[Computer-assisted anatomical evaluation of the nasal sinuses in 1-2 years old children].

OBJECTIVE: To analyze the anatomical and developmental characteristics of nasal sinuses in 1-2 years old children; and provide potential evidence for the diagnosis and treatment of sinusitis in 1-2 years old children. METHOD: Coronal CT scans of 60 cases (1-2 years old children) were studied with the imaging station. Reference datas were identified on these images using imaging station. The gasification conditions of the sinuses were identified and measured. RESULT: 100% of the frontal sinuses were not developed, while 100% of the maxillary sinuses were developed. The mean of the transverse

Conventional sinus radiography compared with CT in the diagnosis of acute sinusitis.

OBJECTIVES: To evaluate the value of plain film radiography in a prospective investigation of patients with clinical suspicion of acute sinusitis, using standard CT as a gold standard. METHODS: 47 consecutive patients were examined. Each patient underwent conventional X-ray and standard dose CT examinations on the same day. The sensitivity and specificity of the plain film examination were calculated. RESULTS: The specificity of the plain film examination was high, but the sensitivity was low except for the maxillary sinus (sensitivity 80%). Thus, for maxillary sinusitis, plain film examinat

Pott's puffy tumor presenting as a frontal swelling under a Swiss army helmet.

In this report, we describe an uncommon presentation of a Pott's puffy tumor, which is defined as a subperiosteal abscess related to a chronic frontal sinusitis. This condition has become rare in our part of the world because of the widespread use of antibiotics. Clinical history, investigations, and management are presented.

Sinus Penetration of a Pulsating Device Versus the Classic Squeeze Bottle in Cadavers Undergoing Sinus Surgery.

OBJECTIVES: Nasal irrigation is standard in the management of chronic rhinosinusitis both before and after surgical intervention. Numerous irrigation devices are commercially available. The aim of this study was to compare the efficacy of a handheld pulse irrigation device against the gold standard manual squeeze bottle after endoscopic sinus surgery (ESS). METHODS: Five cadaveric specimens were prepared with video visualization ports into each sinus. Endoscopic sinus surgery was performed on each cadaver from minimal to maximal dissection. Sinuses were irrigated with fluorescein solution usi

Frontal epidural empyema (Pott's puffy tumor) associated with Mycoplasma and depression.

UNLABELLED: A 37-year-old male has left exophthalmia, which gradually evolved in the last two years, finally with a deviation of left eye (LE), down side and out, with gradually decrease of visual acuity (VA). These symptoms are accompanied with headache and psychiatric manifestations with irritability, decreased attention, anxiety, insomnia, depressed mood. Brain Computed Tomography (CT) shows a tumor mass in air leakage sinus, bilateral frontal and bilateral ethmoidal, with left orbital invasion. This tumor mass lysis by pressure the supero-posterior wall of the left orbit, with the delimita

Myoepithelial carcinoma of the orbit: a clinicopathological and histopathological study.

Two cases of invasive myoepithelial carcinoma arising from the paranasal sinuses and invading the orbit are presented. Patient 1, a 53-year-old man, had a 3-month history of proptosis, pain and epiphora of the right eye. The second patient, a 24-year-old man, had for a week been complaining of protrusion of his left eye and of orbital pain. Computed tomography scan and magnetic resonance imaging revealed tumour masses in the frontal, ethmoidal and maxillary sinuses with invasion of the orbit and the frontal lobe. Biopsies from both cases showed spindle and epithelioid tumour cells. Mitotic fig

Challenging the gold standard: methods of sampling for microbial culture in patients with chronic rhinosinusitis.

PURPOSE: Chronic rhinosinusitis (CRS) is a highly prevalent multifactorial disorder. Culture-directed antibiotics are frequently prescribed to patients with CRS and the middle nasal meatus (MM) is traditionally believed to be a representative sampling site of the sinuses as a whole. The purpose of our study was to reevaluate the reliability of the MM as a sampling site in patients with CRS who suffer from impaired drainage from the sinuses to the MM. METHODS: Swabs and tissue biopsies were collected from the MM, maxillary sinus and frontal sinus from 50 patients with CRS. The results of bacte

Investigation of sinonasal microbiome spatial organization in chronic rhinosinusitis.

BACKGROUND: Chronic rhinosinusitis (CRS) is a multifactorial inflammatory airway disorder in which bacteria are implicated in the initiation and/or sustenance of disease in some patients. The sinuses are colonized by bacteria even in health, and the potential for sinus-specific niches harboring unique microbial consortia raises questions for clinical and research investigation. The objective was to determine the degree to which resident upper airways microbiota differ between individuals and anatomic sites, in order to determine the optimal site of microbial sampling for study in CRS. METHODS

The endoscopic management of sphenoid and ethmoid mucoceles with orbital and intranasal extension.

Mucoceles of the sphenoidal and ethmoidal sinuses act as benign neoplasms and can result in bony erosion extending from within the confines of the sinuses into the intracranial and orbital spaces. Endoscopic management of such mucoceles has been debated, and, by some, considered a radical form of therapy. A review of consecutive patients with sinus mucoceles revealed eight sphenoid and six ethmoid mucoceles. Four of these were confined to the sinuses and 11 extended outside of the confines of the sinuses. There were four with intracranial extension, two with orbital extension, three with both

The tenacity of the frontal sinus mucosa.

A number of persons in other surgical disciplines express a blatant disregard for the potential dangers of frontal sinus mucosal entrapment. Although bone removal is universally recommended by the otolaryngologic proponents of ablative procedures in the frontal sinus, no comprehensive explanation has been proffered to justify this dictum. To this end, 27 cats had simple excision of the frontal sinus mucosa to see if it is necessary to bur away the inner table of bone to completely eliminate all vestiges of mucosa. Mucosal regrowth was seen in 72%, and suppurative infection, in 17% of the sinus

Fronto-ethmoidal osteoma. Open treatment.

INTRODUCTION: Osteoma is a benign tumour, composed of mature compact or cancellous bone, which can arise in any facial bone. Among the paranasal sinuses, the frontal and ethmoid sinuses are most frequently involved. Osteoma grows very slowly and small lesions are often not symptomatic. On the other hand, sometimes patients present with symptoms such as sinusitis, headache or ophtalmologic manifestations. These findings are secondary to obstruction of the involved sinus cavity with secondary mucocele formation or occasionally an expansile lesion with distortion of the facial contour. MATERIALS

Morphometric study of the paranasal sinuses in normal and pathological conditions.

Prospective and retrospective morphometric CT (axial and coronal) research was performed with 100 healthy persons and 163 patients (145 males, 118 females, mean age 50 years, range 1-88 years). The patients were classified into the following groups: chronic sinusitis (n = 85), polyposis (n = 25), mucoceles (n = 13), benign tumors (n = 20) and malignant tumors (n = 20). After initial calibration with the scale (in cm) displayed on the CT image, each paranasal sinus was outlined following its bone inner surface. The data were processed with a high-resolution analysis system, and volumes were cal

Impact of Draf III, Draf IIb, and Draf IIa frontal sinus surgery on nasal irrigation distribution.

BACKGROUND: Delivery of topical pharmacotherapy to the paranasal sinuses remains integral to the management of chronic rhinosinusitis. The frontal sinus remains a difficult access site for irrigations, often limited by its position relative to the nostril and ethmoid sinus. In view of the previous demonstration of improved frontal sinus irrigation with Draf III vs Draf IIa, in this work we sought to evaluate topical access of Draf IIb relative to Draf IIa and Draf III modification of the frontal sinus outflow tract. METHODS: Unfixed human cadaver heads were dissected using Draf IIa, Draf IIb,

Computed tomographic characteristics of frontal sinus ossifying fibroma in a dog.

A 10-year-old female spayed Kelpie cross was presented to The Austin Vet Specialists for further investigation of a mineralized, lobulated frontal sinus mass that had previously been detected radiographically. Computed tomography (CT) revealed a large, expansile, well-defined, heterogeneously mineral attenuating mass invading both frontal sinuses. The mass was surgically debulked via a frontal sinusotomy approach. Histopathology was consistent with ossifying fibroma. This  is the first published report to describe frontal sinus ossifying fibroma in a dog, and the second to describe CT features

Frontal Sinus Cholesteatoma Presenting with Intracranial and Orbital Complications: Diagnosis and Treatment.

Frontal sinus keratoma or cholesteatoma is a rare disease of paranasal sinuses and presents as a slow-growing mass that becomes symptomatic as it grows to the surrounding structures. Intracranial complications are not a common presentation and are potentially life-threatening. Frequently the final diagnosis is only made intraoperatively because several other frontal sinus tumors behave likewise. Definitive treatment requires complete removal of the keratoma, and a combined endoscopic and external frontal sinus approach is a good treatment option. In this report, we presented a 68-year-old fema

Trigeminal neurosensory changes following acute and chronic paranasal sinusitis.

OBJECTIVE: The aim of this study was to document sensory changes in the supraorbital, infraorbital, and mental nerve distributions following acute and chronic maxillary and frontal sinusitis. METHOD AND MATERIALS: Seven patients with a total of 14 infected sinuses were included in the study. Neurosensory function was evaluated by measuring the electrical detection threshold for large myelinated nerve fibers and heat detection thresholds for the assessment of the thin unmyelinated nerve fibers. The sensory tests were conducted in the infraorbital, supraorbital, and mental dermatomes. Patient e…

[Computed tomographic and endoscopic analysis of the supraorbital ethmoid cell anatomy].

OBJECTIVE: To understand the anatomy of the supraorbital ethmoid cell and its relationship with the frontal sinus drainage pathway. METHODS: Five patients (5 sides) who had supraorbital ethmoid cell underwent endoscopic frontal sinus surgery. Computed tomographic (CT) scans of the sinuses were obtained in coronal and axial views. The frontal sinus ostium and the supraorbital ethmoid cell were endoscopically identified respectively. RESULTS: On coronal CT scans, the supraorbital ethmoid cell was a separate cell lateral to the frontal sinus. And on axial CT scans, it was lateral and posterior

Type III frontal sinusotomy: surgical technique, indications, outcomes, a multi-university retrospective study of 120 cases.

Draf in 1991. The procedure--which is also known as the modified endoscopic Lothrop procedure--aims to create the largest possible anteroposterior and lateral to lateral opening between both frontal sinuses and the nasal cavities. This requires the resection of the medial floor of both frontal sinuses, the intersinus septum and the superior nasal septum. The authors present a retrospective study including a cohort of 120 patients who underwent surgery in six Belgian university ENT departments. Mean follow-up was 24.6 months (range: 5-36 months). This paper describes the surgical procedure and
