Hand Osteoarthritis in the Elderly: The Prevalence of Articular Cartilage Defects in Radiographically Normal and Affected Joints.

Background: Hand osteoarthritis (OA) is a highly prevalent disease that significantly impairs quality of life among many patients. The direct evaluation of cartilage defects associated with OA in vivo is challenging, and indirect assessments using X-ray images are commonly employed. The aim of this study was to evaluate the relationship between X-ray images of finger joints and cartilage defects. Methods: This study included 42 hands from cadavers that were fixed with alcohol and formalin. After X-ray posteroanterior images of all the finger joints were taken, the extent of the cartilage defec…

Osteocutaneous flaps for thumb and digit reconstruction. In unique situations.

Osteocutaneous flaps from the foot are being utilized more for thumb and digit reconstruction. We have presented a few unique situations in which these flaps are beneficial. Not all patients with deformities similar to those described in this presentation should have these reconstructive procedures. The patient must understand the risks of donor site morbidity and possible complications. However, the advantages of this technique are that only one surgical procedure is necessary, the hospitalization is short, the cosmetic result is good, and the functional restoration is satisfactory.

Reconstruction of the thumb amputation at the carpometacarpal joint level by groin flap and second toe transfer.

INTRODUCTION: Traumatic loss of thumb at the carpometacarpal (CMC) joint level is very disabling to an individual. Pollicisation is the recommended technique of reconstruction for loss of thumb at this level. On occasions, injury to the rest of the hand or amputation of additional fingers may make pollicisation an impossible option. Microsurgical transfer of second toe is an option in such situations. Although many large series of toe transfers are available in the literature, no series deals exclusively with this subset of patients. MATERIALS AND METHODS: Eight patients who had amputation of

Augmenting natural skin stretch during active finger motion reshapes hand proprioception.

There is strong evidence that skin stretch at the joint contributes to proprioception, the perceptual representation of body position and motion. Previous studies on fingers and hand dorsum focused on illusory movement elicited by skin-stretch stimuli when no actual motion was performed, often combined with local anesthesia and mostly delivered qualitatively, leaving unclear how controlled, augmented skin deformation influences proprioception during active movement. Here, we addressed this question by applying precisely scaled skin-stretch stimuli across the proximal interphalangeal (PIP) join

Friction of the gliding surface. Implications for tendon surgery and rehabilitation.

Finger flexor tendon rehabilitation has come a long way, but further advances are possible. Ideally, a healing tendon should move, but under the minimum load necessary to achieve motion. It is possible to design suture repairs that minimize the friction between tendon and sheath while simultaneously maintaining adequate strength to provide a wide margin of safety during therapy. A looped, four-strand modified Kessler repair is a good example of this type of high-strength, low-friction repair. At the same time, rehabilitation methods can also be optimized. A new modified synergistic motion prot

Proximodistal interphalangeal arthrodesis of the little finger: A series of 7 cases.

As an alternative to amputation of the little finger, we report here seven cases of shortening arthrodesis carried out by resecting the middle phalanx and proximodistal interphalangeal (PDIP) arthrodesis. Our cohort consisted of 6 males and 1 female (58years average age), with a stiff little finger secondary to Dupuytren's disease or trauma. All fingers were approached dorsally; after resection of the middle phalanx and decortication of the subchondral bone, fusion of the remaining phalanges was performed using an intramedullary self-breaking screw-pin. At a mean follow-up of 34.9months, pain

Experience with the pedicled latissimus dorsi flap for finger reconstruction.

The pedicled latissimus dorsi flap was used for the reconstruction of finger function in 12 patients with severely disabled hands. The fascial origin of the muscle was sutured directly to finger flexors in 6 patients, and to extensors in the other 6 patients in the forearm without severing the neurovascular pedicle. Voluntary contraction of the muscle was obtained in all patients. Useful tendon gliding was achieved in 6 patients, thereby providing total active motion of fingers of >100 degrees. In the remaining 6 patients, however, several problems such as stiff finger joints prevented appropr…

Flexor digitorum superficialis tendon transfer for intrinsic replacement. Long-term results and the effect on donor fingers.

This study is a review of 127 hands in 100 patients in whom one or two FDS tendons were used to correct claw-hand deformity and/or loss of opposition of the thumb. In lumbrical replacement the results were graded as excellent in 16 hands (21%) and good in 43 hands (57%). For opponensplasty the results were excellent in 26 hands (32%) and good in 42 hands (51%). Possible defects that can develop in the donor finger are: swan-neck deformity, flexion posture of the DIP joint, not as part of the swan-neck deformity, check-rein deformity or flexion contracture, and insufficient finger flexion. [New methods of joint replacement in the hand yield better function and pain relief].

Arthroplasty of the hand has hitherto been performed with rather simple methods such as the interposition of soft tissue or silicone implants. Surface replacement techniques, both cemented and cementless, currently routine methods in orthopaedic surgery, are becoming more and more commonly used in hand surgery. The article consists in a review of old and new methods for the replacement of wrist and finger joints, and a report of the results of osseo-integrated titanium implants in metacarpophalangeal (MCP) and proximal interphalangeal (PIP) joints, and of short-term outcome of non-constrained …

Open Reduction and Internal Fixation of Dorsal Fracture-Dislocation of the Proximal Interphalangeal Joint Using a Plate: A Series of 37 Patients.

Background: Open reduction and internal fixation with a plate is one of the alternative treatments for fracture-dislocation of the proximal interphalangeal (PIP) joint. However, it does not always lead to satisfactory results. The aim of this cohort study is to describe the surgical procedure and discuss the factors affecting the treatment results. Methods: We retrospectively reviewed 37 cases of consecutive unstable dorsal fracture-dislocation of the PIP joint treated using a mini-plate. The volar fragments were sandwiched with a plate and dorsal cortex, and screws were used as subchondral su

Osteointegration of a modular metal-polyethylene surface gliding finger implant: a case report.

INTRODUCTION: Primary press fit and secondary osteointegration is a precondition for component anchoring in articular surface replacements, also in the case of proximal interphalangeal (PIP) joint. Nevertheless, many existing prostheses for the PIP joint have failed to show sufficient osteointegration. CapFlex-PIP(©) implant is a modular metal-polyethylene surface replacement for the PIP joint consisting of a proximal and distal component each having a titanium pore backside, which allows secondary osteointegration at the bone-implant interface. To evaluate osseous integration of this implant,

Magnetic resonance imaging of the collateral ligaments of the thumb.

Injury to the ulnar collateral ligament of the metacarpal phalangeal joint of the thumb can usually be diagnosed on the basis of a combination of history, radiographic examination, and clinical stress testing of the joint when appropriate. Occasionally, when the patient is seen late and the diagnosis is doubtful, stress evaluation may be ill-advised because a nondisplaced ligament may be displaced in the process. We had recent experience with three patients in whom the use of magnetic resonance imaging was helpful in diagnosing a displaced ulnar collateral ligament in two cases, and an intrali

Predictive Force Control of Fingertip Induced by Functional Electrical Stimulation Based on a Hill-Type Muscle Model.

Functional electrical stimulation (FES) is an effective technique for restoring or enhancing hand motor function in patients with neurological impairments, such as those recovering from stroke or spinal cord injuries. Although many studies have used phenomenological models to investigate the control of FES, few studies have simultaneously employed both methods to study finger output force. This study aims to accurately predict finger output force using the Hill model and a multi-joint finger model under different current conditions. The Hill model describes the mechanical properties of muscles

[Thumb and finger reconstruction with the pedal digit transplantation: 541 cases report].

Thumb and finger reconstruction by the method of pedal digit transplantation had been successfully performed in 541 casee from 1977 to 1996, which contained 404 cases of thumbs and 78 cases of fingers. The thumb reconstruction was mainly the simple transplantation of distal phalanx (42 cases) and the compound transplantation of hallucal nail-cutaneous flap with iliac bone segment (16 cases) for the defect of thumbs in degree 1 and 2. The combined transplantation of hallucal nail-cutaneous flap with the joint and tendons of the second toe (34 cases) and the transplantation of the distal part of

Involvement of the Interosseous and Lumbrical Muscle-Tendon Units in the Lateral and Spiral Cords in Dupuytren's Disease of the Middle Fingers.

BACKGROUND: The nature of intrinsic muscle involvement in Dupuytren's disease of the middle fingers (long and ring) remains poorly characterized. Over the years, the authors have observed that both the spiral and lateral digital cords in the middle fingers receive contribution from intrinsic muscle-tendon units. This report describes the anatomical characteristics and frequency of intrinsic muscle-tendon unit involvement in Dupuytren's disease of the middle fingers. METHODS: Intrinsic muscle involvement in the middle digits was recorded in the operative reports of patients undergoing Dupuytre

Pitting scars in progressive systemic sclerosis.

The digital pitting scar is a common clinical feature in patients with progressive systemic sclerosis (PSS). Its pathogenesis is unclear, bu … ically and pathogenetically, they were similar clinically. The scar locations on the fingertips and finger joints (proximal interphalangeal and metacarpophalangeal) suggest that exogenous trauma, vibration injury and/or cold exposure may play a role in their pathogenesis.
