The relationship between herpes simplex virus (HSV) infection and specific cell-mediated immunity was investigated in 141 patients before and for the first four months after marrow transplant. Sixty-two (82%) of 76 seropositive patients but only one of 65 seronegative patients developed HSV infection. Lymphocyte responses to HSV antigen were suppressed immediately after transplant and subsequently became reactive in those patients with HSV infection. The presence or absence of antibody to HSV in the donor before transplant did not influence the response. Seventy long-term survivors of marrow t…

A 23-year-old patient developed acute upper abdominal symptoms during in-patient treatment for vulvo-vaginitis. Clinical picture, scintigraphy and angiography suggested a space-occupying process of the liver. Laparotomy showed ascites and a congested liver covered with cell necroses. The patient died the day after the operation from circulatory failure. Virological investigations of liver and spleen cell suspensions as well as the ascites fluid led to the demonstration of herpes virus type 2. The complement binding reaction gave a titre of 1:20 to herpes simplex virus. Disseminated herpes simp…

Purpose: To describe a rare case of congenital herpes simplex virus type 2 (HSV-2) infection with bilateral retinal involvement in a neonate. Methods: A single case is presented along with a brief literature review. Results: A neonate born to a mother with unrecognized HSV-2 infection presented with skin lesions initially thought to be secondary to amniotic band syndrome. Subsequent evaluation revealed positive skin cultures for HSV-2, suggesting in utero transmission, most likely during the early second trimester. Fundus examination demonstrated large, bilateral hyperpigmented chorioretinal s…

Pemphigus vulgaris (PV) is a mucocutaneous autoimmune disorder that can be challenging to diagnose and treat. We discuss a clinical case of a patient with a previous medical history of autoimmune gastritis and vitiligo who presented with painful oral ulceration but was devoid of dermatological manifestations. Immunoglobulin M (IgM) antibodies and polymerase chain reaction (PCR) amplification of Herpes simplex virus type 1 (HSV-1) were detected from the base of the ulcers. Treatment with acyclovir only modestly improved the lesions. Anti-desmosome antibodies were positive, and an oral biopsy sh…

Herpes simplex encephalitis (HSE) is a severe central nervous system (CNS) infection associated with cognitive decline, psychotic symptoms, and epilepsy. Despite aggressive antiviral and corticosteroid treatments, post-HSE complications remain challenging to manage. We report a case of a 33-year-old female patient with post-HSE neuropsychiatric symptoms who showed significant improvement following Cerebrolysin treatment. The patient was treated with acyclovir and dexamethasone in the acute phase, followed by Cerebrolysin administration in the post-acute phase. This case highlights the potentia…

INTRODUCTION AND IMPORTANCE: The herpes simplex virus (HSV) is the most common cause of acute sporadic encephalitis, a severe and often fatal disease in humans. It is associated with high mortality and morbidity rates in untreated patients.
CASE PRESENTATION: An 11-month-old child was admitted to the hospital presenting with acute fever and seizures characterized by staring episodes and spastic movements affecting the left side of the body. Diagnostic workup revealed abnormal T2 flair hyperintense foci in bi-temporoparietal lobes and right thalamus, and bilateral otomastoiditis were detected.…

As one of the common infectious corneal diseases, herpes simplex keratitis (HSK) has diverse clinical manifestations, is prone to recurrence, and can lead to blindness. In recent years, as new virus detection technologies, treatment drugs and surgical methods have emerged, there are more options for the diagnosis and treatment of HSK, but many problems still exist. In order to further standardize the clinical diagnosis and treatment of HSK and provide guidance and reference for clinical work, the Ocular Infection Group of Chinese Ophthalmologist Association has gathered relevant domestic exper…

BACKGROUND: Over the past several decades, there have been advances in diagnosis and treatment of neonatal herpes simplex virus (HSV) disease. There has been no recent comprehensive evaluation of the impact of these advances on the management and outcomes for neonates with HSV.
METHODS: Clinical data for initial presentation, treatment, and outcomes were abstracted from medical records of neonates with HSV treated at Seattle Children's Hospital between 1980 and 2016.
RESULTS: One hundred thirty infants with a diagnosis of neonatal HSV were identified. Between 1980 and 2016, high-dose acyclov…

Herpes simplex labialis (HSL) is a viral disease that affects the perioral region. No guidelines recommending an effective treatment exist. The treatment of HSL with three different products was examined. Herpatch Serum, a film-forming patch, was compared to Compeed Patches, a set of semiocclusive hydrocolloid patches, and Zovirax Cream (ingredient: 5% acyclovir). In this prospective, randomized, examiner-blind study, 180 patients with recurrent HSL were split into three groups (Compeed: n = 60, Herpatch: n = 60, Zovirax: n = 60) and examined within 24 hours of HSL outbreak (DRKS Registration …

Herpes simplex virus-1 (HSV-1) establishes latency preferentially in sensory neurons of peripheral ganglia. A variety of stresses can induce recurrent reactivations of the virus, which spreads and then actively replicates to the site of primary infection (usually the lips or eyes). Viral particles produced following reactivation can also reach the brain, causing a rare but severe form of diffuse acute infection, namely herpes simplex encephalitis. Most of the time, this infection is clinically asymptomatic. However, it was recently correlated with the production and accumulation of neuropathol…

A retrospective chart review examined adult patients with herpes simplex (HSV) and zoster (HZO) keratitis at a single institution. Patients who suffered a poor outcome (defined as visually significant corneal scarring, neurotrophic keratitis, secondary glaucoma, or requiring corneal surgery) were identified and each outcome type was analyzed in relation to age, sex, diabetes mellitus, immunosuppression, and a prior history of ≥2 ocular procedures.Advanced age, diabetes mellitus, and a prior history of ≥2 ocular procedures may be risk factors for poor outcomes in HSV, but not HZO, keratitis. In…

Herpes simplex viruses (HSV) types 1 and 2 are ubiquitous. They both cause genital herpes, occasionally severe disease in the immunocompromised, and facilitate much HIV acquisition globally. Despite more than 60 years of research, there is no licensed prophylactic HSV vaccine and some doubt as to whether this can be achieved. Nevertheless, a previous HSV vaccine candidate did have partial success in preventing genital herpes and HSV acquisition and another immunotherapeutic candidate reduced viral shedding and recurrent lesions, inspiring further research. However, the entry pathway of HSV int…

Viruses commandeer host cell 26S proteasome activity to promote viral entry, gene expression, replication, assembly, and egress. Proteasomal degradation activity is critical for herpes simplex virus (HSV) infection. The proteasome inhibitor bortezomib (also known as Velcade and PS-341) is a clinically effective antineoplastic drug that is FDA approved for treatment of hematologic malignancies such as multiple myeloma and mantle cell lymphoma. Low nanomolar concentrations of bortezomib inhibited infection by HSV-1, HSV-2, and acyclovir-resistant strains. Inhibition coincided with minimal cytoto…
