Germline polygenic score for prostate cancer aggressiveness.

BACKGROUND: Risk stratification for prostate cancer (PCa) progression or aggressiveness is often based on clinicopathologic features, some of which may be influenced by genetic factors. We developed a novel, germline polygenic risk score (PRSagg) to predict likelihood of developing aggressive PCa. METHODS: PRSagg was developed using data from 38,688 patients with PCa (case-only analysis) from the Million Veteran Program (MVP) through a genome-wide search for variants associated with PCa grade group at diagnosis. We tested associations of PRSagg with grade group using the entire MVP dataset us

Combined Effect of Genetic Predisposition and Socioeconomic Deprivation on Atrial Fibrillation Risk.

AIMS: The association between socioeconomic deprivation and atrial fibrillation (AF) remains debated, with limited evidence on its interaction with genetic predisposition. We aimed to evaluate the combined effect of socioeconomic deprivation and genetic risk on incident AF. METHODS: Participants without prior AF from the UK Biobank were included. Socioeconomic deprivation was defined as the top 5% of the Index of Multiple Deprivation (IMD). Genetic risk was assessed using a polygenic risk score (PRS) for AF. The primary outcome was incident AF. Findings were replicated in an independent South

Counselling under genetic heterogeneity: a practical approach.

Hereditary diseases due to mutation at different gene loci may be indistinguishable phenotypically. In these situations genetic risk predictions using polymorphic markers may be hampered if an individual family is not sufficiently informative to permit it to be assigned to one or the other linkage group. To provide the most usefull estimates of risk, the probability of linkage to a particular chromosome region should be determined prior to calculating risk estimates using the marker system. The probability can be calculated directly using the lod score generated for the family. The individual

An algorithm to identify patients aged 0-3 with rare genetic disorders.

BACKGROUND: With over 7000 Mendelian disorders, identifying children with a specific rare genetic disorder diagnosis through structured electronic medical record data is challenging given incompleteness of records, inaccurate medical diagnosis coding, as well as heterogeneity in clinical symptoms and procedures for specific disorders. We sought to develop a digital phenotyping algorithm (PheIndex) using electronic medical records to identify children aged 0-3 diagnosed with genetic disorders or who present with illness with an increased risk for genetic disorders. RESULTS: Through expert opin

Genetic evidence for a causal relationship between type 2 diabetes and peripheral artery disease in both Europeans and East Asians.

BACKGROUND: Observational studies have revealed that type 2 diabetes (T2D) is associated with an increased risk of peripheral artery disease (PAD). However, whether the two diseases share a genetic basis and whether the relationship is causal remain unclear. It is also unclear as to whether these relationships differ between ethnic groups. METHODS: By leveraging large-scale genome-wide association study (GWAS) summary statistics of T2D (European-based: Ncase = 21,926, Ncontrol = 342,747; East Asian-based: Ncase = 36,614, Ncontrol = 155,150) and PAD (European-based: Ncase = 5673, Ncontrol = 35

Application of the North American Familial Chylomicronemia Syndrome Score (NAFCS Score) in monogenic hypertriglyceridemia patients: A single-center Turkish cohort study.

BACKGROUND AND OBJECTIVES: Hypertriglyceridemia (HTG) may result from various etiologies, including the rare autosomal recessive disorder of chylomicron metabolism known as Familial Chylomicronemia Syndrome (FCS). Due to the risk of recurrent acute pancreatitis and subsequent pancreatic insufficiency, early and accurate diagnosis of FCS is clinically critical. Although genetic testing is considered the gold standard for diagnosis, access remains limited in many settings. To address this gap, the North American Familial Chylomicronemia Syndrome score (NAFCS Score) has been developed as a clinic

Polygenic Risk Score Informed Clinical Model for Improving Abdominal Aortic Aneurysm Screening.

BACKGROUND: Abdominal aortic aneurysm (AAA) is a complex disease with environmental and genetic risk factors. Polygenic risk scores (PRSs) based on disease-specific risk-associated single nucleotide variants (SNVs) have demonstrated effectiveness in stratifying individual-level disease risk for cardiovascular diseases. This prospective cohort study assessed associations of PRS of AAA (PRSAAA) with risk of incident AAA, analyzed the effectiveness of a combined clinical-genetic risk model, and explored the clinical utility of the model in identifying high-risk individuals for AAA screening. Genetic and phenotypic heterogeneity of type 2 diabetes across Russian ancestry groups.

INTRODUCTION: Type 2 diabetes mellitus (T2D) is a highly polygenic disease involving multiple biological pathways. Genetic ancestry may influence the predominant mechanisms driving T2D. Understanding how genetic background shapes T2D risk is crucial for developing personalized prevention and treatment strategies. METHODS: We analyzed ancestry-specific differences in T2D mechanisms and assessed the prevalence of T2D-associated genetic clusters, reflecting biological mechanisms underlying T2D onset and progression, in individuals from three Russian ancestry groups: Chechens, Tatars, and Yakuts.

Gene expression integration and similarity score-based modeling improve risk stratification in idiopathic venous thrombophilia.

BACKGROUND: Idiopathic venous thromboembolism (VTE) occurs in the absence of provoking factors, limiting the efficacy of current risk stratification. In parallel, the lack of integration between transcriptomic data and established risk factors prevents the identification of individuals with a high baseline predisposition. OBJECTIVES: We aimed to improve risk stratification of idiopathic VTE beyond traditional clinical models by developing a similarity-based risk score that integrates transcriptomic profiles with conventional risk factors. METHODS: We analyzed 790 individuals from the Genetic

Underestimation of risk of a BRCA1 or BRCA2 mutation in women with high-grade serous ovarian cancer by BRCAPRO: a multi-institution study.

PURPOSE: Identification of the 10% to 15% of patients with ovarian cancer who have germline BRCA1 or BRCA2 mutations is important for management of both patients and relatives. The BRCAPRO model, which estimates mutation likelihood based on personal and family cancer history, can inform genetic testing decisions. This study's purpose was to assess the accuracy of BRCAPRO in women with ovarian cancer. METHODS: BRCAPRO scores were calculated for 589 patients with ovarian cancer referred for genetic counseling at three institutions. Observed mutations were compared with those predicted by BRCAPR

Predicting embryonic aneuploidy rate in IVF patients using whole-exome sequencing.

Infertility is a major reproductive health issue that affects about 12% of women of reproductive age in the United States. Aneuploidy in eggs accounts for a significant proportion of early miscarriage and in vitro fertilization failure. Recent studies have shown that genetic variants in several genes affect chromosome segregation fidelity and predispose women to a higher incidence of egg aneuploidy. However, the exact genetic causes of aneuploid egg production remain unclear, making it difficult to diagnose infertility based on individual genetic variants in mother's genome. In this study, we

Multi-Trait Polygenic Risk Score, Nongenetic Determinants, and Cardiovascular Disease Death: A Cohort Study of 14 086 Japanese Individuals.

BACKGROUND: Although utility of composite trait-specific polygenic risk score (multi-trait PRS) has been examined among European ancestries, few studies investigated among East Asians and incorporated modifiable risk factors. We examined the associations of multi-trait PRS for cardiometabolic factors with cardiovascular disease mortality by integrating nongenetic determinants. Lysosomal polygenic risk is associated with the severity of neuropathology in Lewy body disease.

Intraneuronal accumulation of misfolded α-synuclein is the pathological hallmark of Parkinson's disease and dementia with Lewy bodies, often co-occurring with variable degrees of Alzheimer's disease related neuropathology. Genetic association studies have successfully identified common variants associated with disease risk and phenotypic traits in Lewy body disease, yet little is known about the genetic contribution to neuropathological heterogeneity. Using summary statistics from Parkinson's disease and Alzheimer's disease genome-wide association studies, we calculated polygenic risk scores a

[Contribution of the CoLaus study to decipher the determinants of serum uric acid].

Asymptomatic hyperuricemia affects one in five adults in the general population and is associated with elevated cardiovascular risk. It is however not clear whether asymptomatic hyperuricemia is a cause or simply a marker of conditions associated with high cardiovascular risk. Sex, age, obesity, renal function and selected drugs are major determinants of serum uric acid. Moreover, recent genome-wide association studies have identified new genes involved in the control of serum uric acid levels, in particular SLC2A9, which encodes a urate transporter located in the kidney. A genetic score based

Inference of Causal Relationships Between Genetic Risk Factors for Cardiometabolic Phenotypes and Female-Specific Health Conditions.

Background Cardiometabolic diseases are highly comorbid, but their relationship with female-specific or overwhelmingly female-predominant health conditions (breast cancer, endometriosis, pregnancy complications) is understudied. This study aimed to estimate the cross-trait genetic overlap and influence of genetic burden of cardiometabolic traits on health conditions unique to women. Methods and Results Using electronic health record data from 71 008 ancestrally diverse women, we examined relationships between 23 obstetrical/gynecological conditions and 4 cardiometabolic phenotypes (body mass i
