External monitoring of buried radial forearm free flaps in hypopharyngeal reconstruction.

CONCLUSION: The radial forearm free flap (RFFF) is one of the optimal choices for hypopharyngeal reconstruction. Our series demonstrates that the technique of an indicator flap for the monitoring of the buried flap is simple, reliable, and inexpensive. The condition of the indicator flap can be easily interpreted by the physicians and the nursing staff. Therefore, the success rate of this microsurgical reconstruction may be improved. OBJECTIVES: The RFFF is increasingly applied in reconstruction of the hypopharynx after radical resection for advanced hypopharyngeal cancer. However, postoperat…

Corrective osteotomy in forearm fracture malunion improves functional outcome in adults.

Malunions of forearm fractures in adults can present with limitation of forearm rotation, or as distal radioulnar joint instability with functional impairment. This contrasts with paediatric patients in whom malunions of similar severities are often better tolerated. We did a retrospective review of six adult patients after corrective forearm osteotomy for symptomatic malunited forearm shaft fractures. The corrective operations were done between 7 and 168 months after initial injury, using oblique or wedge osteotomies. Median follow-up was 22.5 months. The patients recovered well, with statist…

Paediatric diaphyseal forearm refractures after greenstick fractures: operative management with ESIN.

BACKGROUND: One of the complications of forearm shaft fracture is refracture. Elastic stable intramedullary nailing represents an alternative method for refracture treatment to cast immobilisation for another five to seven weeks. Operative treatment often necessitates an open reduction in most cases due to blocked or narrowed medullary canals. The purpose of this study was to examine the expense of the operative procedure, technique (closed or open intramedullary nailing) and postoperative complications in diaphyseal forearm refractures. METHODS: We retrospectively examined the expense of ope…

Prefabricated bony radial forearm flap for secondary mandible reconstruction after radiochemotherapy.

BACKGROUND: Primary reconstruction of the mandible is the golden standard of surgical treatment after ablative tumor surgery. Many different microvascular bone grafts are used to reduce wound healing complications at the severely compromised recipient site. The loss of primary grafts due to radiotherapy or osteoradionecrosis can make secondary mandibular reconstruction necessary. To address this problem, we developed the technique of the prefabrication of a radial forearm flap with cancellous bone. The aims were to establish these techniques into the clinical routine and to create a safe and r…

Insertion loads and forearm muscle activity during flexible hose insertion tasks.

OBJECTIVE: To quantify the physical demands of hose insertion tasks in automotive assembly operations and how they are affected by method an … the axial force attributable to twisting may be helpful during an insertion, despite the fact that forearm muscle activity was highest for both male and female participants during twisting insertions. APPLICATION: The results of this study can be applied to the future design of tasks that involve the joining of two parts such as a hose and flange.

Adipofascial radial forearm flap after failed surgical treatment of lateral epicondylitis.

Secondary cases of lateral epicondylitis after failed surgical treatment are a particular therapeutic challenge. Excision of the scar tissue and muscle transposition have been advocated as alternative procedures, although the optimal treatment has yet to be established. We present one desperate case of failed surgical treatment of lateral epicondylitis, where an adipofascial radial forearm flap was used successfully to alleviate the patient's symptoms.

[Reconstruction of hypopharynx after pharyngolaryngectomy with U-shaped radial forearm flap].

BACKGROUND: After pharyngolaryngectomy reconstructive procedures of the upper digestive tract are necessary. Since the introduction of microvascular tissue transplantation jejunal loops proved to be a versatile transplant. Nevertheless it has some disadvantages as i. e. the increased donor morbidity, a rather high sensitivity to hypoxia and the tendency of shrinkage and formation of stenosis. METHOD: We perform the reconstruction of the hypopharynx with the radial forearm flap. The fasciocutaneous flap is harvested on the radial artery in a size of 12 x 6 cm in average and is sutured U-shaped…

Electrophysiological study of forearm sensory fiber crossover in Martin-Gruber anastomosis.

Although anatomical studies have shown that a crossover of sensory fibers is not rare in forearm Martin-Gruber median-ulnar anastomosis (MGA), it has been electrophysiologically described only in rare subjects. Using a near-nerve needle technique, the possibility of electrophysiologically detecting a forearm median-ulnar crossover of sensory fibers was investigated in 24 arms of 21 subjects with unilateral or bilateral MGA, by stimulating the fifth digit of the hand and recording along the median nerve. Small-amplitude elbow responses were found in the median nerve in 10 of the 24 arms but, in…

Metallosis mimicking osteomyelitis from a forearm plate retained for 50 years.

A forearm plate retained for 50 years was removed because an acute inflammation mimicked chronic infection. The radiographs were very confusing. During the operation metallosis was obvious. Despite the general opinion that forearm plates should be kept in place, this probably should not apply to old "historical" hardware of uncertain chemical composition.

Changes in bone mass and bone turnover following distal forearm fracture.

Bone loss occurs close to a fracture and is associated with increased bone turnover. Fracture healing itself results in increased markers of bone turnover. But the exact patterns of these changes after different fractures are unclear. We aimed to investigate the changes in bone density and biochemical markers following distal forearm fracture. Twenty women (mean age 63 years) were recruited following fracture of the distal radius and ulna. Bone mineral density (BMD) of the hand and forearm were measured by dual-energy X-ray absorptiometry (DXA) and quantitative ultrasound (QUS) of the fingers …

Results of reconstruction with free forearm flap following laryngopharyngo-oesophageal resection.

The main challenge after total laryngopharyngeal and cervical oesophageal resection for laryngopharyngeal carcinoma, is to provide a suitable feeding conduit which improves the quality of life for the patient. Réconstruction with a tubed free forearm flap can solve the feeding problem to some extent, providing a thin and pliable fasciocutaneous conduit that accommodates well with the thin mucosa of the pharynx and oesophagus. In this study immediate reconstruction of the pharyngo-oesophagus by free forearm flaps is reported in nine patients with advanced laryngopharyngeal and oropharyngeal car…

Surgical treatment of isolated forearm non-union with segmental bone loss.

Twenty-four isolated radius and ulna non-unions with segmental bone loss were operated on. Nine non-unions were in the radius, 15 in the ulna. The surgical technique consisted of removal of the necrotic bone, filling of the bone defect with an intercalary bone graft and internal fixation with a cortical bone graft fixed opposite to a plate. The average length of bone defect after freshening of the bone ends was 3.6 cm (range 2-10.5 cm). The average length of follow-up was 90 months. In 23 cases union was achieved. In three cases a postoperative infection developed. Resolution of the infection

Reorganization of human cortical motor output maps following traumatic forearm amputation.

We report the results of serial transcranial magnetic stimulation mapping of cortical motor outputs to the face and upper extremity in a subject studied before and repeatedly after traumatic amputation of the right arm immediately below the elbow. The results of the mapping studies illustrate the time course of plastic changes in the motor cortical representation in humans following a traumatic amputation and allow the correlation of subjective perceptions of phantom limbs with the reorganization of cortical outputs.

Recombinant human insulin-like growth factor-I increases forearm blood flow.

Several recent reports have described systemic effects associated with insulin-like growth factor-I (IGF-I) infusions in humans, including dizziness, syncope, unconsciousness, bradycardia, and asystole. Eleven healthy volunteers (10 males and 1 female) underwent brachial arterial and deep forearm venous catheterizations to evaluate the effects of an intraarterial infusion of IGF-I on blood flow. Saline was infused into the brachial artery for 1 h, followed by infusions of recombinant human IGF-I (1.25, then 10 micrograms/kg BW.h) over 2 subsequent 90-min periods. Blood flow across the forearm …

Salt loads attenuate potassium-induced vasodilation of forearm vasculature in humans.

To evaluate the role of the sodium pump in resistance control in vivo, we studied vascular responses to potassium, which produces vasodilation by the activation of vascular Na+, K(+)-ATPase, in normotensive volunteers receiving a high salt diet compared with volume-depleted subjects receiving diuretic treatment. Forearm blood flow was measured by strain-gauge plethysmography during small increments in local concentrations of potassium with intrabrachial arterial infusions of KCl. Infusions of 0.12 and 0.24 mEq/min KCl increased forearm blood flow and decreased forearm vascular resistance in a …

Refinement of the radial forearm flap donor site using skin expansion.

The radial forearm flap has proven to be versatile for free vascularized composite tissue transfers as well as for ipsilateral upper extremity reconstructions that require no microsurgical expertise. The most common objection to this otherwise advantageous donor area has been the subsequent nonaesthetic donor-site deformity. In addition, skin grafts frequently fail over the flexor carpi radialis tendon leading to chronic skin breakdown or at best tendon adhesions. Both these concerns may be ameliorated by means of tissue expansion of the remaining dorsal forearm skin to then allow removal of t…
