Failed internal fixations for trochanteric fractures have a strong negative impact owing to increased postoperative mortality and high medical costs. However, evidence on the prognostic value of postoperative radiographic findings for failed internal fixations is limited. We aimed to clarify the association between comprehensive immediate postoperative radiographic findings and failed internal fixation using relative and absolute risk measures. We followed the meta-analysis of observational studies in epidemiology guidelines and the Cochrane handbook. We searched specific databases in November

We present the case of an 82-year-old female who had difficulty walking due to right thigh pain caused by incomplete atypical femoral fracture (AFF). The femoral bowing was so severe that intramedullary nail insertion was impossible, so we performed a corrective osteotomy of the femur and inserted the intramedullary nail. Postoperatively, the femoral pain disappeared, and bone fusion was achieved at one year and two months postoperatively. In cases of incomplete AFF with very severe femoral bowing, internal fixation with an intramedullary nail combined with corrective osteotomy of the femur is

BACKGROUND: Unstable femoral neck fractures with medial calcar defects are difficult to manage. The optimal fixation methods for these fractures have been a subject of ongoing debate among orthopedic surgeons. In this study, three different fixation techniques for vertical, medial defected femoral neck fractures were compared.
METHODS: In this study, a biomechanical analysis was conducted to compare three fixation methods: cannulated screws (Group 1), cannulated screws combined with a medial buttress plate (Group 2), and intramedullary nails (Group 3). Synthetic composite bone models represen

Since the late 1950s, open reduction and internal fixation has been advocated to restore bone anatomy and enable early mobilization. This approach often necessitated extensive dissection and tissue devitalization, creating an environment less favorable for fracture union and more prone to bone infection. As a result, other methods, such as intramedullary nailing, have become the standard treatment for most diaphyseal fractures of the femur and tibia. However, internal fixation with plates and screws remains the treatment of choice for most periarticular fractures and other complex fractures in

PURPOSE: Osteogenesis imperfecta (OI) is a condition characterised by bone fragility and multiple fractures, which cause considerable morbidity in the affected patients. Most cases are associated with mutations in one of the type I collagen genes. Recently, bisphosponates have been used widely to reduce pain and the incidence of fragility fractures in OI in children, even though there have been concerns raised regarding the long-term complications of it due to their effect on the bone. The fragility fractures involving the neck of the femur in children with intramedullary rods in the femoral s

CASE: Gorham-Stout disease (GSD) is a rare disorder marked by massive bone destruction. A 56-year-old woman with GSD presented to our hospital with a proximal femur fracture. She was initially diagnosed with a simple fracture and underwent open reduction and internal fixation using an intramedullary nail. However, the delayed diagnosis of GSD resulted in postponed appropriate treatment. Although the intramedullary nail helped prevent mechanical bone failure, extensive osteolysis continued, ultimately necessitating total femur replacement.
CONCLUSION: This case highlights the importance of con

Simultaneous diaphyseal fractures of the radius and ulna, often referred to as both-bone forearm fractures, are frequently encountered by orthopaedic surgeons. Adults with this injury are typically treated with open reduction and internal fixation because of the propensity for malunion of the radius and ulna and the resulting loss of forearm rotation. Large case series support the use of plate and screw fixation for simple fractures. More complex fractures are managed according to strain theory, with the intention of controlling rather than eliminating motion at the fracture site. This can be

OBJECTIVE: To compare the effectiveness of dynamic fixation and plate augmentation combined with bone grafting for femoral nonunion after interlocking intramedullary nails.
METHODS: Between January 2008 and December 2022, a total of 128 patients who developed femoral nonunion following static fixation with interlocking intramedullary nailing were retrospectively analyzed. All patients underwent either dynamic intramedullary nail fixation or plate fixation with bone grafting while retaining the original intramedullary nail. There were 104 males and 24 females;the age ranged from 19 to 59 years

Femoral midshaft fracture is one of the most common clinical injuries and is often caused by high-energy traffic accidents. Intramedullary nailings, plates, and external fixators are all used as treatment alternatives for a variety of patients depending on fracture location, displacement, comminution, soft tissue condition, and local tradition. Locked intramedullary nailing is currently the preferred treatment method for most diaphyseal fractures and has good clinical results. The goal of this study was to compare expandable and locked intramedullary nailing for the treatment of AO type 32A an

Management of tibia fractures by internal fixation, particularly intramedullary nails, has become the standard for diaphyseal fractures. However, for metaphyseal fractures or those at the metaphyseal-diaphyseal junction, choice of fixation device and technique is controversial. For distal tibia fractures, nailing and plating techniques may be used, the primary goal of each being to achieve acceptable alignment with minimal complications. Different techniques for reduction of these fractures are available and can be applied with either fixation device. Overall outcomes appear to be nearly equiv

We report on a 54-year old male with traumatic brain injury, flail chest and floating shoulder undergoing intramedullary stabilization of a midshaft clavicle fracture in beach chair position. Intraoperatively the patient developed instable atrial fibrillation triggered by implantation of intramedullary nail. Secondary this case shows feasibility of cardioversion in latero-lateral electrode-position due to inaccessible standard positions and patient fixation between the operation table and the X-ray apparatus.

PURPOSE: The purpose of this study was to compare fracture nonunion rates in tibial shaft fractures fixed with intramedullary nails using fixed angle locking screws (FALS) that thread into the intramedullary nail versus standard locking screws in a cohort of tibial diaphyseal fractures. Secondarily, this study evaluated whether FALS location was associated with nonunion risk in tibial diaphyseal fractures.
METHODS: A retrospective review of surgically treated OTA 42 A-C tibial shaft fractures from 2014 to 2024 at a single academic institution was performed. Inclusion criteria were age > 18, i

INTRODUCTION: Ankle fractures are a common orthopaedic injury that often require surgical fixation. Because the comorbid population in the United States continues to survive longer, it has become routine to treat comorbid patients with unstable ankle fractures. The literature has identified comorbidities known to increase the risk of ankle fracture complications to include age 55 years or older, body mass index >29.9, polytrauma, open fractures, diabetes mellitus, smoking, peripheral neuropathy, and alcohol use.
METHODS: We retrospectively reviewed 37 patients who received retrograde intramed

INTRODUCTION: Osteogenesis imperfecta (OI) is a rare inherited connective tissue disorder caused by defective type I collagen, which predisposes patients to recurrent fractures, skeletal deformities, and long-term disability. Intramedullary nailing is widely regarded as the gold standard for surgical stabilization of long bone fractures in OI due to its load-sharing capacity, deformity correction, and ability to prevent future fractures.
CASE REPORT: We describe the case of a 34-year-old female with type IA OI who presented with a spontaneous mid-shaft femur fracture while weight-bearing. She

BACKGROUND: Femoral shaft fractures are usually seen in the young population as a result of high energy traumas and are often accompanied by major organ injuries. In this paper, we aimed to assess the clinical results of expandable femoral intramedullary nails in the treatment of 20 femoral shaft fractures.
METHODS: The average age was 34.7. One fracture was the result of a gunshot wound, type 3A open fracture, and the other 19 fractures were closed. Under fluoroscopic control, all patients underwent elective closed reduction and internal fixation with intramedullary expandable femoral nails

INTRODUCTION: Closed repair of peritrochanteric fractures with cephalomedullary nail fixation with the patient in the lateral decubitus position on a flat table with manual traction may allow improved fracture reduction and fixation in comparison with what is possible in a supine setup.
STEP 1 OPERATING ROOM PREPARATION: Perform sterile preparation, have the patient brought to the operating room, induce anesthesia, and place the patient in the lateral decubitus position before sterile draping.
STEP 2 FRACTURE REDUCTION: Reduce the fracture using manual traction and slight internal or externa

OBJECTIVE: To investigate the clinical effect of the treatment of the ununited lower limb fracture with reamed intramedullary interlocking nail.
METHODS: Fifty-two patients with ununited lower limb fracture were treated with intramedullary interlocking nail (femur 36, tibia 16). The main cause of nonunion was interternal fixation loosening or fracture (41 cases, 78.8%), other reasons included bone defection, bone infection or too much traction of fracture (11 cases, 21.2%). According to X-ray classification: 41 cases in type hypertrophy, 11 cases in type atrophy. All these patients need opera
