Endoscopic therapy of the buried bumper syndrome: a clinical algorithm.

BACKGROUND: Buried bumper syndrome is a rare complication of percutaneous endoscopic gastrostomy with an overgrowth of the inner flange of the tube. Various therapeutic approaches in case studies in the literature are described. METHODS: We present the prospective data collection and analysis of 18 cases of buried bumper syndrome treated in our institution between 1 January 1998 and 31 December 2005. RESULTS: The median length of time after the first percutaneous endoscopic gastrostomy (PEG) tube insertion was 35 months (range = 1-168 months). In 37 endoscopic interventions all ingrown PEG's

Should upper gastrointestinal endoscopy be part of the evaluation for supraesophageal symptoms of GERD?

The past decade has witnessed increasing interest in the supraesophageal (ear, nose, and throat (ENT)) symptoms potentially caused by gastroesophageal reflux disease (GERD). Hoarseness, halitosis, problems with vocal presentations, excessive phlegm, frequent throat clearing, globus sensation, sore throat, cough, aspiration, laryngospasm, and laryngeal carcinoma are some, but not all of the ENT symptoms that have been attributed to GERD (1). The availability of highly effective medical and surgical therapies allows for control of these symptoms in many patients adding to the importance of accur

Indications for percutaneous endoscopic gastrostomy insertion: ethical aspects.

Percutaneous endoscopic gastrostomy (PEG) is a popular technique for long-term enteral nutrition. However it is not beneficial in all cases, and may even prolong the process of dying. The present article discusses the main indications for PEG insertion, and the ethical considerations involved. Three main questions need to be answered: (1) for what purposes should PEG be used; (2) for what type of patients, and (3) when should PEG be inserted in the natural history of the patient's illness? PEG is used in patients unable to maintain sufficient oral intake. It has been found to improve quality o

[Congestive gastropathy in liver cirrhosis].

Congestive gastropathy has emerged as a new nosological entity that can be included among the complications of advanced liver cirrhosis. It has been defined as the macroscopic changes of gastric mucosa occurring in portal hypertension that are associated with vascular mucosal and submucosal dilatation and ectasia without significant inflammatory changes. The pathogenesis of congestive gastropathy has not been completely cleared up. Many epidemiological and clinical studies and some tests on animals lead most Authors to think that the cause of this disease is a chronic increase of pressure in t

Symptomatic gastric amyloidosis in patients with primary systemic amyloidosis.

We reviewed the clinical records of 769 patients with primary systemic amyloidosis who had been examined at Mayo Clinic Jacksonville (Jacksonville, Florida) or Mayo Clinic Rochester (Rochester, Minnesota) during a 12-year period (1978 through 1989). Of these 769 patients, 59 (8%) had biopsy-established gastrointestinal amyloidosis, and 8 (1%) had symptomatic gastric amyloidosis. All eight patients with symptomatic gastric amyloidosis had hematemesis or prolonged nausea and vomiting in association with weight loss. Additional findings were gastroparesis (in three patients), gastric tumor (in on

Diagnosis of gastric polypoid lesions by magnifying endoscopy and dye endoscopy.

From July 1989 to June 1990, a total of 19 patients with gastric polyps were studied by magnifying endoscopy and dye endoscopy. After pretreatment with pronase, both close and magnified pictures were observed before and after sprays of 0.2% indigo carmine. Polypectomy or multiple biopsies were done after endoscopic examinations. Of the 19 patients; 13 were classified as hyperplastic polyps and 6 as neoplastic polyps by histologic examination. Twelve of the 13 hyperplastic polyps showed regular sulciolar or sulciolar-mesh mucosal patterns after magnification, only 1 showed an irregular sulciola

Endoscopic discovery and capture of Necator americanus in the stomach.

We report a case of the endoscopic discovery and removal of a specimen of Necator americanus hooked into the gastric mucosa of a black woman from Zaire. We extracted the parasite by means of biopsy forceps. Signs of previous bites were seen in the antrum. This exceptional localization might be due to a jejuno-duodeno-gastric reflux.
