Fracture of the Second Rib: An Indirect Sign of Serious Trauma Like Fracture of the First Rib?

BACKGROUND: The first 3 ribs are anatomically well-protected, shielded by the large thoracic muscles, the shoulder girdle, and the arm. A significant force is required to fracture these ribs; thus, such fractures suggest a high-energy trauma and are associated with injuries to vital organs of the thorax, such as the aorta, the heart, the lungs and the great vessels. METHODS: A retrospective analysis was conducted over a 10-year period at a single hospital. The study assessed patients with fractures of the second rib, including their concurrent injuries and the overall severity of their trauma

Traumatic rupture of a papillary muscle of the mitral valve due to blunt thoracic trauma.

Traumatic injuries of the mitral valve are rare but life threatening complications of high speed motor vehicle accidents. The physician must recognize traumatic mitral valve injuries early and arrange for immediate surgical repair, because if the papillary muscle ruptures completely, florid congestive failure and pulmonary edema develop rapidly. The signs and symptoms of traumatic mitral valve injuries vary. In general, the patient complains of chest pain and is hypotensive, dyspneic and cyanosed. He may have a bruised anterior chest wall, fractured ribs, unstable sternum and a flail chest, bu

Estrogen accelerates heart regeneration by promoting the inflammatory response in zebrafish.

Sexual differences have been observed in the onset and prognosis of human cardiovascular diseases, but the underlying mechanisms are not cle … on of the female-specific protein vitellogenin in male zebrafish. Transcriptomic analyses suggested heart injuries triggered pronounced immune and inflammatory responses in females. These responses, previously shown to elicit heart regeneration, could be enhanced by estrogen treatment in males and reduced by tamoxifen in females. Furthermore, a prior exposure to …

Predictors of poor outcomes after significant chest trauma in multiply injured patients: a retrospective analysis from the German Trauma Registry (Trauma Register DGU®).

BACKGROUND: Blunt thoracic trauma is one of the critical injury mechanisms in multiply injured trauma victims. Although these patients present a plethora of potential structural damages to vital organs, it remains debated which injuries actually influence outcome and thereby should be addressed initially. Hence, the aim of this study was to identify the influence of critical structural damages on mortality. METHODS: All patients in the database of the TraumaRegister DGU® (TR-DGU) from 2002-2011 with AIS Chest ≥ 2, blunt trauma, age of 16 or older and an ISS ≥ 16 were analyzed. Outcome paramet

Minute Myocardial Injury as Measured by High-Sensitive Troponin T Serum Levels Predicts the Response to Intracoronary Infusion of Bone Marrow-Derived Mononuclear Cells in Patients With Stable Chronic Post-Infarction Heart Failure: Insights From the TOPCARE-CHD Registry.

RATIONALE: Cell-based therapies are a promising option in patients with chronic postinfarction heart failure (ischemic cardiomyopathy [ICM]). However, the responses after intracoronary infusion of autologous bone marrow-derived mononuclear cells (BMCs) are heterogeneous, which may be related to impaired cell retention in patients with ICM. Ischemic injury is associated with upregulation of prototypical chemoattractant cytokines mediating retention and homing of circulating cells. The development of ultrasensitive tests to measure high-sensitive troponin T (hs-TnT) serum levels revealed the pre

[Pathology of vein, artery and heart catheterization. Studies of autopsies].

Among 17,161 autopsies of children and adults 319 cases with complications after catheterization (catheterization of veins, arteries and heart) were analysed. The following pathologo-anatomical findings were established: Haemorrhages, thromboses, thrombophlebitis, incorrect positions of the catheter, embolism after catheterization, perforations of vessels, heart injuries, pleura injuries, pneumo- and haemothorax as well as sepsis and septicopyaemia. The complications were subdivided according to early and late injuries. In 10% the complications after catheterization were directly and indirectl…

Nonskeletal injuries related to cardiopulmonary resuscitation: An autopsy study.

The current standard technique for cardiopulmonary resuscitation (CPR), initially described in the early 1960s, has quickly become the expected response for all persons found without a pulse or respiration. Despite the potentially lifesaving properties of external cardiac massage, the mainstay of resuscitation, it consists of repeated blunt force trauma to the chest, which can lead to extensive traumatic skeletal and nonskeletal injuries. Numerous autopsy-based studies have documented the incidence and patterns of rib and sternal fractures associated with attempted CPR, but there is relatively

Deciding the Cause of Death in a Victim of a Road Traffic Accident Became Difficult Due to Interventions: A Case Report.

Though uncommon, puncture injury to the heart can occur during cardiac resuscitation and when inserting a lifesaving drug directly into the left ventricle of the heart. Utmost precaution must be taken to avoid damaging the conduction system of the heart, particularly the nodal part, as it can cause cardiogenic shock, arrhythmia, and sudden death. Our index case report describes a 55-year-old male who was fatally injured after being hit by a truck while riding his bike. The autopsy revealed multiple puncture injuries to the atrioventricular node area of the septum, observed on both sides of the

Seat belt injuries: radiologic findings and clinical correlation.

The seat belt syndrome consists of skeletal, soft-tissue, and visceral injuries associated with use of two- and three-point restraints in patients involved in motor vehicle accidents. Skin abrasions of the neck, chest, and abdomen--the classic seat belt sign--indicate internal injury in 30% of cases. Neck abrasions are associated with injuries to the carotid artery, larynx, and cervical spine; chest abrasions, with fractures of the sternum, ribs, and clavicles and injuries to the heart and thoracic aorta; and abdominal abrasions, with mesenteric tears, bowel perforation and hematoma, Chance fr

Bullet embolisation from the right subclavian vein to the right ventricle: a case report.

Bullet embolism to the heart is an unusual complication of penetrating gunshot injuries. A bullet may reach the heart by direct cardiac penetration or entry into the peripheral venous system with embolisation to the heart, which must be differentiated. This is a report of an unusual case of bullet embolism to the heart that was extracted by direct cardiotomy without cardiopulmonary bypass.

Acquired post-traumatic aortic coarctation presenting as new-onset congestive heart failure: treatment with endovascular repair.

Acquired coarctation of the thoracic aorta is a rare phenomenon in adults. The etiology is often idiopathic, but severe stenosis can develop from prior surgery, blunt thoracic aortic injuries, or severe atherosclerotic/atheroembolic disease. Common symptomatic presentations include refractory upper extremity hypertension and new-onset congestive heart failure. We present the case of a 52-year-old man who developed acquired thoracic aortic coarctation 30 years after a blunt trauma and deceleration injuries to the aorta requiring open surgical aortic repair. He presented with poorly controlled h

[A comparative analysis of the incidence of the occurrence of injuries to the soft tissues, bones and internal chest organs in trauma from a fall from a height and from the collision of a moving car with a pedestrian].

The author compares the incidence of injuries to soft tissues, bones, and thoracic organs in two types of traumas: falling from height and collision of a pedestrian with a moving car. A total of 100 corpses of subjects who died from such traumas were examined. The incidence of the detected injuries is shown on figures. Comparison of the graphic images indicates that trauma from falling from height results in more massive and numerous injuries to the chest than from collision with a moving car. Fractures of the spine, bleedings to the pleural cavity, mediastinum, and the heart coating, and rupt

Maternal injuries during the periconceptional period and the risk of birth defects, National Birth Defects Prevention Study, 1997-2005.

Maternal injuries during pregnancy are common (∼7% prevalence). However, few studies have examined the association between maternal injuries and birth defects. The National Birth Defects Prevention Study is a population-based case-control study of birth defects in 10 U.S. states. Cases were ascertained through surveillance; controls were randomly selected from infants delivered without major birth defects in the study regions. Mothers completed a telephone interview on exposures before and during pregnancy, including injuries. We assessed associations between periconceptional (month before unt

[Thoracic trauma in childhood. Radiologic findings].

In childhood blunt trauma to the chest wall is more frequent than penetrating injuries. Most of these are the result of traffic accidents. Solitary or serial rib fractures are seen more often than fractures of the sternum. Complications of thoracic injuries are pulmonary contusion, hemothorax and, less frequently, pneumothorax. Pulmonary contusion can result in post-traumatic pneumatocele or chronic pulmonary hematoma. Injuries of the heart, the great vessels and bronchotracheal rupture, presenting initially with pneumothorax, followed by atelectasis, rarely occur. Blunt thoracic trauma is fre

Efficacy and safety of SP16 in preventing acute kidney injury in at-risk subjects with chronic kidney disease undergoing elective cardiac surgery using the heart-lung-machine (EASE-AKI): study protocol for a prospective, randomised, double-blind, placebo-controlled clinical trial.

BACKGROUND: Cardiac surgery using cardiopulmonary bypass uses controlled hypoperfusion which leads to relative organ damage. Acute kidney injury is the most frequent and most important organ failure, in particular in patients with chronic kidney disease. To date, there are no approved drug treatments that could effectively prevent acute kidney injury. SP16, an agonist of the low-density lipoprotein receptor-related protein 1, has been shown to exert both kidney- and cardioprotective effects in preclinical trials. Early clinical use of SP16 in phase I trials was safe. Administration of SP16 had
