Radial head fractures.

Fractures of the radial head are the most common fractures in the elbow, and they frequently have associated ligamentous, cartilaginous, or other bony injuries. Clinical assessment and radiological investigation allow for accurate diagnosis and the formulation of a management plan. Undisplaced or minimally displaced fractures with no rotational block to motion can be treated nonoperatively with excellent results expected. The minimum amount of displacement in a partial articular radial head fracture required for open reduction and internal fixation to provide a superior outcome to nonoperative

Complications and technical problems with the gamma nail.

A retrospective review of complications with the gamma nail has been carried out in 152 patients with stable and unstable pertrochanteric fractures. Operative complications occurred in 10 patients. Screws penetrated the femoral neck in 3, immediate varus was associated with a short screw in 2, diaphyseal fractures occurred at the site of the distal screws in 3 and comminuted femoral fractures in 2. Fifteen patients (10.5%) died in hospital. At follow up, pseudarthrosis was present in 2 cases and fractures of the femoral shaft in 4. The incidence of complications could be reduced by using of sh

Zygomatico-maxillary fractures. Late results after traction-hook reduction.

A simple, closed method for zygomatico-maxillary fracture treatment is described i.e. replacement by the use of a traction bone-hook. After X-ray and clinical examination had excluded undisplaced fractures, blow out fractures and comminuted fractures, the postreduction stability was assessed peroperatively and again one week after. Sophisticated preoperative fracture-classifications predicting postoperative stability appeared redundant. In the present study, this procedure could be performed in 55.4% of the total fractures which needed to be reduced. At re-evaluation, two to eight years later,

Predictors of nonunion after nonoperative treatment of displaced midshaft clavicle fractures.

BACKGROUND: Nonunion is a significant complication following nonoperative treatment of displaced midshaft clavicle fractures, potentially leading to impaired shoulder function, pain, and decreased quality of life. This study aims to identify predictors of nonunion in adults treated nonoperatively to optimize treatment decisions and improve outcomes. METHODS: A retrospective cohort study was conducted using data from 374 patients treated nonoperatively between 2012 and 2024. Patient and fracture characteristics, including age, sex, smoking, diabetes mellitus, and fracture comminution, were ass

Surgical Management of a Pediatric Cuboid Nutcracker Fracture: A Case Report.

CASE: Cuboid impaction fractures (nutcracker fractures) result from high-energy trauma and are exceedingly rare in children. We present a case of an 8-year-old boy who sustained a comminuted cuboid nutcracker fracture after a fall from height. The patient underwent open reduction and internal fixation with a locking plate. At 1 year postoperatively, lateral column length and articular congruency were maintained, and the patient return to full function and activity. CONCLUSION: Cuboid nutcracker fractures in children are rare and can be successfully treated with open reduction and internal fix

[The coexistence of fractures of the capitellum and the radial head: a rare case].

The coexistence of fractures of the capitellum and the radial head in the elbow joint is a rare entity. A 36-year-old man presented with complaints of pain, swelling, and limited range of motion in the right elbow after a fall. Radiography, computed tomography, and three-dimensional reconstruction of computed tomography images revealed displaced and comminuted fractures in the capitellum and the radial head. The fractures were treated with open reduction via a lateral approach and osteochondral fragments were fixed with Herbert screws. Clinical and radiographic results were excellent at the en

[Delayed fracture healing of the upper arm].

The analysis of 35 cases of delayed healing of fractures of the upper arm treated from 1981 to 1990 shows that fractures resulting from road accidents (16/35) and comminuted fractures (16/35) are particularly frequent in this group. Delayed fracture healing after conservative treatment (10 patients) was caused mainly by inadequate reduction (4/10) or by the fact that the conservative treatment performed was in fact inappropriate (4/10). Primary osteosynthesis was performed in 25 patients. Inadequate stabilization (9/25) was the most frequent reason for delayed healing, followed by the omission

External fixation of the upper extremity.

External fixation of forearm and arm fractures is an excellent treatment option, particularly in compound, comminuted fractures that result from high-energy trauma. Limb salvage and good function have been achieved in 75% of cases by applying the principles outlined. Problems with delayed bone union have been recognized and emphasize the importance of supplemental bone grafting. A number of external fixation systems are applicable to these upper limb fractures, and the authors have not found one system superior to the others. Open pin insertion is preferred, and pins (three proximal and three

Irreducible tibial pilon fracture caused by incarceration of the fibula in the tibial medullary canal.

Fractures can be irreducible for several reasons, including soft tissue or bone fragment interposition. We report an unusual fracture configuration of a comminuted tibial pilon fracture in which the distal fibular shaft fragment was occupying the medullary canal of the proximal tibial shaft fragment and inhibiting reduction and fixation. To the best of our knowledge, this has not been previously reported in a published study.

Radial head prosthesis after radial head and neck fractures - current literature and quality of evidence.

Due to the elbow joint's complex functional anatomy, the multifragmentary nature of many fractures and concomitant destabilizing associated injuries, dislocated fractures of the radial head and neck still present a serious challenge for the orthopedic surgeon. Thorough knowledge of the elbow's anatomy and biomechanics is essential to analyze and understand the injury and plan its treatment. The aim of a differentiated therapy approach is to restore the joint's anatomy and kinetics, stable and painless joint function, and to avoid or at least delay posttraumatic joint changes. The degree of dis

Tibial fractures associated with civilian gunshot injuries.

Forty patients with 41 fractures of the tibia produced by civilian gunshot injuries were reviewed. The usual patient was male, 28 years of age, and had been shot by an unknown assailant. Thirty-two of the 41 fractures (78%) were the result of low-energy missiles (less than 500 foot pounds or 680 Nm) while six (15%) were from intermediate (500 to 1,200 foot pounds or 680-1,627 Nm) and three (7%) from high-energy missiles (greater than 1,200 foot pounds or 1,627 Nm). Characteristic fracture patterns for the low energy group consisted of the drill hole, unicortical, oblique, spiral, butterfly, an

Use of surgical glue for Mason type III radial head fractures: A case report.

RATIONALE: A Mason type III radial head fracture, which is characterized by comminuted fragments of the radial head, is a severe injury. Open reduction and internal fixation (ORIF) is an alternative treatment method; however, the technique of using an on-table reduction in combination with surgical glue is rarely reported. PATIENT CONCERNS: A 48-year-old man was admitted to our department with complaints of elbow pain after falling down. Elbow radiography and computed tomography (CT) demonstrated characteristics of fractures before the operation. DIAGNOSIS: Radiographic images showed a Mason

Surgical management of a traumatic calcaneal fracture in a kitten using three-dimensional CT imaging.

CASE SUMMARY: A 4-month-old castrated male Birman kitten (2.2 kg) presented with a recent history of non-weightbearing lameness in the right hindlimb after blunt trauma. Significant orthopaedic examination findings included instability at the right proximal intertarsal joint level. Radiographic abnormalities included a medial slab fracture of the right calcaneus. Further evaluation with a three-dimensional CT scan revealed a comminuted, intra-articular calcaneal fracture. Images from three-dimensional CT fracture reconstruction were used for surgical planning. Partial tarsal arthrodesis was pe

[Over-reduction after distal radius fracture pinning in young active patients: Prevention by multiple mixed pinning].

Over-reduction is a classical complication following pinning of distal radius fractures. Indeed, the pinning techniques derived from Kapandji's technique do not allow anterior stabilization. A literature review and our experience show that it is an underestimated complication that can affect up to one-third of cases, but that is well tolerated if the anterior tilt is less than 20°. Excessive angulation of the dorsal intrafocal K-wires becomes a significant risk factor beyond 60°. Volar comminution of the fracture is an obvious predisposing factor. We will outline our ideas on the topic and pre

[Sternal fractures and their surgical treatment].

PURPOSE OF THE STUDY: Sternal fractures occur most frequently in vehicle accidents, then due to falls from a height and by other blunt chest trauma. Most of these injuries are simple, non-displaced fractures only rarely requiring surgical management. Based on a retrospective analysis, the authors present their experience with the treatment of sternal fractures, emphasizing the use of osteosynthesis. MATERIAL AND METHODS: A group of 293 patients treated for fresh sternal fractures in the period from 2004 to 2013 were evaluated. Their median age was 47.5 years (range, 2 to 86 years). They were

Flexible intramedullary nailing for metacarpal fractures.

A 5-year retrospective review of 83 patients with 98 metacarpal fractures was performed. Fractures of the thumb metacarpal were excluded. Ninety-six closed metacarpal fractures were reduced closed and fixed with flexible intramedullary fixation, using multiple 0.8-mm prebent rods. Two open metacarpal fractures were also fixed with this technique. Fractures amenable to flexible intramedullary fixation include short oblique and transverse fractures. Contraindications include long oblique and bicortical comminuted fractures. The average follow-up time was 9 months (range, 2 to 34 months) with 15

Management of fractures of the zygomaticomaxillary complex.

The zygomaticomaxillary complex (ZMC) has important aesthetic, structural, and functional roles that need to be preserved and/or restored during treatment of facial fractures. Surgical treatment of ZMC fractures is indicated when there is displacement of the bony fragments, and open reduction and internal fixation is the treatment of choice in cases of comminution or fracture instability. The surgical approaches used for fracture reduction as well as the type, number, and location of the fixation will be determined by the pattern of the fracture and the surgeon's preference.
