Sonographic assessment of the extra-abdominal fetal small bowel in gastroschisis: a retrospective longitudinal study in relation to prenatal complications.

OBJECTIVE: An explorative retrospective study following a case-series of fetuses with isolated gastroschisis, to evaluate if small-bowel dilatation may be indicative for emerging obstetric complications. The secondary aim was to establish preliminary normative curves for the external diameter and wall thickness of eventerated fetal small bowel in gastroschisis during the second and third trimester of pregnancy. METHODS AND MATERIALS: Fourteen fetuses with isolated gastroschisis were followed at a single center. Repeated ultrasound examinations for fetal surveillance with measurement of fetal

Ultrasonically guided transabdominal encephalocentesis.

Transabdominal craniocentesis for the decompression of an advanced hydrocephalic fetal skull was performed under high-speed, sector scanning ultrasonic guidance. This technique can be used to facilitate delivery of a hydrocephalic fetal skull via the vaginal or abdominal route. The ability to monitor internal landmarks while advancing the needle is a decided advantage over the fluoroscopic technique.

Legionnaire's disease complicating pregnancy: a case report with intrauterine fetal demise.

OBJECTIVE: Legionnaire's disease complicating pregnancy is an unusual event that can seriously compromise both the mother and the fetus. CASE REPORT: We describe one case of such association, with an unfavourable intrauterine fetal outcome, secondary to acute placental insufficiency, related to infection. DISCUSSION: It is important in these high risk pregnancies complicated by acute pneumonia to take into consideration the diagnosis, as early as possible, and the appropriate treatment or the careful monitoring of fetal wellbeing.

Effects of magnesium sulfate on toxemic patients in labor.

One hundred forty-four toxemic patients were studied by continuous electronic maternal and fetal monitoring. Ninety-seven were treated by intravenous bolus of 2 g MgSO4 every 1-2 hours, and 47 by intravenous drip MgSO4 at a rate of 1 g/hour. Clinical doses of MgSO4 do not affect maternal ECG, temperature, uterine contractions, or fetal heart rate. Intravenous bolus administration is frequently associated with slowing of maternal respirations and hypotension and occasionally with transient apnea.

Genetic and maternal environmental contributions to estimated fetal weight at 20 weeks gestation compared with birthweight.

INTRODUCTION: Birthweight reflects fetal growth from conception. The complications arising from the extremes of fetal growth are well known, but addressing these effectively depends on understanding how and when fetal growth is affected by various maternal and fetal factors. We aimed to compare associations of fetal genetic and maternal environmental factors with fetal weight mid-pregnancy and birthweight. MATERIAL AND METHODS: We studied 3110 mother-child pairs from three population-based birth cohorts (UK and China). Estimated fetal weight at ~20 weeks gestation (EFW20) and birthweight were

Abnormal fetal growth patterns. Ultrasonic diagnosis and management.

Diagnostic ultrasound is a useful tool in the management of high-risk pregnancies and provides valuable information about abnormal fetal growth patterns including excessive fetal size and intrauterine growth retardation. Two patterns of growth retardation can be distinguished ultrasonically. Fetuses exhibiting "reduced growth potential" type patterns have little risk of fetal distress and are readily recognized as small for gestational age at birth. Fetuses suffering from uteroplacental insufficiency frequently exhibit a pattern of biparietal growth arrest in the third trimester. These fetuses

Anesthesia for non-obstetric surgery in the pregnant patient.

Surgery during pregnancy is relatively common. The present review of the literature will focus on relevant issues such as maternal safety during non-obstetric surgery in pregnancy, teratogenicity of anesthetic drugs, the avoidance of fetal asphyxia, the prevention of preterm labor, the safety of laparoscopy, the need to monitor the fetal heart rate and will finally give a practical approach to manage these patients.

A proposed novel method for CHD screening by fetal heart murmur detection using phonocardiography.

The paper presents a novel screening method to indicate congenital heart diseases (CHD) which otherwise would remain undetected because of their low level. Therefore, not belonging to the high-risk population, they are omitted from the regular fetal monitoring with ultrasound echocardiography. Based on the fact that CHDs are morphological defects of the heart causing turbulent blood flow, this turbulence appears as a murmur, which can be detected by phonocardiography (PCG). The proposed method applies measurements on the maternal abdomen and from the recorded sound signal a sophisticated proce…

Error traps in anesthesia for fetal interventions.

A wide range of fetal interventions are being performed worldwide to save the fetus's life, prevent permanent fetal organ damage, and allow a successful transition to extrauterine life. However, these are invasive procedures and can be associated with serious complications. This article focuses on promoting a culture of safety by highlighting five common error traps while anesthetizing patients for fetal interventions. They include failure to preserve uteroplacental perfusion and gas exchange, failure to achieve adequate uterine relaxation prior to hysterotomy, failure to monitor the fetus and

Variation in prenatal surveillance and management of anti-SSA/Ro autoantibody positive pregnancies.

OBJECTIVE: To describe international surveillance and treatment strategies for managing anti-SSA/Ro autoantibody positive pregnancies. STUDY DESIGN: An electronic REDCap questionnaire was distributed to Fetal Heart Society and North American Fetal Therapy Network members which queried institution-based risk stratification, surveillance methods/frequency, conduction abnormality treatments, and postnatal anti-SSA/Ro pregnancy assessment. RESULTS: 101 responses from 59 centers (59% US, 17% international) were collected. Most (79%) do not risk stratify pregnancies by anti-SSA/Ro titer; those tha

Quick measurement of serum unconjugated estrogens, and its clinical application to abnormal pregnancies and ovarian follicle maturation.

Using an automated HPLC system reported recently by us (J Clin Lab Anal 4:410-413, 1990), serum unconjugated estriol and estradiol (u-E3 and u-E2) can be measured within 22 min. The clinical value of this method was evaluated in this study with abnormal pregnancies, twin pregnancies, and pregnancies associated with fetal anencephaly or complicated by gestational toxicosis or diabetes mellitus. A significant correlation was demonstrated between an abrupt decrease in serum u-E3 level and an abnormal pattern on fetal heart rate monitor. Since urinary E3 was not sensitive enough to detect any depr

Rapid detection of fetal compromise using input length invariant deep learning on fetal heart rate signals.

Standard clinical practice to assess fetal well-being during labour utilises monitoring of the fetal heart rate (FHR) using cardiotocography. However, visual evaluation of FHR signals can result in subjective interpretations leading to inter and intra-observer disagreement. Therefore, recent studies have proposed deep-learning-based methods to interpret FHR signals and detect fetal compromise. These methods have typically focused on evaluating fixed-length FHR segments at the conclusion of labour, leaving little time for clinicians to intervene. In this study, we propose a novel FHR evaluation

Fetal and Neonatal Reticulocyte Count Response to Intrauterine Transfusion for the Treatment of Red Blood Cell Alloimmunization.

Management of hemolytic disease of the fetus and newborn relies on monitoring of maternal antibody titers, fetal ultrasound, and fetal middle cerebral artery peak systolic velocity studies and is generally treated by intrauterine transfusion (IUT). Few studies have explored fetal and neonate physiological responses to IUT. Our objective was to examine fetal erythropoietic response and to examine neonatal erythropoietic effects after treatment. Thirty-six patients treated from 2005 to 2015 were identified retroactively. The time course of treatment, including gestational age and number of IUT,

Misidentification of maternal for fetal heart rate patterns after delivery of the first twin.

BACKGROUND: The 2nd born in twin delivery is at higher risk of fetal asphyxia. We tested the hypothesis that inadequate, fetal heart rate (FHR) monitoring has contributed to an inadvertent outcome. STUDY DESIGN: A database was reviewed consisting of 41 twin deliveries with a pH in the umbilical artery of the 2nd twin below 7.05. Cardiotocograms were examined for possible signs of a maternal instead of a fetal origin of the heart rate pattern. RESULTS: In four of the reviewed cases, a good outcome had been anticipated by the attending physician based on the cardiotocogram, whereas the 2nd twi

A case of monochorionic twin pregnancy complicated with intrauterine single fetal death with successful treatment of intrauterine blood transfusion in the surviving fetus.

We report a case of monochorionic twin pregnancy complicated with single fetal demise that received successful treatment of intrauterine transfusion for severe anemia of the surviving fetus. A single fetal demise occurred at 20 weeks of gestation and middle cerebral artery peak systolic velocity (MCA-PSV), a marker for fetal anemia, showed marked elevation in the surviving fetus. Fetal blood sampling was immediately done and severe fetal anemia (hemoglobin = 5.5 g/dl, hematocrit = 16.8%) was confirmed, and then intrauterine transfusion was performed. After transfusion, MCA-PSV rapidly decrease

[In utero treatment of fetal anemia].

Fetal anemia may be central or peripheral and results in fetal hyperflow that progresses to hydrops, a state of generalized edema (a therapeutic emergency because it carries a high risk of fetal death). Diagnostic suspicion is based on ultrasound measurement of peak systolic velocity in the middle cerebral artery, which is then increased. The primary cause is anti-D alloimmunization, followed by parvovirus B19 and then anti-Kell alloimmunization, both of which are associated with the risk of early anemia. Pregnancies at risk for fetal anemia should be monitored by a fetal medicine center. In u

[Indications and technic of operative vaginal delivery in vertex presentation].

In operative vaginal delivery in vertex presentation we pay attention to a separation in the indication for using a forceps or using a vacuu … 4.1% and of vacuumdeliveries was 0.4%. Caused by a separation of the indication and by a consequent fetal monitoring there was an improvement in fetal and maternal morbidity and mortality. Using a handle in forceps delivery the procedure is more easy and less harmful for the baby.
