Relation between radionuclide imaging and pathologic findings of ureteropelvic junction obstruction in neonatal hydronephrosis.

OBJECTIVE: An excessive amount of collagen fibers around the muscle cells in the ureteropelvic junction could be responsible for obstruction in patients with hydronephrosis. We aimed to elucidate the ultrastructure of the ureters and correlate this finding with the prognostic outcome and to correlate the histopathological findings with diuretic radionuclide renography findings. MATERIAL AND METHODS: Biopsy specimens of 20 children who underwent dismembered pyeloplasty for ureteropelvic junction obstruction were analyzed. The patients were grouped according to their age: infants (<12 months) a…

Congenital nephrogenic diabetes insipidus presented with bilateral hydronephrosis: genetic analysis of V2R gene mutations.

Most cases of hydronephrosis are caused by urinary tract obstruction. However, excessive polyuric syndrome rarely gives rise to non-obstructive hydronephrosis, megaureter, and a distended bladder. The authors report here on two cases of congenital nephrogenic diabetes insipidus (NDI) with severe bilateral hydronephrosis and megaureter. It is Interesting that the patients were symptomless except for their polyuria, and they both presented with bilateral hydronephrosis. Fluid deprivation testing revealed the presence of AVP resistant NDI. Gene analysis for these patients showed the AVP receptor …

49, XXXXY syndrome with severe vesico-ureteral reflux and hydronephrosis: report of one case.

The 49, XXXXY syndrome was first reported in 1960. It represents a rare sex chromosome aneuploidy syndrome. Some consider it as the most sev … ent ductus arteriosus and hypotonia. Moreover, he also got grade V vesico-ureteral reflux, R't with hydronephrosis and urinary tract infection. We hope to remind clinicians to arrange chromosomal analysis if multiple congenital anomalies exist in a neonate. And remember to exclude congenital genitourinary tract anomaly if karyotype 49, XXXXY is diagnosed.

Management of congenital hydronephrosis with ureteropelvic junction obstruction: the Vienna-AKH experience 1986-2001.

One of the most common causes of congenital hydronephrosis is obstruction of the ureteropelvic junction. The obstruction can be detected with prenatal ultrasonography screening and treated before renal function is reduced; the obstruction may also resolve spontaneously. Currently, there is no test for predicting the outcome of this obstruction. Management guidelines for neonates with asymptomatic obstruction of the ureteropelvic junction are based on expert opinions, but not on evidence-based data. In our retrospective study, we evaluated management and outcome of 26 renal units in 23 infants …

Ultrastructural analysis of human proximal tubules and cortical interstitium in chronic renal disease (hydronephrosis).

A systematic ultrastructural analysis of proximal tubule atrophy and cortical interstitial changes was carried out in human chronic nephropathy. The investigation was based on human hydronephrotic kidneys, which had been surgically removed and subsequently perfusion-fixed for light and electron microscopy. Normal kidney tissue, which was derived from nephrectomy specimens with pathological changes confined to part of the kidney or to the renal pelvis, was used for control material. A slight degree of proximal tubule atrophy was characterized by reduction of mitochondria and basolateral membran

A comparative analysis of retroperitoneal small-incision open pyeloplasty and laparoscopic pyeloplasty for infantile hydronephrosis.

BACKGROUND: With the development of minimally invasive surgery, laparoscopic pyeloplasty has become the mainstream surgery for ureteropelvic junction obstruction(UPJO), while open surgery is used less frequently. However, in regions where endoscopic techniques are not yet well-developed or in very young infants, open pyeloplasty still plays a significant role. This study aims to evaluate the clinical efficacy and safety of the retroperitoneal small-incision open pyeloplasty in infants by comparing it with laparoscopic pyeloplasty. METHODS: This retrospective cohort enrolled 117 infants diagno

Fibroepithelial Polyp of the Proximal Ureter Presenting as Chronic Hematuria and Hydronephrosis in an Adolescent: A Case Report.

Fibroepithelial polyps of the ureter are rare benign lesions that can produce long-standing obstruction and hematuria and may be misinterpreted as calculi or malignancy, risking delayed diagnosis and renal injury. We report the case of a 17-year-old male with a 10-year history of intermittent left flank pain and recurrent macroscopic hematuria, whose recent symptom progression prompted imaging that revealed grade-3 left hydronephrosis and a focal intraluminal soft-tissue density at the ureteral transition zone. Diagnostic ureteroscopy demonstrated a fungating polypoid mass in the upper third o…

Inguinal bladder hernia with bilateral hydronephrosis: a case report of urodynamic and functional recovery assessments.

We report a case of a 50-year-old man referred to our department with a marked inflammatory response and severe renal dysfunction. Computed tomography scans revealed an inguinal bladder hernia with bilateral hydronephrosis. A urethral catheter was placed to relieve urinary retention, and antimicrobial therapy was initiated for pyelonephritis. Despite adequate urine output, renal function improved only partially, and hydronephrosis persisted. Further imaging and intraoperative findings confirmed that the hernia sac was adjacent to the bladder and the bilateral lower ureters, which were signific…

The utility of renal sonographic measurements in differentiating children with grades 2, 3, and 4 hydronephrosis.

INTRODUCTION: Prior analysis of children with grade 3 and 4 congenital hydronephrosis demonstrated that renal medullary pyramidal thickness (PT) is predictive of subsequent pyeloplasty (area under the curve [AUC] = 0.78). The objective of this study was to further analyze the utility of sonographic measurements including PT, anteroposterior pelvic diameter (APD), and renal length with an expansion of the number of infants with hydronephrotic kidneys including grades 2, 3, and 4 hydronephrosis. METHODS: A retrospective review was performed of 166 consecutive infants with hydronephrosis (188 hy…

Levopositioned caudal vena cava and circumcaval left ureter with subsequent hydronephrosis and hydroureter in a dog.

The presentation and investigation of a levopositioned caudal vena cava, a rare congenital vascular abnormality seen mainly in Bernese Mountain Dogs, which resulted in ureteral compression, hydronephrosis and hydroureter. Surgical transection, transposition and anastomosis were performed to reposition the circumcaval ureter. A 19-month-old male neutered Bernese Mountain Dog was presented with a 13-month history of vague signs including intermittent abdominal pain, inappetence and diarrhoea, which were poorly responsive to medical management. Abdominal ultrasound revealed left-sided hydronephro…

Outcome of Patients with Antenatally Diagnosed hydronephrosis with Respect to Postnatal Diagnosis and Need for Surgical Intervention.

AIMS: This study aims to determine the etiology of antenatal hydronephrosis (ANH) and predict need for surgical intervention based on antenatal renal pelvis anteroposterior diameter (APD). MATERIALS AND METHODS: Combined prospective and retrospective study (2012-2018) of ANH cases with postnatal follow-up. Surgical intervention was correlated with the degree of hydronephrosis (HDN) and pelvis APD measured at the 2nd trimester, 3rd trimester, and postnatal 6-week follow-up. RESULTS: One hundred and sixty-five patients were studied with a total of 219 ANH units. Transient HDN was seen in 116 u…

Role of ureterocalicostomy in management of giant hydronephrosis in children in contemporary practice: Indications, outcomes and challenges.

INTRODUCTION: Given the rarity of giant hydronephrosis (GH), ideal surgical approach, assessment of drainage after surgery, expected improvement in renal function and morphological changes in the kidney on follow up have not been studied extensively. The aim of this study was to investigate the role of ureterocalicostomy (UC) in unilateral GH with respect to its current indications, outcomes and the challenges associated with it. MATERIALS AND METHODS: We retrospectively analysed data of 25 children (up to the age of 18 years) who underwent UC between January 2008 and January 2019 using open,…

Predictive values of haematuria and hydronephrosis in suspected renal colic: An emergency department retrospective audit.

OBJECTIVE: To determine the predictive values of haematuria and hydronephrosis in suspected renal colic. METHODS: We undertook a retrospective audit of adult patients who received a computed tomography (CT) of the kidneys, ureter and bladder (CTKUB) for suspected renal colic, presenting to Eastern Health Emergency Departments in Melbourne, Australia, between 1 January 2016 and 30 June 2016. CT hydronephrosis was used as a substitute for ultrasound hydronephrosis. We looked at the association of haematuria and CT proven hydronephrosis with CT proven ureteric and obstructing pelvic calculi. RE…

Adult giant hydronephrosis diagnosed in the second trimester of pregnancy: A case report and literature review.

Adult giant hydronephrosis in a normally sited kidney is unusual during pregnancy. The most frequent cause is congenital obstruction at the ureteropelvic junction. Ultrasound accompanied by magnetic resonance imaging (MRI) are valuable in reaching the diagnosis, especially when clinical assessment of an abdominal mass is inconclusive regarding aetiology. We report a case of giant hydronephrosis in a woman who presented at 23 weeks of gestation with abdominal distension. She was managed conservatively. Unfortunately, the pregnancy was complicated by severe pre-eclampsia at 32 weeks of gestation…
