The morphological features of a functional parathyroid carcinoma in a 34-year-old female are presented. The case was unique in the high frequency of mitotic figures permitting a tentative diagnosis of malignancy already on frozen sections. The parathyroid nature of the lesion was established ultrastructurally by demonstration of secretory granules, glycogen particles and lipid material within the cytoplasm. BACKGROUND: Myxofibrosarcomas are notoriously highly infiltrative soft-tissue sarcomas, making negative surgical margins difficult to obtain. Recently, vacuum-assisted closure (VAC) is used to delay wound closure until a negative margin has been achieved; however, this can delay care and increase costs. OBJECTIVE: Chylous fistulas can occur after neck surgery. Both nonoperative measures and direct fistula ligation may lead to fistula resolution. However, a refractory fistula requires upstream thoracic duct ligation. This can be accomplished minimally invasively. Success depends on lymphatic flow interruption where the duct enters the thorax. We report a 37-year-old Chinese woman who presented with a groin lump associated with constant and non-catamenial pain. Ultrasonography was suggestive of an inguinal hernia. Intraoperatively, an endometriotic lump with no laparoscopical evidence of endometriosis in the abdominopelvic region was found. Isolated endometriotic lesions in the inguinal canal is a rare occurrence. Surgeons, however, should consider this diagnosis in women of reproductive age who present with a painful groin lump.

Epidermoid cyst of the testis, which is regarded as benign monodermal development of differentiated teratoma and comprises less than 1% of all testicular tumors, was found within the tunica albuginea in a 23-year-old man. OBJECTIVE: To evaluate intraoperative factors predicting appendiceal pathology during gynecologic oncology surgery for suspected mucinous ovarian neoplasms.
METHODS: We conducted a retrospective study on 225 patients with mucinous ovarian neoplasms who underwent surgery for an adnexal mass with concurrent appendectomy between 2000 and 2018. Regression analyses were used to evaluate intraoperative factors, such as frozen section of the ovarian mass and surgeon's impression of the appendix in predicting appendiceal pathology.

RATIONALE: Accurate resection of papillary thyroid carcinoma (PTC) is key to prognosis, but frozen section has sampling and time limitations.
PATIENT CONCERNS: A 50-year-old female was diagnosed with suspected PTC and underwent surgical treatment.
DIAGNOSES: Intraoperative EndoCell detected residual atypical cells, and postoperative pathology confirmed bilateral PTC.
INTERVENTIONS: The patient underwent a total thyroidectomy, with intraoperative EndoCell (real-time cellular imaging) used with nanocarbon/indocyanine green to assess margins and preserve parathyroids.
