Pompe disease: a country-wide molecular screening in a cohort of 15,068 study participants.

INTRODUCTION: Pompe disease (PD) is a rare inherited recessive autosomal disorder caused by pathogenic nucleotide variants within the gene GAA, encoding Acid alpha-glucosidase (GAA), the lysosomal enzyme catalyzing glycogen breakdown to glucose. METHODS: We performed molecular screening in a cohort of 15,068 participants with suspected PD of a wide range of age from several regions of Russia. Two screenings had been undertaken from 2014 to 2025, and 2021 to 2025, respectively: 13,128 patients were screened using "two-tier one-gene" algorithm (measurement of GAA activity in dried blood spots f

Distributed quantile regression for big data with missing covariates.

UNLABELLED: Data missing caused by non-response or drop-out appears routinely in modern medical studies. With the rapid development of information technology, medical data are becoming more and more massive in volume, and thus usually require distributed analysis in real-world applications, especially for cases where the data are collected from multiple centers or contain patient privacy. In this paper, we focus on developing efficient distributed algorithms to support quantile regression analysis in missing big data, which provides a powerful tool to model the skew and heterogeneous medical s

TorchGWAS2: Cost-Effective Phenome- and Genome-Wide Association Testing in Related Samples.

Modern large-scale genetic association analyses of imaging and omics data reveal unprecedented details of the genetic architecture of complex traits. Such analyses involve scanning thousands of phenotypes using linear mixed model-based genome-wide association study tools to control for sample relatedness. However, current LMM tools are not designed for such scale, creating a computational burden that hinders discovery. We propose TorchGWAS2, a cost-effective solution that overcomes the bottleneck using a deterministic variance-correction algorithm for LMMs, making it well-suited for GPU accele

Autosomal dominant polycystic kidney disease without apparent family history: A single-center experience.

Autosomal dominant polycystic kidney disease (ADPKD) - the most common hereditary kidney disease - accounts for nearly 5% of patients with kidney failure. Due to de novo mutations or other causes, nearly 25% of patients with a clinical diagnosis of PKD have no apparent family history. This poses a challenge for practicing nephrologists in making an accurate diagnosis. We evaluated ADPKD patients with and without a family history and compared genetic testing, kidney imaging, and kidney function results. We reviewed the genetic testing results of 54 PKD patients at Thomas Jefferson University Ho

Genetic architecture of bone marrow fat fraction implies its involvement in osteoporosis risk.

Bone marrow adipose tissue, as a distinct adipose subtype, has been implicated in the pathophysiology of skeletal, metabolic, and hematopoietic disorders. To identify its underlying genetic factors, we utilized a deep learning algorithm capable of quantifying bone marrow fat fraction (BMFF) in the vertebrae and proximal femur using magnetic resonance imaging data of over 38,000 UK Biobank participants. Genome-wide association analyses uncovered 373 significant BMFF-associated variants (P-value < 5 × 10-9), with enrichment in bone remodeling, metabolism, and hematopoiesis pathway. Furthermore,

An Extended Twin-Pedigree Study of Neuroticism in the Netherlands Twin Register.

For the participants in the Netherlands Twin Register (NTR) we constructed the extended pedigrees which specify all relations among nuclear and larger twin families in the register. A total of 253,015 subjects from 58,645 families were linked to each other, to the degree that we had information on the relations among participants. We describe the algorithm that was applied to construct the pedigrees. For > 30,000 adolescent and adult NTR participants data were available on harmonized neuroticism scores. We analyzed these data in the Mendel software package (Lange et al., Bioinformatics 29(12):

The chromosome 22q11.2 deletion: from the unification of biomedical fields to a new kind of genetic condition.

How can genetics reshape nosology? This paper examines the way knowledge about a genetic mutation - the microdeletion at chromosomal locus 22q11.2 - transformed our understanding of several rare clinical syndromes and designated a qualitatively new population of patients. Taking the 1400 papers about the 22q11.2 deletion and the clinical conditions with which it was associated, we generate a network of papers tied by citations for each of the last 35 years. Using a modularity algorithm, we identify communities and evaluate their salience for the networks' overall structure. This analysis, supp

[From the clinical to the genetic diagnosis of Prader-Willi and Angelman syndromes].

INTRODUCTION AND DEVELOPMENT: Angelman syndrome (AS) is characterised by severe mental retardation (MR), the absence of language, ataxia and/or tremors in the extremities and a characteristic behavioural phenotype with a happy behaviour and hyperactivity. Patients often show signs of microcephaly and convulsions. Prader-Willi syndrome (PWS) is characterised by acute hypotonia and feeding problems in the neonatal period, and triggers an uncontrollable appetite in the infant that leads to obesity. Most patients have some degree of MR, behavioural disorders and hypogonadism. Both pathologies are

Predictors of warfarin therapeutic dose in cancer patients: a preliminary comparative study using machine learning-integrated explainable artificial intelligence and Bayesian models.

BACKGROUND: Warfarin dosing in cancer patients is challenging due to altered pathophysiology and variable responses. While machine learning offers predictive potential, clinical adoption requires interpretability. This study is an initial attempt at addressing whether different dosing strategies are required in this vulnerable population. RESEARCH DESIGN AND METHODS: This observational study analyzed 1,746 patients (124 with cancer) from the International Warfarin Pharmacogenetics Consortium database. Five machine learning algorithms were developed and compared with the best-performing model

Large vessel vasculitis combined with inflammatory bowel disease: epidemiology, shared pathogenesis, and therapeutic advances.

Concomitant existence of large vessel vasculitis (LVV) and inflammatory bowel disease (IBD) is rare but has gained increasing clinical and research attention. LVV, comprising Takayasu arteritis (TAK) and giant cell arteritis (GCA), is characterized by granulomatous inflammation of the aorta and its branches. Accumulating evidence demonstrates a substantial overlap between LVV and IBD in epidemiological patterns, genetic susceptibility loci, immune cell subsets, and proinflammatory cytokine networks, supporting shared immunopathogenic mechanisms. IBD, including Crohn's disease (CD) and ulcerati

Genetic Influences on Disease-Modifying Therapy Response in Multiple Sclerosis: Current Insights and Future Directions.

Multiple sclerosis (MS) is a clinically and biologically heterogeneous, immune-mediated disease of the central nervous system, with substantial interindividual variability in disease course and response to disease-modifying therapies (DMTs). Over the past three decades, the MS therapeutic landscape has expanded considerably; however, treatment selection and switching remain guided primarily by clinical phenotype and imaging findings rather than molecular predictors of response. Despite extensive clinical trial evidence, prospectively identifying responders and non-responders to specific DMTs r

Bleeding risk assessment tools in patients with atrial fibrillation taking anticoagulants: a comparative review and clinical implications.

INTRODUCTION: Bleeding risk assessment plays a critical role in the anticoagulation management for atrial fibrillation (AF), to balance stroke prevention with risk of major hemorrhage. Traditional bleeding risk models, such as HAS-BLED, ORBIT, and ATRIA, offer valuable insights but have limitations in predictive accuracy and clinical applicability. Recent advances in risk stratification have introduced novel models integrating biomarkers, genetic data, and artificial intelligence (AI)-driven algorithms to improve precision and individualized patient care. Deciphering the pathogenicity of PADI4 missense variant L117M in rheumatoid arthritis: An integrated in silico and gene expression study.

Rheumatoid Arthritis (RA) is a complex autoimmune disorder characterized by chronic joint inflammation and systemic immune dysfunction. Its etiology involves genetic, environmental, and immunological factors. Among the genetic contributors, single nucleotide polymorphisms (SNPs) in the PADI4 gene encoding peptidylarginine deiminase type 4 have gained prominence. PADI4 mediates citrullination, a post-translational modification linked to RA. The missense mutation rs1748033 has been associated with increased RA susceptibility. To investigate the pathogenic role of rs1748033, an integrated computa

Diagnostic and prognostic perspectives of Fabry disease via fiber evanescent wave spectroscopy advanced by machine learning.

Fabry disease (FD) is a rare disorder resulting from a genetic mutation characterized by the accumulation of sphingolipids in various cells throughout the human body, leading to progressive and irreversible organ damage, particularly in males. Genetically-determined deficiency or reduced activity of the enzyme (alpha - Galactosidase; α-Gal) leads to the accumulation of sphingolipids in the lysosomes of various cell types, including the heart, kidneys, skin, eyes, central nervous system, and digestive system, triggering damage, leading to the failure of vital organs, and resulting in progressiv

MARGINAL: An Automatic Classification of Variants in BRCA1 and BRCA2 Genes Using a Machine Learning Model.

Implementation of next-generation sequencing (NGS) for the genetic analysis of hereditary diseases has resulted in a vast number of genetic variants identified daily, leading to inadequate variant interpretation and, consequently, a lack of useful clinical information for treatment decisions. Herein, we present MARGINAL 1.0.0, a machine learning (ML)-based software for the interpretation of rare BRCA1 and BRCA2 germline variants. MARGINAL software classifies variants into three categories, namely, (likely) pathogenic, of uncertain significance and (likely) benign, implementing the criteria est
