Factors that impede women's contraceptive care needs during and immediately after incarceration in New South Wales, Australian. Qualitative perspectives of professional staff working with incarcerated women.

INTRODUCTION: Women with a history of incarceration report higher rates of pregnancy and abortion, lower contraception use and poorer birth outcomes compared to other women in the community. This suggests family-planning and reproductive-health needs that are not well served by current models of care. Despite this, little is known about how contraception is provided within Australian carceral settings, or the structural barriers experienced by professional staff working in these environments. We report findings from a qualitative study conducted in New South Wales (NSW), Australia, describing

Structural and interagency influences on mandated health assessments for children in out-of-home care in Sweden: a qualitative study of child protection services workers.

BACKGROUND: Children in out-of-home care (OOHC) have significantly poorer physical, mental and dental health than their peers. In Sweden, child protection services (CPS) are required to initiate health assessments (HAs) when a child is placed in OOHC. Despite this statutory requirement, fewer than half of eligible children undergo an HA. Factors influencing implementation remain poorly understood. METHODS: This qualitative study explored CPS caseworkers'change experiences of initiating and coordinating HAs for children placed in OOHC in Sweden. Five semi-structured focus group discussions wer

Investment in family medicine to improve health outcomes in sub-Saharan Africa.

Family physicians (FPs), as educators, capacity builders and clinical governance leaders of primary care (PC) teams, work to make them more effective and responsive to the needs of their patients. Various strategies are required to raise the profile of Family Medicine (FM) to ensure stronger representation in health sector planning and policy development for advocacy on behalf of the communities they serve. An illustration is given of the need for FP leaders to become equal partners in the National Surgical, Obstetric and Anaesthesia Planning process to ensure safer surgery at district hospita

Multisectoral cost analysis of a human and livestock anthrax outbreak in Songwe Region, Tanzania (December 2018-January 2019), using a novel Outbreak Costing Tool.

OBJECTIVES: We applied a novel Outbreak Costing Tool (OCT), developed by the US Centers for Disease Control and Prevention (CDC), to estimate the costs of investigating and responding to an anthrax outbreak in Tanzania. We also evaluated the OCT's overall utility in its application to a multisectoral outbreak response. METHODS: We collected data on direct costs associated with a human and animal anthrax outbreak in Songwe Region (December 2018 to January 2019) using structured questionnaires from key-informants. We performed a cost analysis by entering direct costs data into the OCT, grouped

Patients Are Knowledge Workers in the Clinical Information Space.

BackgroundLimited research exists on patient knowledge/cognition or "getting inside patients' heads." Because patients possess unique and privileged knowledge, clinicians need this information to make patient-centered and coordinated treatment planning decisions. To achieve patient-centered care, we characterize patient knowledge and contributions to the clinical information space. Methods and objectivesIn a theoretical overview, we explore the relevance of patient knowledge to care provision, apply historical perspectives of knowledge acquisition to patient knowledge, propose a representation

Modelling cholera transmission dynamics in the presence of limited resources.

OBJECTIVES: We study the transmission dynamics of cholera in the presence of limited resources, a common feature of the developing world. The model is used to gain insight into the impact of available resources of the health care system on the spread and control of the disease. A deterministic model that includes a nonlinear recovery rate is formulated and rigorously analyzed. Limited treatment is described by inclusion of a special treatment function. Center manifold theory is used to show that the model exhibits the phenomenon of backward bifurcation. Matlab has been used to carry out numeri

Design and development of a smartphone app for hypertension management: An intervention mapping approach.

BACKGROUND: Several research studies have demonstrated the potential of mobile health apps in supporting health management. However, the design and development process of these apps are rarely presented. OBJECTIVE: We present the design and development of a smartphone-based lifestyle app integrating a wearable device for hypertension management. METHODS: We used an intervention mapping approach for the development of theory- and evidence-based intervention in hypertension management. This consisted of six fundamental steps: needs assessment, matrices, theoretical methods and practical strate

Preparedness and response to bioterrorism.

As we enter the 21st century the threats of biological warfare and bioterrorism (so called asymmetric threats) appear to be more real than ever before. Historical evidence suggests that biological weapons have been used, with varying degrees of success, for many centuries. Despite the international agreements to ban such weapons, namely the 1925 Geneva Protocol and the 1975 Biological and Toxin Weapons Convention, there is no effective international mechanism for challenging either the development of biological weapons or their use. Advances in technology and the rise of fundamentalist terror

The evacuation of two rural hospitals following a tsunami warning: What happened next.

This paper describes the lessons gained and actions taken following the evacuation of two small rural hospitals following a tsunami warning. These evacuations were unique in that the normal protocol to transfer within the health system was not feasible due to their geographical isolation. The after action review process provided valuable insight to all stakeholders, who entered into discussions to ensure procedures, supplies, equipment and arrangements would be put in place to support the safe and effective evacuation of patients in the event of a future emergency. A key component of this proc

The central role of housing key workers in supporting healthcare interactions for people experiencing homelessness and implications for palliative care: a qualitative study.

BACKGROUND: People experiencing homelessness access specialist palliative care late in their illness trajectory, if at all. There is also little evidence they receive generalist palliative care or are given opportunities to engage in Advance Care Planning. This qualitative study describes the central role of key workers in supporting access to healthcare in homeless communities and identifies implications for improving palliative care provision. METHODS: Qualitative data were collected via focus groups and individual interviews with staff working for a key provider of support and housing/acco

'I Would Feel Failed By the Medical Community Entirely': Transgender Americans' Calls for Advocacy From Health Professionals in a Care-Restricted World.

BACKGROUND: Gender-affirming care (GAC) improves the mental health, quality of life, and social functioning of gender-diverse patients. In spite of this, GAC in the United States (as elsewhere) has become a major focus of debate, with several federal policy changes enacted through Executive Orders and legislation creating major restrictions on transgender (trans) Americans' ability to obtain the care they need. OBJECTIVE: We aimed to understand how trans Americans are responding to the anticipated changes in their access to GAC and to provide healthcare professionals with strategies to suppor

Expectant and new fathers say they need resources and sources of support.

OBJECTIVES: It is critical to exhaust the range of opportunities to reduce racial inequities in maternal mortality. Developing interventions to optimize how fathers can support mothers during pregnancy and childbirth remains under developed, particularly in the context of racial inequities. In this study, we collected formative data from expectant and new fathers to identify the types of supports and resources they need to promote positive maternal health outcomes. DESIGN: We used a phenomenological thematic approach to analyze data from 80 new fathers in 10 focus groups from five of the six
