Hypercalcemia in squamous cell carcinoma of the renal pelvis? Parathyroid or paraendocrine in origin.

Hypercalcemia associated with metastatic cancer may be due to osteolytic invasion or paraendocrine effects of the neoplasm. A patient with refractory hypercalcemia of long duration underwent cervical parathyroid exploration; three glands were found, but hypercalcemia persisted. After localization studies, a fourth gland was uncovered from the mediastinum. Hypercalcemia persisted, and at operation for kidney stones, squamous cell carcinoma of the renal pelvis was discovered, which progressed to cause the patient's death in hypercalcemia.

Parathyrin measured concurrently with free or total calcium in the differential diagnosis of hypercalcemia.

We assessed the diagnostic utility of measuring C-terminal immunoreactive parathyrin (i-PTH) concurrently with free (CaF) or total (CaT) calcium in four groups of patients: (1) 91 patients with histologically proven primary hyperparathyroidism; (2) seven patients without histological evidence of hyperparathyroidism; (3) 70 patients with non-parathyroid malignancies with hypercalcemia; and (4) 28 similar patients without hypercalcemia. In patients in Group 1, the use of either calcium measurement along with i-PTH increased diagnostic accuracy from 95 to 99%. In Group 2, values consistent with p…

Parathyroid hormone and 25-hydroxycholecalciferol levels in hypercalcemia of acute renal failure.

A 38-year-old man developed symptomatic hypercalcemia during the diuretic phase of acute renal failure secondary to rhabdomyolysis. Although early secondary hyperparathyroidism was documented, parathyroid hormone levels fell when hypercalcemia occurred. The 25-hydroxycholecalciferol level was normal on two separate occasions during the hypercalcemia. Remobilization of muscle calcium deposits during diuresis is proposed as the cause of the hypercalcemia in this syndrome.

Rhabdomyolysis-Induced Resistant Hypercalcemia During the Recovery Phase of Acute Kidney Injury.

Rhabdomyolysis is a clinical syndrome characterized by skeletal muscle injury and the release of intracellular contents into the circulation. While early hypocalcemia is common, hypercalcemia during the recovery phase is a rare but serious complication. We present a case of a 36-year-old male who developed severe, refractory hypercalcemia following rhabdomyolysis-induced acute kidney injury. Despite standard treatment with loop diuretics, low-calcium hemodialysis, and calcitonin, the hypercalcemia persisted. In this report, we highlighted a rebound hypercalcemia occurring during recovery from …

French national diagnosis and care protocol (PNDS) for infantile idiopathic hypercalcemia (IIH).

The objective of this study was to establish a French National Diagnosis and Care Protocol (PNDS: Protocole National de Diagnostic et de Soins), with the aim of providing health professionals with free, open access synthesis on optimal management and care of patients with Infantile Idiopathic Hypercalcemia (IIH) (https://www.has-sante.fr/jcms/p_3522489/fr/hypercalcemie-infantile-idiopathique-hii). The process involved a critical review of the literature and a multidisciplinary expert consensus. IIH is a rare genetic disorder, with a prevalence estimated at 1 in 33,000. Its clinical manifestati…

Neonatal hypocalcemia caused by maternal hypercalcemia: clinical characteristics, etiology, treatment, and outcome.

BACKGROUND: Maternal hypercalcemia is a rare but potentially serious cause of late-onset neonatal hypocalcemia. Data on the clinical characteristics and outcomes of mother-baby couplets with maternal hypercalcemia and subsequent neonatal hypocalcemia remains scarce. METHODS: This retrospective study reviewed medical records of neonates with late-onset hypocalcemia (≥3 days after birth) caused by maternal hypercalcemia at a tertiary medical center in Taiwan from 2014 to 2021. A stepwise diagnostic approach was applied to exclude other secondary causes. The etiologies of maternal hypercalcemia …

Multiple Myeloma Presenting as Hypercalcemia-Induced Acute Severe Pancreatitis: A Case Report.

Hypercalcemia is a known presentation of multiple myeloma along with anemia, renal failure, and bone pains (CRAB: hypercalcemia, renal failure, anemia, and bone lesions). Hypercalcemia is a known cause of pancreatitis besides the common causes like gallstones, alcohol and hypertriglyceridemia. However, hypercalcemia secondary to multiple myeloma is a rare cause of pancreatitis and only a few cases of the same have been reported. We present a case of a 60-year-old female who presented with acute severe pancreatitis induced by hypercalcemia secondary to multiple myeloma.

Hypercalcemia-Associated Hodgkin's Lymphoma Presenting as Normal Pressure Hydrocephalus: A Case Report.

Hodgkin's lymphoma is a malignancy of the lymphatic system that can rarely present with advanced-stage features such as spinal cord involvement and hypercalcemia. We present the case of a 63-year-old Caucasian male with advanced Hodgkin's lymphoma, presenting with hypercalcemia and symptoms resembling normal pressure hydrocephalus. The patient exhibited gait impairment, altered mental status, and urinary incontinence, forming the classic triad seen in normal pressure hydrocephalus. Laboratory findings revealed leukocytosis, anemia, hypercalcemia, and acute kidney injury. Imaging showed extensi…

Posterior reversible encephalopathy syndrome secondary to malignancy-associated hypercalcemia: A case report.

Posterior reversible encephalopathy syndrome (PRES) is a reversible clinical syndrome usually characterized by a range of neurological manifestations and distinctive neuroimaging findings reflecting vasogenic edema. PRES has been described in the context of various clinical settings including: renal failure, blood pressure fluctuations, use of cytotoxic drugs, autoimmune disorders and eclampsia. Hypercalcemia is rarely associated with PRES. We report a patient with lung cancer presenting as PRES secondary to hypercalcemia.

Cinacalcet increases renal calcium excretion in PTHrP-mediated hypercalcemia: a case report.

BACKGROUND: In the acute setting, PTH-independent hypercalcemia is typically treated with anti-resorptive agents such as zoledronic acid or denosumab. When these agents are no longer able to control hypercalcemia, several case reports have shown the utility of cinacalcet. However, it is not known if cinacalcet can be effective in patients naïve to anti-resorptive therapy or how cinacalcet ameliorates the hypercalcemia. CASE PRESENTATION: A 47-year-old male with a history of alcohol-induced cirrhosis was admitted for left cheek bleeding and swelling from an infiltrative squamous cell carcinoma…

Ionized hypercalcemia in 238 cats from a referral hospital population (2009-2019).

BACKGROUND: Ionized calcium concentration ([iCa]) is more sensitive for detecting calcium disturbances than serum total calcium concentration but literature on ionized hypercalcemia in cats is limited. Urolithiasis is a possible adverse consequence of hypercalcemia. HYPOTHESIS/OBJECTIVES: To describe clinical details of diagnoses associated with ionized hypercalcemia in cats and association with urolithiasis. ANIMALS: Cats (238) seen between 2009 and 2019 at a referral hospital with [iCa] above the normal reference interval. METHODS: Observational cross-sectional study. Signalment, serum bi…

A Rare Triad: Hypercalcemia-Induced Necrotizing Pancreatitis Presenting as Severe Diabetic Ketoacidosis.

Primary hyperparathyroidism (PHPT) typically occurs in persons above 45 years, with a female predominance. PHPT induces a state of hypercalcemia, but acute pancreatitis is a rare sequelae of this hypercalcemia. We report a case of a 31-year-old man with no known medical history who presented in diabetic ketoacidosis with electrolyte abnormalities. His clinical course progressed to multi-organ dysfunction despite correction of metabolic derangements. Further workup led to the discovery of the uncommon triad by which previously undiagnosed PHPT precipitated severe diabetic ketoacidosis.

Silicone-Induced Granulomatous Reaction Causing Severe Hypercalcemia: Case Report and Literature Review.

A 67-year-old woman presented to the hospital with complaints of abdominal pain. Physical exam was significant for signs of severe dehydration, mild epigastric tenderness and multiple non-tender hard nodules over her arms and thighs. Incidental finding of severe hypercalcemia led to negative workups for hyperparathyroidism, vitamin D intoxication, and malignancy. However, elevated levels of 1,25-hydroxy vitamin D raised the possibility of granulomatous diseases. Imaging and patient report revealed silicone-induced foreign body granulomatous reaction as the cause of hypercalcemia. Use of silico…
