Incontinence-associated dermatitis (IAD) describes damage to the skin caused by repeated and prolonged contact with urine and/or feces. Patients of all ages can be affected; the diagnosis is preferably used in adults and adolescents. The predilection sites are perineal, perianal, the inner thighs and the convex areas of the buttocks. Increased moisture on the skin surface, increased pH, occlusion, feces digestive enzymes and repeated skin cleansing procedures are pathophysiologically relevant factors. Typical clinical signs of IAD are erythema, erosions, excoriations and pain. Diagnosis is oft

In idiopathic anorectal incontinence there is weakness of the pelvic floor sphincter muscles. This is evident on digital examination of the external anal sphincter muscle, and can be confirmed by anorectal manometry. This pelvic floor weakness is commonly associated with abnormal descent of the pelvic floor below the plane of the perineum during straining, as during the act of defaecation. This perineal descent is itself a feature of weakness of these muscles as shown by the prominent appearance of this sign in patients with cauda equina lesions. Almost all patients with this syndrome are wome

PURPOSE: Few reports have determined the efficacy of olive oil enemas for severe constipation. Here, we review our experience with olive oil enemas in children with severe chronic constipation.
METHODS: In our outpatient pediatric surgery department, the charts of 118 patients prescribed with olive oil enemas between January 2010 and November 2019 were retrospectively reviewed. A 1-2 ml/kg olive oil enema was given either alone or followed several hours later by a glycerin enema. Ratings included "very effective (VE)," "effective (E)," "limited (L)," "ineffective (I)," and "unknown (U)."

Recent evidence shows that consensual anal penetrative intercourse has become more prevalent, not only limited to gay, bisexual, and other men who have sex with men but also for women who are in a sexual relationship with men. The aim of this review was to study the influence of consensual anal intercourse on pelvic floor function and the role of pelvic floor physical therapy treatment in preventing or treating consensual anal intercourse-induced anodyspareunia and/or fecal incontinence. We reviewed 68 references that showed that anal penetrative intercourse is a risk factor for anodyspareunia…

Irritant diaper dermatitis (IDD) is frequently observed in young children but can affect individuals throughout their lifespan. The broader term incontinence-associated dermatitis (IAD) is used to describe inflammatory skin reactions from prolonged exposure to urine and/or feces. This review compares IAD and IDD in pediatric and geriatric populations, focusing on their pathogenesis, clinical considerations, and management. Both conditions involve prolonged bodily fluid exposure disrupting the skin barrier, making infants and older adults particularly susceptible due to their age-related skin c

Prevalence and incidence of both urinary and fecal incontinence increase with age. They reduce everyday competence and quality of life in all age groups. It is often not recognized because those affected do not express their complaints, but even if the diagnosis has been made, therapy and care often remains inadequate. The aim of the article is to know types of incontinence and their treatment options for a maximum of continence according to the established continence profiles. In older, especially multimorbid frail people, urinary incontinence is not primarily to be seen as a symptom of a dis…

OBJECTIVE: To critically appraise peer-reviewed evidence concerning the value, or implied sense of worth or benefit, of nurses specialized in wound, ostomy, and continence (WOC) care.
DATA SOURCES: The Preferred Reporting Items for Systematic Reviews and Meta-analyses was used to systematically review current literature in a single database from 2009 to the date of search (July 2019).
STUDY SELECTION: The initial search retrieved 2,340 elements; 10 studies were retained following removal of duplicate records, title and abstract reviews, and application of the inclusion/exclusion criteria.

An estimated 14 million adults experience incontinence in England alone, and this population is at a very high risk of developing incontinence-associated dermatitis (IAD). The risk associated with developing IAD is further increased among older adults, due to the effects of the ageing process, which leave the skin thinner and more fragile. Understanding the causative factors of IAD in addition to understanding the impact of IAD on the skin enable the clinician to make informed choices regarding treatment, management and prevention. Additionally, understanding the association between IAD and pr

PURPOSE: The purpose of this study was to describe the biological changes after incontinence-associated dermatitis (IAD) induction by pancreatin in the guinea pigs and to explore the potentially appropriate timing and pancreatin concentration for IAD induction with different severity.
DESIGN: In vivo, experimental study.
SUBJECTS AND SETTING: An experimental animal model (guinea pig) in a controlled laboratory setting was used for investigation.
METHODS: We developed an IAD model in guinea pigs by occluded application of 1%, 5%, and 10% pancreatin solutions for 1, 3, and 5 days, respectivel

This article defines the diagnostic criteria and reviews clinical evaluation and management of fecal incontinence, anorectal pain, dyssynergic defecation, and rectal hyposensitivity and hypersensitivity. Diagnostic evaluation includes anorectal manometry, balloon expulsion test, anal ultrasound, magnetic resonance imaging, defecography, and neurophysiology testing. Fecal incontinence is defined as recurrent uncontrolled passage of fecal material for 3 months. Management includes antidiarrheals, Kegel exercises, biofeedback therapy, dextranomer injection, surgery, sacral nerve stimulation, and …

INTRODUCTION: Anal fissure is a common disease. Usually chronic anal fissures are managed medically. When conservative management fails, surgical treatment should be considered. Lateral internal sphincterotomy has been advocated as the first choice invasive treatment but it has a reported rate of major fecal incontinence of 5%. In order to reduce the onset of major fecal incontinence after anal fissure surgery, it has been proposed to use the anal stretching plus fissurectomy.
METHODS: From 2008 to 2011, 457 patients have been operated for chronic anal fissure.
RESULTS: Twenty-seven patients…
