necrotizing Fasciitis (LRINEC) score is a diagnostic tool used to help clinicians identify necrotizing fasciitis (NF) in its early stages. This tool uses six laboratory values including the patient's white blood cell count, C-reactive protein level, serum sodium level, creatinine level, and hemoglobin level to help with risk stratification. Each of these laboratory values is assigned a point value and the total score is used to determine the likelihood that a patient has NF, with a score of 6 or higher considered to be strongly indicative. The LRINEC score has…

Utility of Magnetic Resonance Imaging for Differentiating Necrotizing Fasciitis from Severe Cellulitis: A Magnetic Resonance Indicator for Necrotizing Fasciitis (MRINEC) Algorithm.

We developed a new magnetic resonance indicator for necrotizing fasciitis (MRINEC) algorithm for differentiating necrotizing fasciitis (NF) from severe cellulitis (SC). All adults with suspected NF between 2010 and 2018 in a tertiary hospital in South Korea were enrolled. Sixty-one patients were diagnosed with NF and 28 with SC. Among them, 34 with NF and 15 with SC underwent magnetic resonance imaging (MRI). The MRINEC algorithm, a two-step decision tree including T2 hyperintensity of intermuscular deep fascia and diffuse T2 hyperintensity of deep peripheral fascia, diagnosed NF with 94% sens…

Abdominal Necrotizing Fasciitis Secondary to Perforated Colon Cancer.

Necrotizing Fasciitis (NF) is a severe, potentially fatal soft tissue infection leading to death of muscles, fascia, and surrounding tissue. Perforated colon cancer (CC) is an exceedingly uncommon cause of NF. Here, we present a case of NF secondary to perforated colon cancer. This case emphasizes the significance of prompt identification, aggressive surgical excision of affected tissue, and consideration of rare, yet potentially fatal underlying etiologies in the management of NF as well as underscores the importance of CC screening to prevent devastating complications and mortality.

Nivolumab-Induced Fasciitis Mimicking Scleroderma: A Diagnostic Challenge.

A 74-year-old male on nivolumab for desmoplastic melanoma developed progressive skin tightening, woody induration, and restricted mobility, initially suggestive of systemic sclerosis. He lacked classic scleroderma features (e.g., Raynaud's, digital ulcers) but had eosinophilia (1100 cells/µL) and antinuclear antibody (ANA)/ ribonucleoprotein (RNP) positivity. MRI confirmed eosinophilic fasciitis (EF), prompting discontinuation of nivolumab and initiation of high-dose corticosteroids and mycophenolate, leading to gradual improvement over six months. This case highlights the diagnostic overlap b…

Case Series: Extracutaneous Findings of Eosinophilic Fasciitis Patients.

OBJECTIVE: Eosinophilic fasciitis (EF) is a connective tissue disorder characterized by cutaneous changes similar to scleroderma, usually associated with peripheral eosinophilia. This case series highlights clinical findings, particularly extracutaneous involvement, in EF patients to enhance clinician awareness of this rare condition. METHODS: EF patients' skin and visceral organ involvement, musculoskeletal findings, laboratory tests (including acute phase reactants, autoantibodies, protein electrophoresis, etc.), magnetic resonance imaging (MRI), skin biopsy results, and treatments were eva…

Necrotizing Fasciitis after Panniculectomy Caused by Finegoldia magna.

Necrotizing fasciitis is a rare yet severe complication after body contouring surgery. We present a case of a 54-year-old woman with a complex medical history who developed necrotizing fasciitis 9 days after panniculectomy and epigastric hernia repair. Microbiological examination revealed Finegoldia magna as the causative agent, a rare pathogen in necrotizing fasciitis. Patients undergoing body contouring may be at increased risk of developing necrotizing fasciitis; therefore, increased attention should be paid to this differential diagnosis in case of postoperative signs of infection. This ca…

Periorbital necrotizing fasciitis: clinical perspectives on nine cases.

A multicenter review of Periorbital Necrotizing Fasciitis including nine cases, aged 41 to 82, mostly female, and mainly post-traumatic or idiopathic. Streptococcus pyogenes was the most frequent pathogen. Treatment involved debridement alongside antibiotic therapy in all cases. Two cases experienced toxic shock, with no fatalities. Visual outcomes varied from exenteration to preserved visual acuity with minimal aesthetic impact. Early detection and prompt intervention are paramount due to the significant risks associated with this condition, which may lead to severe complications ranging from…

Morphologically malignant nodular fasciitis with CALD1-USP6 fusion.

Nodular fasciitis is a benign myofibroblastic neoplasm that characteristically enlarges rapidly and then usually regresses spontaneously. While the vast majority of tumors are benign, there are rare reports of morphologically benign nodular fasciitis giving rise to metastases, not predictable on histologic grounds. Here, we report what we believe is an example of morphologically malignant nodular fasciitis, which occurred in the upper extremity of a 7-year-old male. The tumor was composed of short, intersecting fascicles of myofibroblastic cells in a loose myxoid matrix, with keloidal hyaliniz…

Eosinophilic Fasciitis following Checkpoint Inhibitor Therapy with Pembrolizumab.

With the increasing number of indications for checkpoint inhibitor therapy in cancer patients, rheumatology specialists are often involved in the diagnosis and management of immune-related adverse events (irAEs). The most common rheumatic irAEs are arthritis, sicca syndrome, polymyalgia rheumatica, and myositis. Eosinophilic fasciitis, an already rare rheumatic disease, is a very unusual rheumatic irAE. Eosinophilic fasciitis, whether idiopathic or checkpoint inhibitor-associated, has very particular clinical symptoms and findings, such as the orange peel skin appearance and the Groove sign, w…
