INTRODUCTION: Total joint replacement in patients suffering from developmental dysplasia of the hip poses specific technical difficulties due to insufficient bone stock at the site of the original and secondary acetabulum and a narrow, cranially displaced proximal femur.
MATERIALS AND METHODS: Twelve hips with severe congenital dislocation (4 Crowe type II, 5 type III, 3 type IV) were treated with cementless, porous structured total hip replacement. The cup was implanted at the anatomic height, a femoral segment was resected below the minor trochanter to reduce the femoral component in all ca…

BACKGROUND: In recent studies, good intermediate-term results have been reported after primary hybrid total hip arthroplasty (a cementless acetabular component with a cemented femoral stem) for the treatment of primary osteoarthritis. However, few studies have described the results of this technique in patients with developmental dysplasia of the hip.
METHODS: One hundred primary hybrid total hip replacements were performed in ninety patients to treat degenerative arthritis of the hip secondary to developmental dysplasia. Seventy-one patients (eighty-one hips) were available for clinical and …

Fractures of the hip in children have been associated with a very high rate of serious complications including avascular necrosis (up to 47%) and coxa vara (up to 32%). Over a period of 20 years, we have treated displaced fractures by early anatomical reduction, internal fixation and immobilisation in a spica cast to try to reduce these complications. We have reviewed 18 patients who had a displaced non-pathological fracture of the hip when under 16 years of age. Their mean age at the time of the injury was eight years (2 to 13). They returned for examination and radiography at a mean follow-u…

We describe a prospective study of 328 patients undergoing arthroscopy of the hip. Arthrography, CT or MRI was performed when clinically indicated. A preoperative diagnosis was reached in 174 patients (53%), while the remaining 154 were diagnosed as having 'idiopathic hip pain'. In seven patients, access to the hip was inadequate. Arthroscopy altered the diagnosis in 176 hips (53%). The new primary diagnoses were osteoarthritis (75 patients), osteochondral defects (34), torn labra (23), synovitis (11) and loose bodies (9). In 172 hips (52%) an operative procedure was undertaken. In the remaini…

Therapeutic success in dysplasia and congenital dislocation of the hip depends on an early diagnosis. The physiopathology remains very debatable and several concepts are propounded. For a better physiopathologic understanding, the authors have carried out a study of the morphology and development of 22 pre- and neonatal hips. At first, the acetabulum is cartilaginous and distorted by the moving femoral head; this acetabulum is histologically affected by the femoral pressure. The pathologic hip is characterized by defective posterior bony coverage of the femoral head by the acetabulum. The acet…

Treatment of congenital dislocation of the hip (CDH) by closed reduction and immobilization in the hip spica cast is one of the accepted methods for use in patients under one year of age. We report 74 congenitally dislocated hips treated with premanipulation skin traction, closed reduction under anesthesia, adductor tenotomy and immobilization in the hip spica cast. Satisfactory results were obtained in 60 hips. In seven hips, avascular necrosis of the capital femoral epiphysis was observed with careful management, closed reduction and immobilization in a spica cast provides good results for t…

Results of 1193 dislocated and dysplastic hips in 695 children of a consistent treatment by skin traction and splinting in a mitigated abduction device (Hanausek position) are given with regard to the acetabular development and avascular head necroses. The acetabular development depends on the initial acetabular dysplasia and is overall satisfactory through final acetabular angles (Hilgenreiner) between 22 and 25 degrees even in highly dysplastic acetabula in late cases. The procedure is non-aggressive proven by low rate of femoral head necroses of 3.4%. Factors contributing to higher rates an…

The treatment of congenital dislocation of the hip in 16 patients (32 hips) older than 9 years of age at the time of this follow-up study was evaluated. Most of the patients were treated at the age between 1 and 3 years. Right and left hips treated by different methods, either conservative treatment, Colonna capsular arthroplasty or open reduction, were examined radiologically and compared. Consistent results were not obtained in the patients who underwent conservative treatment or Colonna capsular arthroplasty. Among the patients of open reduction, better results were obtained in those of "op…

Thirty-two synovial chondromatoses of the hip have been treated surgically. Nine cases were discovered accidentally in arthrotic hips being treated by total arthroplasty. Eleven partial joint clearances were made, five by an anterior approach and six by a lateral approach without dislocation of the hip, with bad results due to persistence of pathological synovium in the depth of the acetabular fossa. Twelve complete joint clearances were made with subtotal synovectomy and removal of all loose bodies, especially from the acetabular fossa, by dislocating the hip. The results were better and the …

The extensive work on early diagnosis and treatment of CDH (in the broad sense of this term) in Sweden has formed the basis of our present organization of prevention of hip dislocation. Practically all children are born in hospital, at present about 93,000 per year (1980). Routine examination of the hip joints has been included in the check-ups of newborns since the beginning of the 1950's. The centralization of obstetrics, especially during the last decade, has facilitated the organization of these examinations by paediatric consultants in all obstetric departments. All cases of hip joint ins…

We analyzed the preoperative and perioperative data on sixty-nine consecutive complex total hip replacements performed in fifty-seven patients using miniature or microminiature custom-made femoral components. The patients' average age was 45.7 years. The preoperative diagnosis was congenital dysplasia or dislocation of the hip in 72 per cent of the hips. Acetabular bone grafts were required in thirty-seven of the sixty-nine hips. Operative complications occurred in thirteen (19 per cent) and postoperative dislocation occurred in eleven (16 per cent) of the sixty-nine hips. Fifty-five hips in f…

Total hip arthroplasty in adolescents is rare but increasingly performed due to advances in implant technology. The indications differ from adults and are primarily osteonecrosis, secondary osteoarthritis or residual deformities from pediatric hip diseases, such as developmental dysplasia of the hip or slipped capital femoral epiphysis. Anatomical challenges, smaller implant sizes and prior surgery demand careful preoperative planning with advanced imaging. Modern uncemented implants and enhanced wear-resistant articulations, such as ceramic-on-ceramic (CoC) or ceramic-on-highly cross-linked p…

The hip is a complex load-bearing joint susceptible to both acute traumatic and chronic overuse injuries. Traumatic injuries, such as acute fractures, dislocations, apophyseal avulsions, and muscle tears, typically follow high-energy events. In contrast, overuse injuries, for example, femoroacetabular impingement, stress fractures, labral degeneration, and greater trochanteric pain syndrome, arise gradually from repetitive mechanical overload. Early and accurate diagnosis requires a multimodal imaging approach: radiographs for baseline assessment, computed tomography for detailed osseous injur…

Knee and hip osteoarthritis (OA) are two of the leading causes of disability globally. Knee OA is characterized by gradual degeneration of articular cartilage, leading to pain, stiffness, and functional limitations. Patients older than 50 years typically present with knee OA, but it can manifest earlier, particularly following traumatic knee injuries. Symptoms include pain, effusion, stiffness, and reduced range of motion. Radiographs commonly are used to confirm a diagnosis of knee OA. Nonsurgical management options include weight loss, lifestyle interventions, orthotic and assistive devices,…

Approximately 800,000 total knee arthroplasties and 450,000 total hip arthroplasties are performed annually in the United States. These procedures provide significant pain relief and restore function in patients with advanced osteoarthritis, rheumatoid arthritis, and other degenerative joint conditions. Patient evaluation before surgery includes a history, physical examination, laboratory tests, and imaging. After surgery, the rapid recovery protocol typically is used. This involves early mobilization of the replaced joint with ambulation as soon as 4 to 8 hours after surgery. Postoperative an…

The evaluation and treatment of adolescents and young adults with hip pain has seen tremendous growth over the past 20 years. Labral tears are well established as a common cause of hip pain but often occur because of underlying bony abnormalities. Femoroacetabular impingement (FAI) and acetabular dysplasia are now well-established causes of hip osteoarthritis and are increasingly treated in the prearthritic stage in hopes of improving symptoms and prolonging the longevity of the native hip. Beyond FAI and acetabular dysplasia, this patient population can present with a complex and variable gro…
