Breath hydrogen response to lactulose in healthy subjects: relationship to methane producing status.

In order to assess the relationship between methane (CH4) producing status and the breath excretion of hydrogen (H2) in healthy subjects, breath CH4 and H2 were simultaneously measured for 14 hours after oral ingestion of 10 g lactulose in 65 young volunteers. Forty were breath CH4 producers and 25 were not. Statistically significant differences were observed between both groups, with lower values for CH4 producers recorded for the following parameters: fasting basal value of breath H2 (8.1 (4.9) v 5.2 (3.7) ppm, p less than 0.05), mouth-to-caecum transit time (68 (24) v 111 (52) min, p less t

The effect of size and density on mean retention time of particles in the gastrointestinal tract of sheep.

The selective retention of particles in the reticulo-rumen and in the gastrointestinal tract distal to the reticulo-rumen was studied in fistulated sheep maintained on a roughage diet. Polyethylene glycol and plastic particles of different lengths (1 and 10 mm) and densities (0.92, 1.03, 1.22 and 1.44 g/ml) were either fed or were introduced into the omasum. The mean retention time in the reticulo-rumen (MRTRR) of 1 mm long particles with a density of approximately 1.0 g/ml was about 67 h, that is eight times longer than the MRTRR of fluid; the heavier particles were retained only three times

Orocaecal transit time in health and in thyroid disease.

Altered bowel habit commonly occurs in thyroid disease. We measured orocaecal transit in healthy volunteers and in hyperthyroid and hypothyroid patients before and after treatment, using the lactulose hydrogen breath test incorporating a standard liquid meal to obtain a more physiological assessment. Mean transit time in 12 control subjects was 85 (8) minutes (mean (SE)) (mean coefficient of variation between replicate studies, 8.6% (3%)). In eighteen hyperthyroid patients transit was more rapid at 49 (4) minutes (p less than 0.01). Ten hypothyroid patients had a transit time similar to contro

Latest possible timing for endoscopic-assisted intervention in capsule endoscopy.

BACKGROUND: Delayed upper gastrointestinal transit during small bowel capsule endoscopy (SBCE) can lead to incomplete or failed examinations but can be treated by endoscopic-assisted intervention (EAI). The aim of this study was to investigate the latest possible timing of EAI. METHODS: Patients who underwent SBCE and received EAI between July 2007 and December 2020 were retrospectively reviewed. A novel T-value was developed that accounted for the varied battery life of different generations of PillCam when determining the latest possible timing of EAI, where T is calculated as EAI time/[min…

SmartPill™ Administration to Assess Gastrointestinal Function after Spinal Cord Injury in a Porcine Model-A Preliminary Study.

Gastrointestinal (GI) complications, including motility disorders, metabolic deficiencies, and changes in gut microbiota following spinal cord injury (SCI), are associated with poor outcomes. After SCI, the autonomic nervous system becomes unbalanced below the level of injury and can lead to severe GI dysfunction. The SmartPill™ is a non-invasive capsule that, when ingested, transmits pH, temperature, and pressure readings that can be used to assess effects in GI function post-injury. Our minipig model allows us to assess these post-injury changes to optimize interventions and ultimately impro

Oxytocin regulates gastrointestinal motility, inflammation, macromolecular permeability, and mucosal maintenance in mice.

Enteric neurons express oxytocin (OT); moreover, enteric neurons and enterocytes express developmentally regulated OT receptors (OTRs). Although OT (with secretin) opposes intestinal inflammation, physiological roles played by enteric OT/OTR signaling have not previously been determined. We tested hypotheses that OT/OTR signaling contributes to enteric nervous system (ENS)-related gastrointestinal (GI) physiology. GI functions and OT effects were compared in OTR-knockout (OTRKO) and wild-type (WT) mice. Stool mass and water content were greater in OTRKO mice than in WT. GI transit time in OTRK

[Translational research advances in peritoneal metastasis of gastrointestinal cancer: from molecular mechanisms to precision therapy].

Peritoneal metastasis is an advanced stage of gastrointestinal cancers. The Transforming growth factor-β (TGF-β), phosphatidylinositol 3-kinase/protein kinase B/mammalian target of rapamycin (PI3K/Akt/mTOR), and Wnt/β-catenin signaling pathways are associated with epithelial-mesenchymal transition, mesothelial-mesenchymal transition, and the formation of an immunosuppressive microenvironment. Moreover, these changes contribute to the development of peritoneal metastasis. Besides, the immune-stromal remodeling also plays an important role in the development of this process. Recent research is i

Understanding Inflammatory Bowel Disease in Women From Puberty to Menopause.

Women with inflammatory bowel disease (IBD) experience distinct physiologic and clinical challenges across their lifespan. In childhood and adolescence, early exposure to corticosteroids, chronic inflammation, and malnutrition may impair pubertal development, disrupt hormonal regulation, and reduce peak bone mass accrual. During puberty, fluctuating levels of estrogen and progesterone can exacerbate gastrointestinal symptoms and IBD activity. In adult women, gynecologic comorbidities such as endometriosis and uterine fibroids may mimic or worsen IBD-related symptoms, contributing to diagnostic
