Helical PHILOS plating in shaft humeral fractures with proximal extension: a safe choice using an anterior approach.

Helical plating for humeral shaft fractures has gained popularity in recent years as it avoids iatrogenic nerve damage. A total of 16 patients with humeral shaft fracture involving the metaphyseal proximal part were treated between Dicembrer 2016 and January 2023. Complications such as pseudarthrosis, infections and nerve lesions were investigated during the postoperative period. Functional outcome was quantified using the Italian version of the DASH 30 quality of life assessment item. Healing of the fracture was achie-ved at the last follow-up with good clinical recovery and no pain. The self

Positional Screw Effect in the Treatment of Humeral Shaft Fractures Using a Minimally Invasive Plate Osteosynthesis Technique.

BACKGROUND: This study compares the difference in the clinical and radiologic outcomes when minimally invasive plate osteosynthesis (MIPO) technique is performed with or without using a positional screw in the treatment of humeral shaft fractures. METHODS: From January 2010 to January 2021, a retrospective study was conducted on a total of 63 patients who underwent the MIPO technique for the treatment of humeral shaft fractures. We divided these patients into 2 groups: in group I, patients underwent MIPO without a positional screw; in group II, patients underwent MIPO with a positional screw.

Treatment Reality of Proximal Humeral Fractures in the Elderly-Trending Variants of Locking Plate Fixation in Germany.

BACKGROUND: The surgical treatment of proximal humeral fractures (PHFs) with locking plate fixation (LPF) in the elderly is associated with high complication rates, especially in osteoporotic bone. Variants of LPF such as additional cerclages, double plating, bone grafting and cement augmentation can be applied. The objective of the study was to describe the extent of their actual use and how this changed over time. METHODS: Retrospective analysis of health claims data of the Federal Association of the Local Health Insurance Funds was performed, covering all patients aged 65 years and older, …

Treatment of humeral shaft fractures with different treatment methods: a network meta-analysis of randomized controlled trials.

PURPOSE: Humeral shaft fractures (HSFs) can be treated non-operatively (Non-OP), with open reduction and plate osteosynthesis (ORPO), minimally invasive plate osteosynthesis (MIPO), or with intramedullary nails (IMN). However, the best treatment for HSFs still remains controversial. We performed a network meta-analysis to explore which should be the best method for HSFs. METHODS: The computerized search had been conducted on electronic databases PubMed, EMBASE, Cochrane Library, and Medline from the establishment of the database to the end of December 2022. The quality evaluation of the includ

Outcomes and complications after open reduction and internal fixation of distal humeral fractures with precontoured locking plates.

OBJECTIVES: The aim of this study was to evaluate functional and radiographic results after open reduction and internal fixation of distal humeral fractures using precontoured locking plates. Our main hypothesis was that patients older than 65 years have inferior outcomes compared with younger patients. METHODS: All patients treated for a distal humeral fracture with precontoured locking plates between 2006 and 2017 at a level 1 trauma center were identified. Included patients underwent a clinical examination, and new radiographs were obtained. Functional outcomes were evaluated using Quick D…

Radial nerve palsies associated with paediatric supracondylar humeral fractures: a caution in the interpretation of neurophysiological studies.

Traumatic and iatrogenic neurological complications associated with paediatric supracondylar humeral fractures are well recognised. The severity of the nerve injury associated with supracondylar humeral fractures can be difficult to assess clinically and relies upon clinical progression or absence of recovery and neurophysiology. It is accepted that complete nerve palsy with neurophysiological complete block and absence of clinical recovery after three months requires surgical exploration and reconstruction. However, we argue that even a partial nerve palsy that is failing to recover as expect…

Scoring of radiographic cortical healing with the radiographic humerus union measurement predicts union in humeral shaft fractures.

PURPOSE: The purpose of this study is to determine if the radiographic humerus union measurement (RHUM) is predictive of union in humeral shaft fractures treated nonoperatively. METHODS: All patients with long bone fracture nonunion presenting to a single surgeon were enrolled in a prospective registry. This registry was queried to identify patients with humeral shaft fractures treated nonoperatively and developed nonunion. The nonunion cohort was matched to a three to one gender- and age-matched control group that were treated nonoperatively for a humeral shaft fracture and achieved union. T

Nationwide trends in management of proximal humeral fractures: an analysis of 77,966 cases from 2008 to 2017.

BACKGROUND: There is no consensus as to the treatment of proximal humeral fractures (PHFs), particularly in elderly patients. There is increasing evidence that nonoperative management may have similar functional outcomes to operative management, which is potentially conflicting with increasingly improved surgical techniques and implants. The aim of this study was to investigate the changes in the incidence and management of PHFs across Australia over a 10-year period. MATERIALS AND METHODS: We retrospectively reviewed all hospitalizations of patients with PHFs from 2 Australian national healt…
