Health students' knowledge and infectious disease exposure: findings from a cross-sectional study in Namibia.

BACKGROUND: Namibia has recently introduced a number of health training programmes that expose students to infectious disease risks such as human immunodeficiency virus (HIV) and tuberculosis (TB). We explored the knowledge of students in relation to HIV and TB and whether or not there was evidence of exposure. METHODS: We conducted two cross-sectional surveys of Namibian health students (medicine and pharmacy) in 2018. RESULTS: There was a strong association between knowledge and exposure to HIV, but not TB (i.e. explicit exposure versus latent). Regression analysis suggested the time-relat

Apobec3G-Based Strategies to Defeat HIV Infection.

BACKGROUND: To suppress HIV infection, host cells have evolved numerous defenses that generally belong to the innate and adaptive immune responses. Over the last decade, extensive efforts have been focused on understanding HIV restriction factors and mechanisms of evasion. The host protein APOBEC3G (A3G) was identified as a member of cytidine deaminase family in 2002, and it was shown that, in the absence of HIV encoded Vif, A3G can block the replication of HIV-1 by introducing viral hypermutations during reverse transcription, also conferring innate immunity to the virus. Hence, therapeutic e

Adipose tissue and immune function: a review of evidence relevant to HIV infection.

Human immunodeficiency virus type 1 (HIV) infection and antiretroviral therapy (ART) have long been associated with abnormalities in adipose tissue distribution and metabolism. More-recent evidence demonstrates that adipocytes and adipose-resident immune cells have a role in the response to HIV. Clinical and laboratory studies indicate that viral proteins and antiretroviral medications alter adipocyte biology to enhance the persistent, systemic inflammatory state characteristic of untreated and treated HIV infection. Relationships between body composition and lymphocyte populations, cellular i

HIV compartmentalization: a review on a clinically important phenomenon.

Viral diversity plays an important role in the pathogenesis of HIV disease. However, within an individual, HIV can vary substantially from one tissue or cell type to another, thereby creating viral compartments. HIV compartmentalization has been well documented in the brain, cerebrospinal fluid, and genital tract, although there are also data for viral compartmentalization within the gut, lung, liver, kidney, and breast milk. The precise mechanisms that lead to the development of HIV compartmentalization have not been adequately examined but likely include differential immune selection pressur

Engagement of adolescent girls and young women into an early oral PrEP open-label study in Southern Africa: lessons learned from HPTN 082.

BACKGROUND: Adolescent girls and young women (AGYW) in southern Africa face a high risk of HIV acquisition. Consistent use of daily oral pre-exposure prophylaxis (PrEP) can significantly mitigate this risk. We used the Good Participatory Practice Framework (GPP) to summarize lessons learned from engaging AGYW in an open-label PrEP trial, highlighting successful strategies that enhanced trial engagement. METHODS: The HIV Prevention Trials Network (HPTN) 082 was an open-label study conducted from 2016 to 2018 that evaluated PrEP uptake and adherence in AGYW 16- to 25-year-old without HIV living

Cardiovascular system characteristics in HIV/AIDS patients with talaromycosis.

Patients with acquired immune deficiency syndrome (AIDS) are more vulnerable to opportunistic infections (OIs) such as Talaromycosis marneffei (TSM), which is associated with a high mortality rate. Nevertheless, the effect of TSM on the cardiovascular system of affected patients remains elusive. To that end, this research aimed to investigate the impact of TSM on the cardiovascular system in the individuals with HIV/AIDS. Participants were assigned to the HIV/AIDS patients group or the HIV/AIDS patients with TSM (HIV/AIDS+TSM) group. A total of 120 individuals were included in the present stud

An educational intervention improves HIV competence of secondary school staff in Uganda: a stepped wedge cluster randomized trial.

HIV competence, encompassing knowledge, skills, attitudes, and practices to prevent, manage, and reduce HIV impact, is critical for meeting young people's HIV prevention and care needs. This study evaluated the effect of the Community of Practice on HIV (CoP-HIV) educational intervention on staff HIV competence and prevention knowledge in Ugandan schools, where HIV prevalence is rising among youth. Using a stepped-wedge cluster randomized trial, the intervention was implemented across six schools in western Uganda (March-September 2023). It included two eight-hour workshops and a handbook on H

Exploring the social meaning of curing HIV: a qualitative study of people who inject drugs in Guangzhou, China.

Our objective was to explore the social meaning of HIV and perceptions of an HIV cure among people who inject drugs (PWID) in Guangzhou, China, which speaks to ethical and resource challenges to development in this field. We conducted a qualitative research study using in-depth interviews. We analyzed interview transcripts from 29 PWID, eight physicians, and three social workers from an outpatient HIV clinic and two methadone maintenance treatment centers. The social meaning of HIV infection and perceptions of an HIV cure reflected patients' relationships with society, health systems, and phys

Double, double toil and trouble: transforming growth factor beta (TGF-β) in HIV infection.

Despite effective suppression of viral replication by antiretroviral therapy (ART), chronic HIV infection remains characterized by persistent low-level inflammation and progressive tissue damage, contributing to premature aging and an array of comorbidities including cardiovascular disease, HIV-associated neurocognitive disorders, liver disease, and fibrosis of multiple organs. Increased levels of transforming growth factor beta (TGF-β), characteristic of chronic HIV infection even in the context of ART, appear to be a common thread explaining these disparate comorbidities. As a pleiotropic cy

Are You Safe or Should I Go? How Perceived Trustworthiness and Probability of a Sexual Transmittable Infection Impact Activation of the Salience Network.

Functional imaging studies indicate that both the assessment of a person as untrustworthy and the assumption that a person has a sexually transmitted infection are associated with activation in regions of the salience network. However, studies are missing that combine these aspects and investigate the perceived trustworthiness of individuals previously assessed with high or low probability of a sexually transmitted infection. During fMRI measurements, 25 participants viewed photographs of people preclassified as having high or low HIV probability and judged their trustworthiness. In a postrati

Estrogen depletion and immune activation and inflammation in women with HIV.

PURPOSE OF REVIEW: As more women with HIV survive into older age, the menopausal transition has emerged as a critical yet underexplored determinant in HIV pathogenesis. Declining exposure to estrogens during menopause alters innate and adaptive immunity, driving inflammation, comorbidities, and viral persistence. RECENT FINDINGS: Estrogen influences both innate and adaptive immune responses. Estradiol enhances plasmacytoid dendritic cell type I interferon (IFN) production through Toll-like Receptor 7 (TLR7), promotes natural killer (NK) cell activity, and tempers monocyte/macrophage activatio

Cultural influences on HIV testing among Latino youth.

Young Latinos aged 13-24 years in the USA are disproportionately impacted upon by HIV. Despite the elevated risk, lower rates of HIV testing have been documented among Latino youth relative to other racial/ethnic groups. The objective of the current study was to examine the influence of acculturation and cultural values on HIV testing among Latino youth. The study consisted of 51 sexually experienced young Latinos aged 13-16 years from a major city in the Southeastern USA. Participants completed a survey on HIV testing history, cultural orientation and Latino cultural values. Results indicate

Blocking of integrins inhibits HIV-1 infection of human cervical mucosa immune cells with free and complement-opsonized virions.

The initial interaction between HIV-1 and the host occurs at the mucosa during sexual intercourse. In cervical mucosa, HIV-1 exists both as free and opsonized virions and this might influence initial infection. We used cervical explants to study HIV-1 transmission, the effects of opsonization on infectivity, and how infection can be prevented. Complement opsonization enhanced HIV-1 infection of dendritic cells (DCs) compared with that by free HIV-1, but this increased infection was not observed with CD4(+) T cells. Blockage of the α4-, β7-, and β1-integrins significantly inhibited HIV-1 infect

Early HIV-1 target cells in human vaginal and ectocervical mucosa.

After translocation through the pleuristratified epithelium of the lower female genital tract, HIV-1 encounters potential target mononuclear cells in the lamina propria of the vagina and ectocervix. Here we show that each major type of genital mononuclear cells, including dendritic cells (DCs), macrophages and lymphocytes, are susceptible to HIV-1 in vitro. Among suspensions of vaginal and ectocervical mononuclear cells, DCs were the first cells to take up virus, containing GFP-tagged virions as early as 15 min after exposure. At 2 hr after exposure, DCs still contained the largest proportion

Role of fibroblast growth factor-binding protein in the pathogenesis of HIV-associated hemolytic uremic syndrome.

A characteristic finding of childhood HIV-associated hemolytic uremic syndrome (HIV-HUS) is the presence of endothelial injury and microcystic tubular dilation, leading to a rapid progression of the renal disease. We have previously shown that a secreted fibroblast growth factor-binding protein (FGF-BP) is upregulated in kidneys from children affected with HIV-HUS and HIV nephropathy. Here, we sought to determine the potential role of FGF-BP in the pathogenesis of HIV-HUS. By immunohistochemical and in situ hybridization studies, we observed FGF-BP protein and mRNA upregulation in regenerating

Active HIV-1 redistribution and replication in the brain with HIV encephalitis.

The central nervous system (CNS) is of particular importance in human immunodeficiency virus type 1 (HIV-1) infection. First, the CNS may be difficult to access for anti-retroviral treatment and may become a sanctuary for residual viruses. Second, HIV-1 infection may lead to AIDS dementia complex (ADC) culminating in HIV-1 encephalitis. In order to examine the pattern of drug resistance and the role of encephalitis in enhancing viral redistribution to the CNS, we compared pol gene quasispecies of the spleen and brain in two patients with and two patients without HIV-1 encephalitis, who had bee

Changes in cognitive performance following combined CogSMART-SA and BrainHQ interventions: a pilot study.

BACKGROUND: Despite effective antiretroviral treatment, neurocognitive impairment (NCI) and functional disability remain concerningly prevalent among people living with HIV (PLWH). Despite this, there are currently no adequate evidence-based treatments targeting NCI in PLWH who reside in low- and middle -income countries. This is a major public health concern given that most the global population of PLWH is concentrated in those countries. Studies emerging from high-income countries of the global north suggest that different forms of cognitive remediation therapy (e.g., compensatory cognitive

Systematic discovery of pro- and anti-HIV host factors in primary human CD4+ T cells.

Host factors that promote or restrict human immunodeficiency virus (HIV) infection in human CD4+ T cells have not been comprehensively identified. We employed orthogonal genome-wide CRISPR activation (CRISPRa) and CRISPR knockout screens in primary CD4+ T cells to discover pro- and anti-HIV host factors systematically. Secondary pooled screens and individual perturbations validated high-confidence hits and revealed diverse mechanisms of action. CRISPRa uncovered multiple potent antiviral factors, including PI16, PPID, SHISA3, and ITM2A. PI16 interacts with host factors involved in HIV fusion a

Novel Approaches for Investigating Host Responses to HIV Infection in Ex Vivo Human Genital Tissues.

Understanding local immune responses and the structural integrity of the genital mucosal barrier is essential for advancing knowledge of both susceptibility to sexual HIV transmission and the pathogenesis of chronic HIV infection. This review highlights innovative methodologies for investigating host responses to HIV infection using ex vivo human genital tissues. Recent advances in spatial transcriptomics and multiplex imaging enable high-resolution analysis of tissue architecture, immune cell distribution, and gene expression within mucosal environments. These technologies reveal spatially he
