Neurorespiratory pattern of gill and lung ventilation in the decerebrate spontaneously breathing tadpole.

A decerebrate, spontaneously breathing tadpole preparation (Taylor-Kollros stages 16-19) was used to test the general hypothesis that the efferent bursting activities of cranial nerves (CN) V, VII and spinal nerve (SN) II are respiratory in nature, and, in particular, to identify separate and specific neural correlates of gill and lung ventilation. Oropharyngeal pressure (POP), intrapulmonary pressure (PIP), electromyogram (EMG) of the buccal levator muscle (interhyoideus), and efferent neural activities of CN V, CN VII and SN II were recorded while the animal was exposed to hyperoxia (100% in

Percutaneous transtracheal ventilation for laser endoscopic procedures in infants and small children with laryngeal obstruction: report of two cases.

The case reports of two children with obstructive lesions of the larynx are presented to illustrate the advantages of transtracheal ventilation for paediatric endoscopic laser surgery. The first patient was a four-month-old infant with inspiratory stridor due to a subglottic haemangioma obstructing 80 per cent of the lumen. The second patient was a five-year-old child with posterior synechiae of the larynx. The anaesthetic and ventilation techniques were the same for both cases. A transtracheal catheter was introduced percutaneously into the trachea under endoscopic control and connected to a

Challenging acquired pulmonary interstitial emphysema in an extreme preterm infant: A case report and review of literature.

BACKGROUND: Pulmonary interstitial emphysema (PIE) can occur in the immature lungs of preterm infants due to overdistension from mechanical ventilation. CASE REPORT: Preterm female infant (26 weeks of gestational age, 950 grams) born via urgent cesarean section, APGAR 1/3/6. Invasive mechanical ventilation during the first day of life transitioned to noninvasive positive pressure ventilation (NIPPV), afterward initially stable. Apnea and bradycardia noted at 20 days of life, requiring endotracheal intubation, with complementary studies showing right PIE. Progressive emphysema deterioration, a

The design of future pediatric mechanical ventilation trials for acute lung injury.

Pediatric practitioners face unique challenges when attempting to translate or adapt adult-derived evidence regarding ventilation practices for acute lung injury or acute respiratory distress syndrome into pediatric practice. Fortunately or unfortunately, there appears to be selective adoption of adult practices for pediatric mechanical ventilation, many of which pose considerable challenges or uncertainty when translated to pediatrics. These differences, combined with heterogeneous management strategies within pediatric critical care, can complicate clinical practice and make designing robust

The practical aspects of combination therapy with angiotensin receptor blockers and angiotensin-converting enzyme inhibitors.

Chronic lung disease (CLD) or bronchopulmonary dysplasia is a recognized sequel of preterm birth. With improving survival of infants at lower gestational ages, the incidence is on the rise. Pathological features of CLD include alveolar maldevelopment, with or without areas of pulmonary fibrosis. Assisted ventilation, infection/inflammation, oxygen administration, and fluid overload are the major risk factors in the evolution of CLD.Interventions, including the treatment of maternal infection, administration of prenatal glucocorticoids, and postnatal surfactant replacement therapy, improve the

Prolonged myasthenic syndrome after one week of muscle relaxants.

A child developed severe, generalized muscle weakness which persisted for 6 weeks, after receiving muscle relaxants for 1 week while requiring ventilator support. Electrodiagnostic studies indicated a presynaptic disorder of the neuromuscular junction which improved with high-frequency stimulation, similar to findings in Lambert-Eaton syndrome. Muscle specimens exhibited neurogenic targetoid fiber atrophy. Ultrastructure of the neuromuscular junction indicated terminal axon degeneration and atrophy with depletion of the secretory vesicles. Most reported patients with post-ventilator paresis ha

[Hemodynamic and ventilatory complications of mechanical ventilation with high intrinsic positive end-expiratory pressure].

In three mechanically ventilated patients ventilatory and circulatory complications resulted from high levels of intrinsic positive end-expiratory pressure (PEEPi): progressive pulmonary hyperinflation due to impairment of the expiration. PEEPi was initially not considered as the cause of shock and low tidal volumes and/or high inflation pressures. In a 74-year-old man the circulation deteriorated further when hand bagging was started in an attempt to improve his ventilatory condition; after reduction of the respiration rate, he recovered well. In a 40-year-old woman with relapsing polychondri

Management of bronchopleural fistula in the adult respiratory distress syndrome.

Bronchopleural fistula occurring as a complication in patients with the adult respiratory distress syndrome typically appears after 1 to 2 wks of illness, and is associated with a poor prognosis. Whether the bronchopleural fistula per se worsens outcome is not known because of the lack of studies on its natural history. There are several potential adverse effects of bronchopleural fistula in adult respiratory distress syndrome (e.g., incomplete lung expansion, loss of effective tidal volume or positive end-expiratory pressure, inability to remove CO2, etc.), but the actual frequency of these p

Sildenafil as “first line therapy” in pulmonary persistent hypertension of the newborn?

Despite the recent advances, the clinical approach to persistent pulmonary hypertension of the newborn (PPHN) still represents an important challenge for neonatologists. The care of newborns with PPHN requires meticulous therapeutic and ventilation strategies including, besides the stabilization of the newborn, the use of nitric oxide and high-frequency ventilation. However, not all the neonates with PPHH are responsive to this clinical approach. Recent studies have proposed the use of sildenafil, a phosphodiesterase 5 inhibitor, in refractory forms of PPHN. The aim of this study is to review …

Surfactant therapy for meconium aspiration syndrome: current status.

Meconium aspiration syndrome (MAS) is an important cause of respiratory distress in the term infant. Therapy for the disease remains problematic, and newer treatments such as high-frequency ventilation and inhaled nitric oxide are being applied with increasing frequency. There is a significant disturbance of the pulmonary surfactant system in MAS, with a wealth of experimental data indicating that inhibition of surfactant function in the alveolar space is an important element of the pathophysiology of the disease. This inhibition may be mediated by meconium, plasma proteins, haemoglobin and oe…

A retrospective study of infants with severe persistent pulmonary hypertension (PPHN) managed without extracorporeal membrane oxygenation (ECMO).

We studied 13 consecutive infants admitted to our Neonatal Intensive Care Unit over 37 months from 1 June 1994 to 30 June 1997, who were diagnosed with severe persistent pulmonary hypertension (PPHN) meeting extracorporeal membrane oxygenation (ECMO) criteria as defined by Bartlett and/or Short. They were managed with conservative ventilation strategy, with emphasis on the use of moderate ventilatory pressures whilst avoiding paralysis. Peak inspiratory pressure (PIP) on intermittent mandatory ventilation was adjusted according to adequate chest excursion. High PIP was avoided. Two main ventil

Adjunctive therapies for treatment of severe respiratory failure in newborns.

BACKGROUND: Severe respiratory failure of the newborn requires adjunctive therapies as application of surfactant, inhalation of nitric oxide (iNO), high frequency oscillatory ventilation (HFOV), or extracorporeal membrane oxygenation (ECMO). We designed this study to analyze the the usage and effectiveness of adjunctive therapies and the mortality of severe respiratory failure. PATIENTS AND METHODS: The survey in Germany was done in collaboration with the "Erhebungseinheit für seltene pädiatrische Erkrankungen" (ESPED). 397 patients within 2 years were included into the study. Iloprost inhalation for the treatment of severe persistent pulmonary hypertension of the newborn, experience at QSNICH.

BACKGROUND: Persistent pulmonary hypertension of the newborn (PPHN) is the most serious condition that causes high mortality in term and post term infants. The authors have an experience of using high frequency oscillatory ventilation (HFOV) and inhaled nitric oxide (iNO) for treatment of this condition with a good result. However, due to high cost of iNo, other pulmonary vasodilators have been use. Sildenafil had some side effects of systemic hypotension. Thus, inhaled iloprost was introduced for treatment of PPHN at our institute. OBJECTIVE: To evaluate the outcome of inhaled iloprost for t

Outcome of congenital diaphragmatic hernia: analysis of implicated factors.

AIM: Congenital diaphragmatic hernia remains a significant challenge for neonatologists and pediatric surgeons. Over the last years, new therapeutic approaches, as high-frequency oscillatory ventilation, inhaled nitric oxide, permissive hypercapnia, extracorporeal membrane oxygenation, have been used for the management of these newborns. We conducted a retrospective study of all infants who were managed for congenital diaphragmatic hernia in our NICU in order to identify possible clinical characteristics which were predictive for survival. METHODS: We reviewed a single institution's experienc

Pulmonary development considerations in the surgical management of congenital diaphragmatic hernia.

Congenital diaphragmatic hernia remains a clinical challenge for both neonatologists and pediatric surgeons. Advancements in mechanical ventilation strategies and neonatal intensive care have improved survival and transformed treatment of congenital diaphragmatic hernia from emergent surgery to early stabilization of the newborn followed by delayed repair of the diaphragmatic defect. Surgical technique has evolved and minimally invasive surgical approaches to close the diaphragmatic defect in these babies will likely improve with increasing experience. Finally, as more patients are diagnosed p

New methods advance treatment for respiratory distress syndrome.

The past 20 years of hard labor in neonatal respiratory distress have resulted in several important advances arising from our understanding of lung chemistry and physiology. Our knowledge of surfactant chemistry has enabled us to identify infants at high risk for developing neonatal distress due to surfactant deficiency, reducing this risk with antenatal corticosteroid treatment, and improving the neonatal outcome by means of administration of effective surfactant replacement. Recognition of the roles of oxygen toxicity and barotrauma in promoting lung injury has premitted the development of m
