The blood supply to the non-weight bearing midsole area is not derived exclusively from the medial plantar artery. This allows elevation of a subfascial mid-plantar cutaneous flap for the reconstruction of the heel. The technique of elevation of the flap is described. This plantar flap can be designed to include sensation without the need for deep dissection. Five clinical cases are discussed. This flap is useful for medium-sized chronic ulcerations of the heel.

INTRODUCTION: Due to medical procedures, preterm infants are at high risk for side effects of pain. In this regard, heel lancing for capillary blood sampling is a common painful procedure. The present study was conducted to assess the effectiveness of a simulated intrauterine sound on behavioral and physiological indices of pain due to heel-prick blood sampling in preterm infants.
METHODS: A double‑blind randomized clinical trial (RCT) was conducted. The data were collected from September 23 to December 22, 2019. We measured the effect of a simulated intrauterine sound on changes in the behav…

Non-healing neuropathic heel ulcer provides a challenge to salvage the limb from a below-knee amputation. Total calcanectomy can prove a reliable option for limb salvage. Given a well-designed orthosis, patients with total calcanectomy do well at any age. We present two case examples of non-healing neuropathic heel ulcers with chronic osteomyelitis of the calcaneus, which were salvaged with total calcanectomy and returned to all activities of daily living.

Isokinetic muscle strength measurements and functional tests are usually performed to evaluate ankle condition in chronic ankle instability (CAI), yet there is no clear demonstration of the relationship between isokinetic muscle strength and functional tests. The objective of this study was to evaluate the relationship between isokinetic muscle strength and functional tests in CAI. Between April 2014 and August 2016, 103 patients with unilateral CAI were studied. Single-leg balance, single-heel raise, and single-leg squat tests were performed for static balancing assessment. Single-leg hop, do…

BACKGROUND: The aim of this study was to evaluate plantar pressure changes after second metatarsal neck osteotomy using the Weil technique.
METHODS: Six below-knee cadaver specimens were used. Each specimen was held in a custom-built apparatus and loaded to 500 N for a period of 3 seconds. Using a computerized Musgrave pedobarograph, pressure measurements were made before and after osteotomy in both neutral and 45-degree heel rise positions. All osteotomies were made at an angle of approximately 20 degrees relative to the long axis of the metatarsal shaft. The metatarsal heads were displaced …

Ghosting is the phenomenon that exists when a footprint has a lighter area around the tip of one or more toes or a shadow-like area at the back of the heel. To date, ghosting has been considered primarily a finding of dynamic (walking) footprints, rather than static (standing) footprints. The prevalence of ghosting in static footprints is unknown, and research on its presence in static and dynamic footprints from the same participant is sparse, as are studies on its occurrence in different geographic populations. This study is among the first to evaluate the occurrence of ghosting in the stati…

BACKGROUND: As common as plantar fasciitis is, there's a lack of evidence regarding the true pathophysiologic process causing plantar fasciitis and plantar heel pain in general. This may partially explain the high variability and outcomes with current treatment of recalcitrant plantar fasciitis. Although Lemont reported myxoid degeneration of plantar fascia with histologic analysis of patients with fasciitis, muscle biopsy results were not reported. So far it appears we have not focused on the muscular component that may be present with plantar heel pain in general and in patients we diagnose …

INTRODUCTION: Heel pain is one of the most common painful conditions of the foot. There are many causes of heel pain, which are usually associated with calcaneal spurs. Hence, it becomes imperative to diagnose and treat them effectively. The development of calcaneal spur is somewhat not well known but is often said to be developed from inflamed plantar fascia. Heel being weight-bearing part of the body, it is very painful something and adversely affects the activity of daily living. Calcaneal spurs are fibrocartilaginous triangular projections from an insertional area of the plantar fascia. Ca…

Acute Achilles tendon rupture is frequently treated conservatively using functional rehabilitation protocols in which the ankle is held in equinus. Equinus is achieved using a variety of means including equinus casts and rigid boots with heel wedges. Concerns have been raised that rigid boots with heel wedges do not achieve adequate equinus. Patients presenting to our institutions with an acute Achilles tendon rupture were randomised to treatment with an equinus cast or rigid boot with heel wedges. After application of these lateral radiographs of the hindfoot and ankle were taken, and these u…

OBJECTIVE: The objective of the study was to investigate the sensitivity of the nociceptive withdrawal reflex to stimulation of different locations on the sole of the foot during hemiplegic gait.
METHODS: Reflexes were evoked by cutaneous electrical stimulation of 4 locations on the sole of the foot of 7 hemiplegic and 6 age-matched healthy persons. The stimuli were delivered at heel-contact, during foot-flat, at heel-off, and during mid-swing. Reflexes were recorded from muscles of the stimulated and the contralateral leg. Ankle, knee, and hip joints angles were recorded using goniometers.

Plantar fasciopathy is a common cause of heel pain. This article covers the imaging anatomy of the hindfoot, the imaging findings on ultrasound and magnetic resonance imaging (MRI) of plantar fasciopathy, plantar fibromas, trauma, Achilles tendonopathy, neural compression, stress fractures of the os calcis and other heel pad lesions. Thickening of the plantar fascia insertion more than 5 mm either on ultrasound or MRI is suggestive of plantar fasciopathy. Ultrasound is superior to MRI for diagnosis of plantar fibroma as small low signal lesions on MRI are similar to the normal plantar fascia s…

Five patients with partial tissue loss of the weight-bearing surface of the heel pad following ankle disarticulation were treated with residual-limb debridement and continued end-weight-bearing using a total-contact cast. The patients, ranging in age from 53 to 76 years, had insulin-requiring diabetes and insensate heel pads and were low-demand, limited-activity, community walkers before amputation surgery. Each underwent amputation surgery as a consequence of peripheral vascular disease. All patients progressed to complete wound healing over 3 to 6 months and were able to return to their prev…

Defects of the skin of the heel need to be repaired by skin which is both thick and sensitive. A technique of isolation of an island flap from the non-weight-bearing part of the medial side of the sole is described. It is vascularised by the medial plantar artery and innervated by the medial plantar nerve. Lesions of the heel, the medial malleolus and the lower part of the tendo Achilles can be covered by a one-stage surgical procedure without microsurgical suture.

Using optical pedobarography, the outcome of open reduction and internal fixation of displaced os calcis fractures was assessed. In 12 patients, well reconstructed heel contact widths and no reduction in heel-ground contact times were found. In conjunction with this, patients showed lateral deviation of the plantar pressures and load bearing, indicating that subtalar and calcaneocuboid joint function remained altered despite reconstruction. These findings could not be linked to the initial fracture severity or clinical outcome score.

Soft tissue and bone defects of the lower leg, ankle, and heel region often require coverage by local or distant flaps. The authors successfully used the distally based peroneus brevis muscle flap for the treatment of 15 patients with osteomyelitis (n = 5), melanoma (n = 1), Achilles tendon defects (n = 6), posttraumatic bone defects (n = 2), and chronic diabetic heel ulcer (n = 1). The size of the defects ranged from 6 to 60 cm. All defects were covered successfully without major complications by the muscle flap. The distally based peroneus brevis muscle represents a very reliable flap for co…

Calcaneal insufficiency fractures are uncommon and often overlooked because their early symptoms resemble more prevalent causes of heel pain. A 68-year-old postmenopausal woman presented with two months of progressive bilateral heel pain and swelling without trauma. Radiographs revealed nondisplaced bilateral calcaneal fractures characterized by subtle vertical lucencies and mild cortical discontinuity. Dual-energy X-ray absorptiometry confirmed severe osteoporosis, and laboratory evaluation excluded secondary metabolic bone disease. The initial symptoms mimicked plantar fasciitis and Achilles…

This study investigated the effects of various shoe features and surfaces on step time variability and pelvis accelerations (RMS) during walking in six younger and 22 older adults. Participants walked at a self-selected speed in five shoe conditions (standard, elevated heel, soft sole, hard sole, and high collar) on two surfaces: level and irregular. Results showed an age-related reduction in step time variability on the irregular surface and increased medio-lateral (ML) acceleration RMS in older people walking on the irregular versus the level surface. When wearing the elevated heel shoes, bo…

BACKGROUND: Touch interventions such as massage and skin-to-skin contact relieve neonatal pain. The Parental touch trial (Petal) aimed to assess whether parental stroking of their baby before a clinically required heel lance, at a speed of approximately 3 cm/s to optimally activate C-tactile nerve fibres, provides effective pain relief.
METHODS: Petal is a multicentre, randomised, parallel-group interventional superiority trial conducted in the John Radcliffe Hospital (Oxford University Hospitals NHS Foundation Trust, Oxford, UK) and the Royal Devon and Exeter Hospital (Royal Devon University…
