Allergy and sensitization during childhood associated with prenatal and lactational exposure to marine pollutants.

BACKGROUND: Breast-feeding may affect the risk of developing allergy during childhood and may also cause exposure to immunotoxicants, such as polychlorinated biphenyls (PCBs), which are of concern as marine pollutants in the Faroe Islands and the Arctic region. OBJECTIVES: The objective was to assess whether sensitization and development of allergic disease is associated with duration of breast-feeding and prenatal or postnatal exposures to PCBs and methylmercury. METHODS: A cohort of 656 singleton births was formed in the Faroe Islands during 1999-2001. Duration of breast-feeding and histor

Early postoperative oral feeding after colectomy: an analysis of factors that may predict failure.

BACKGROUND: Previous studies have shown that early postoperative oral feeding is feasible. Traditionally patients were fed when flatus or defecation documented the return of bowel function. This study was undertaken to determine factors that may preclude early feeding. METHODS: One hundred four successive patients underwent colorectal surgery from October 1999 to January 2001. Eighty-nine patients started an oral diet either on postoperative day 1 or 2. Their clinical outcomes were recorded prospectively. Fifteen of the 104 patients were excluded for small-bowel resection (5 patients), periop

Randomized, multicenter trial of two different formulas for very early enteral feeding advancement in extremely-low-birth-weight infants.

BACKGROUND: In extremely-low-birth-weight (ELBW) infants, formula feeding is required if human milk is not available. The tolerance of a new 'high' lactose (55 g/L), low protein, low phosphate, hydrolyzed protein formula (HLF) for early enteral feeding advancement of ELBW infants was compared with that of a low lactose (1 g/L) hydrolyzed protein formula (LLF). METHODS: In a randomized multicenter trial, 99 ELBW infants were fed according to a standardized protocol beginning at 48 hours of age with 12 ml/kg daily increments. Primary outcome was the cumulative milk feeding volume (CFV) from day

Percutaneous endoscopic gastrostomy and jejunostomy for long-term feeding in patients with cancer of the head and neck.

Enteral feeding is often required in patients with cancer of the head and neck. Percutaneous endoscopic gastrostomies (PEGs) and jejunostomies (PEJs) can facilitate enteral feeding in patients who require this treatment. The endoscopic technique allows for the placement of feeding gastrostomies and jejunostomies without a surgical procedure and eliminates the need for nasal tubes for long-term enteral feeding. Forty-two patients with head and neck tumors were referred for placement of PEGs because of severe dysphagia induced by tumors, surgery, radiation, or chemotherapy. The procedure was per

The use of a multivariate analysis to study between-goat variability in feeding behavior and associated rumen pH patterns.

This study examined feeding patterns and their relation to rumen pH. The measured variables were the cumulative time spent chewing (eating or ruminating) and the mean rumen pH, per 20-min intervals (46 intervals altogether). A total of 333 feeding patterns, defined as time-series of measurements for 1 animal and 1 given day during 15 h and 20 min following the afternoon feeding, were collected from 12 stall-housed dairy goats fed a total mixed ration twice daily. The first objective of this study was to analyze the within-day variability of feeding behavior to characterize different feeding pa

Post-pyloric versus gastric tube feeding for preventing pneumonia and improving nutritional outcomes in critically ill adults.

BACKGROUND: Nutritional support is an essential component of critical care. Malnutrition has been associated with poor outcomes among patients in intensive care units (ICUs). Evidence suggests that in patients with a functional gut, nutrition should be administered through the enteral route. One of the main concerns regarding use of the enteral route is the reduction in gastric motility that is often responsible for limited caloric intake. This increases the risk of aspiration pneumonia as well. Post-pyloric feeding, in which the feed is delivered directly into the duodenum or the jejunum, cou

Insights into the influence of dog and guardian demographics, nutrition, and relationship on raw feeding practices.

INTRODUCTION: Raw meat-based diets (RMBD) are increasingly popular among dog guardians, yet the role of personal nutrition habits and pet-guardian relationships in their adoption remains unclear. This study examines how these factors, alongside previously identified drivers of pet food selection, specifically relate to the decision to feed RMBD. METHODS: Data from 433 dog guardians were collected via an e-survey, from which 46 questions were selected for analysis in this study. Participants were placed into raw or cooked groups based on their dogs' primary diet. Responses were assessed using

Impact of Intravenous Fluids and Enteral Nutrition on the Severity of Gastrointestinal Dysfunction: A Systematic Review and Meta-analysis.

INTRODUCTION: Gastrointestinal dysfunction (GDF) is one of the primary causes of morbidity and mortality in critically ill patients. Intensive care interventions, such as intravenous fluids and enteral feeding, can exacerbate GDF. There exists a paucity of high-quality literature on the interaction between these two modalities (intravenous fluids and enteral feeding) as a combined therapy on its impact on GDF. AIM: To review the impact of intravenous fluids and enteral nutrition individually on determinants of gut function and implications in clinical practice. METHODS: Randomized controlled

Incident HIV among pregnant and breast-feeding women in sub-Saharan Africa: a systematic review and meta-analysis.

OBJECTIVES: A previous meta-analysis reported high HIV incidence among pregnant and breast-feeding women in sub-Saharan Africa (SSA), but limited evidence of elevated risk of HIV acquisition during pregnancy or breast-feeding when compared with nonpregnant periods. The rapidly evolving HIV prevention and treatment landscape since publication of this review may have important implications for maternal HIV incidence. DESIGN: Systematic review and meta-analysis. METHODS: We searched four databases and abstracts from relevant conferences through 1 December 2018, for literature on maternal HIV in

Familial psychosocial risk classes and preschooler body mass index: The moderating effect of caregiver feeding style.

BACKGROUND: Early child weight gain predicts adolescent and adult obesity, underscoring the need to determine early risk factors affecting weight status and how risk factors might be mitigated. Socioeconomic status, food insecurity, caregiver depressive symptomology, single parenthood, and dysfunctional parenting each have been linked to early childhood weight status. However, the associations between these risk factors and children's weight status may be moderated by caregiver feeding styles (CFS). Examining modifiable factors buffering risk could provide key information to guide early obesit
