[Value of bone fluoride determination in osteoporotic patients treated with sodium fluoride].

In order to find out whether the response of osteoporosis to sodium fluoride treatment or complications arising from this treatment are related to the retention of fluoride in bones, we measured bone fluoride concentrations in 30 patients with primary osteoporosis who had been treated with sodium fluoride for 1 to 8 years. We found a significant correlation (r = 0.74, P less than 0.001) between bone fluoride concentration and total amount of fluoride ingested. However, with any given amount of sodium fluoride, the bone fluoride concentration could vary by up to three times from a patient to an

Atypical joint involvement in neuropathic arthropathy secondary to syringomyelia: A case report.

Neuropathic arthropathy (NA) is a chronic, progressive, destructive, aseptic inflammatory joint disorder resulting from sensory neurological deficit. We report a new case of NA with simultaneous involvement of the shoulder, elbow, and hands secondary to syringomyelia. Due to the rarity of this condition and its unusual location. The patient presented with painless deformation of both hands. Clinical examination revealed amyotrophy in both hands associated with hypoesthesia and bilateral areflexia of the upper limbs. The plain radiographs showed bone destruction and reconstruction in the metaca

Computer-guided Versus Conventional Modified Cortical Shell Technique for the Rehabilitation of Posterior Mandibular Deficiency: Randomized Controlled Trial.

PURPOSE: The cortical shell technique lacks anatomical guidance for harvesting and grafting. This study compares the horizontal bone gain and neurosensory function of a modified free- hand and a computer-guided cortical shell graft, which was also assessed for accuracy. MATERIALS AND METHODS: This study included eighteen patients with atrophic mandibles. Nine patients received free-hand cortical shell harvest, lateral convey, and fixation (the control group), and nine patients received guided workflow (the test group) that utilized cutting and positional guides. The interpositional gap was pa

Tuberculosis of the hand.

There has been recent interest in tuberculosis of the hand because of a rising incidence owing to increasing numbers of immigration, an aging population, and immunosuppressed people including affected patients with human immunodeficiency virus. In this article, we review the epidemiology, bacteriology, pathophysiology, diagnosis, and principles of treatment of tuberculosis of the hand. The second part of the report emphasizes the classification of hand tuberculosis (cutaneous lesions, tenosynovitis, bursitis, osteomyelitis, arthritis, and tuberculous hypersensitivity reactions) along with the

The "Standing Peanut" Scaphoid View: A Semi-supinated Radiographic View for Intraoperative Evaluation of Screw Placement in Scaphoid Waist Fractures.

Scaphoid waist fractures are the most common fracture of the scaphoid. Operative management is indicated with unstable fractures and often for nondisplaced waist fractures to decrease time to union and return to work/sport. Screw placement within the central axis of the scaphoid is paramount and correlates with outcomes. Assessment of intrascaphoid screw placement is classically done via intraoperative fluoroscopy. An additional fluoroscopic view is presented to assist in confirming implant positioning. Along with the standard anterioposterior, lateral, pronated oblique, and "scaphoid" view we

Safe approach for fixation of first metacarpal fractures: an anatomical study.

This anatomical study defines a safe zone for percutaneous or minimally invasive fixation of first metacarpal fractures in order to avoid injury of the superficial branch of the radial nerve and the dorsal branch of the radial artery. The courses of the nerve and artery branches were marked in 20 embalmed cadaver specimens. With computer-assisted surgical anatomy mapping, a large diversity in the anatomical patterns for the nerve and a consistent pattern for the artery were found. Based on these findings, we conclude that transfixation of the first and the second metacarpals with K-wires place

Multiple subcutaneous xanthogranuloma at juxta-articular sites with bone cystic changes resembling rheumatoid arthritis: A case report.

Xanthogranuloma is a benign disease represented as histiocytosis with lipoid deposition which usually occurs in children, but rarely in adults. We report a case of an adult patient with multiple subcutaneous xanthogranuloma at juxta-articular sites with bone cystic changes, manifesting similar clinical profiles to rheumatoid arthritis. Although very rare, we should consider the possibility of xanthogranulomatosis in the diagnosis of rheumatoid arthritis, especially in atypical cases.

Structural and Functional Changes of Hands and Legs in Early Rheumatoid Arthritis.

Background and Objectives: The aim of this study was to assess if there are structural and functional changes of hands and legs already in early rheumatoid arthritis (ERA), compared with the population-based control group. Additionally, we aimed to identify if the changes are symmetrical in hands and legs and if there are factors that are associated with these changes. The study was conducted, and, thus far, the results have been controversial. Materials and Methods: The study group consisted of 83 consecutive patients with ERA and 321 control subjects. Resolution of Proximal Humerus Enchondroma in a 53-Year-Old Female with Curettage and Bone Substitution: A Case Report.

INTRODUCTION: Enchondromas are benign cartilaginous tumors commonly affecting the small bones of the hand; however, proximal humeral involvement presenting with persistent pain in older adults is uncommon and may mimic low-grade chondrosarcoma. CASE REPORT: A 53-year-old female presented with right shoulder pain and proximal humeral swelling. Radiographs, magnetic resonance imaging, and computed tomography imaging demonstrated a lobulated intramedullary cartilaginous lesion with cortical thinning and endosteal scalloping. The patient underwent curettage of the lesion, biopsy, and bone graftin

Scaphoid and Lunate Dislocation with Complete Soft-Tissue Avulsion: A Case Report.

CASE: We describe an intercarpal dislocation with proximal and rotatory displacement of the lunate and with volar and radial dislocation of the scaphoid; both bones were stripped of soft tissue. Treatment involved ORIF (open reduction and internal fixation). Three years postoperatively, the patient had a DASH (Disabilities of the Arm, Shoulder and Hand) score of 10, equivalent grip strength and 50% wrist palmar flexion and 98% wrist dorsiflexion compared with the uninjured side, and radiographic evidence of posttraumatic arthritis. CONCLUSION: Treatment of simultaneous divergent lunate and sc

Lemierre's disease: a case with bilateral iliopsoas abscesses and a literature review.

Lemierre's disease is characterized by sepsis, often with an oropharyngeal source, secondary septic emboli and internal jugular vein thrombosis (Lancet 1:701-3, 1936. Clin Microbiol Rev 20(4):622-59, 2007). Septic emboli affecting many bodily sites have been reported, including the lungs, joints, bones, and brain. The case report describes an unusual case of Lemierre's disease in a 64 year old gentleman causing profound sepsis, acute kidney injury, bilateral iliopsoas abscesses and a right hand abscess. To our knowledge, this is the first reported case of Lemierre's disease in the context of b

[Carpal injuries in children].

Carpal injuries in children are uncommon. Radiographic diagnosis can be difficult because of late ossification of carpal bones. Scaphoid fracture occurs in 3% of hand and wrist fractures. Fractures in the distal third or at the waist are the most frequent fracture sites. In case of clinical and X-rays presumption, CT-scan and MRI often diagnose scaphoid fractures. Orthopaedic treatment with closed cast for 8 weeks, or more for adolescents, gives a high rate of union in non-displaced or minimally-displaced fractures. Non-union is the most frequent complication. Closed cast can be the treatment

Prenatal ultrasound findings in a fetus with paternal uniparental disomy 14q12-qter.

We present the prenatal ultrasound findings in a case of postnatally identified paternal uniparental isodisomy 14q12-qter. Increased nuchal translucency and a large omphalocele were identified at 14 weeks' gestation. Karyotyping revealed a normal male, 46,XY. As gestation advanced, polyhydramnios developed, skeletal abnormalities involving the long bones and chest became evident, hand contractures developed, and the presumed large omphalocele was in part found to be a large ventral hernia, as echogenic adipose tissue could be seen in the abdominal wall near to the cord insertion. Prenatal find

Functional fracture bracing in metacarpal fractures: the Galveston metacarpal brace versus a plaster-of-Paris bandage in a prospective study.

A total of 133 patients with fractures of the second through the fifth metacarpal bones were randomized to receive either a functional brace (the Galveston metacarpal brace) or a dorsal/ulnar plaster cast. Only 42% of the patients in the metacarpal-brace group completed the treatment, in contrast to 81% of the patients in the plaster-cast group. Complications with the brace were due to 60% of the exclusion. No difference according to gender, age, fracture type, hand affected (right/left), or mechanism of injury was observed between the patients who completed the treatment and those who were ex

Screw fixation for certain fractures of the phalanges and metacarpals.

Twenty-one patients with fractures of the bones of the hand were treated by open reduction and internal fixation with compression screws. The firm fixation allowed range-of-motion exercises in most patients to be begun on the third postoperative day. On follow-up mild restriction in digital motion was seen in only one patient, and significant disturbance in joint function was not encountered. In spite of these advantages, the method is indicated in only a small percentage of such fractures. Usually traditional methods of open reduction and internal fixation are preferred when an open procedure
