Congenital adrenal hyperplasia (CAH) is a recognized cause of precocious pseudopuberty. Some children with CAH also develop true precocious puberty with early maturation of the hypothalamic-pituitary-gonadal axis. We have seen four such children (three boys and one girl) who had the diagnosis of CAH made between the ages of 3 and 6 yr. Four patients with pigeon breeder's lung and positive lymphocyte transformation tests to pigeon antigen were given 250 mg hydrocortisone intravenously, after which the test was repeated at 6, 24, and 30 hr. A significant reduction in lymphocyte responsiveness to pigeon antigen was evident 6 hr after the steroid dose, which reversed within 24 hr after the treatment. Conventional replacement steroid therapy, currently recommended for the treatment of AI, is associated with increased frequency of metabolic comorbidities due to daily overexposure. Whilst patch testing with corticosteroids in ethanol is more sensitive than either petrolatum or the cream formulation, the frequency of false-negative reactions is not known. We have compared patch testing with corticosteroid at 1% in ethanol with intradermal (i.d.) tests using 1 mg corticosteroid suspended in normal saline.
