New approaches to treatment of secondary hyperparathyroidism.

A large number of patients with chronic kidney disease are affected by secondary hyperparathyroidism. The related mineral metabolism abnormalities are associated with an increased relative risk of morbidity and mortality. The management of secondary hyperparathyroidism is made more complex by the fact that the disease progresses over time. Recent approaches to its management include vitamin D analogs, non-calcium/non-aluminum containing phosphate binders and calcimimetics.

Role of cervical ultrasonography in primary hyperparathyroidism.

AIM: To evaluate the role of USG in the preoperative localization of parathyroid adenomas in patients with symptomatic hyperparathyroidism and to compare its usefulness with that of scintigraphy scan and postoperative findings. MATERIAL AND METHODS: Twenty-five patients with symptomatic primary hyperparathyroidism were subjected to USG of the neck and nuclear scintigraphy, followed by surgery. The results were independently analyzed and compared with per-operative findings. RESULTS: The 25 patients had a total of 28 abnormal glands: 22 solitary adenomas, and 6 multiple adenomas (two each in …

A new paradigm for the treatment of secondary hyperparathyroidism.

The global rise in chronic kidney disease makes secondary hyperparathyroidism (SHPT) a growing medical concern. Conventional therapies for treating SHPT are limited and include calcium-based and calcium-free phosphate binders for reducing serum phosphorus and vitamin D or its analogues for simultaneous stimulation of calcium absorption and suppression of parathyroid hormone (PTH) gene expression. Control of SHPT using these therapies has typically been poor. Recent studies have demonstrated that use of calcimimetics that reduce PTH secretion by increasing the sensitivity of the parathyroid gla…

Predictors of hyperparathyroidism in renal transplant recipients.

The changes in parathyroid hormone secretion after successful renal transplantation remain to be clearly elucidated. Our study was aimed at identifying the predictors of hyperparathyroidism in renal transplant recipients. A retrospective single center study involving 37 renal transplant recipients, with a follow-up of at least one year, was performed. All transplants were performed using kidneys from living related donors. The average age of the study patients was 30 +/- 10 years, with a male-female ratio of 1.31. The mean duration on hemodialysis (HD) prior to transplantation was 25 +/- 18 mo…

A case of Turner syndrome with hyperparathyroidism in an adult.

Turner syndrome is a hereditary chromosomal anomaly that affects girls and women. A result of gonadal dysgenesis, its primary characteristics are short stature, osteoporosis, neck webbing, and cardiac defects. Turner syndrome may also involve the auditory system and kidneys. We report the case of a woman with Turner syndrome who presented late in adulthood with severe osteoporosis and hypercalcemia. She was subsequently diagnosed with primary hyperparathyroidism secondary to a parathyroid adenoma. After excision of the adenoma, the woman's serum calcium level normalized. To the best of our kno…

New perspectives in the management of secondary hyperparathyroidism.

The traditional treatment of secondary hyperparathyroidism, which affects many patients with chronic kidney disease and is associated with an increased risk of morbidity and mortality, is not very effective. Recent approaches to its management include the use of vitamin D analogues, non-calcium non-aluminium containing phosphate binders, and calcimimetics.

Suppressibility of parathyroid hormone in primary hyperparathyroidism.

OBJECTIVE: To describe an unusual case of pathologically confirmed primary hyperparathyroidism in a patient presenting with severe hypercalcemia and an undetectable parathyroid hormone (PTH) level. METHODS: We present a detailed case report and outline the serial laboratory findings. In addition, the possible causes of low serum PTH levels in the setting of primary hyperparathyroidism are discussed. RESULTS: A 16-year-old female patient presented with severe epigastric pain, found to be attributable to acute pancreatitis. At hospital admission, her serum calcium concentration was high (14.0 …

Primary hyperparathyroidism and pathological fractures: a review.

Primary hyperparathyroidism is due most often to a parathyroid adenoma secreting parathyroid hormone. Elevated PTH levels cause bone resorption, the formation of polyostotic lesions and a reduction in bone mineral density, predisposing to pathological fractures. The final stage of this disease is osteitis fibrosa cystica. The authors review the literature about osteitis fibrosa cystica and the treatment options when a pathologic fracture occurs.

Primary hyperparathyroidism and malignancy: "studies by nature".

The American Food and Drug Administration approval of parathyroid hormone (PTH) administration for osteoporosis as well as the possibility for its future therapeutic applications requires an examination of the suggested association between PTH and cancer, particularly osteosarcomas. The objective was to evaluate such a connection by collecting observational data from two groups of patients, designated as "studies by nature". Cohort 1: Medical records of all patients with primary hyperparathyroidism that were treated in a referral center during a 12-year period were retrospectively reviewed for…

[Tumor of the maxilla presenting as primary hyperparathyroidism].

Brown tumors are classic bony lesions of hyperparathyroidism. They usually occur in severe forms with osteolytic subperiosteal lesions. Facial bones are a rare localization: 2% of all cases. The aim of this study was to report a case of multiple craniofacial location of brown tumors and to discuss, within a review of the literature, the role of imagery in the diagnosis of this disease.

The impact of primary hyperparathyroidism on the oral cavity.

CONTEXT: Primary hyperparathyroidism (HPT) is a systemic disease causing bone loss. Periodontal disease is a local inflammatory disease characterized by alveolar bone loss. The older literature records that HPT is associated with loss of radicular lamina dura and brown tumors of the bone, but contemporary studies are lacking. OBJECTIVE: The objective of the study was to determine the effects of HPT on oral bony structures and periodontal disease in a contemporary population. DESIGN: This was a cross-sectional, case-controlled study. SETTING: The study was conducted at the clinics of endocri…

Cinacalcet hydrochloride: calcimimetic for the treatment of hyperparathyroidism.

Mineral metabolism disorders, including those related to secondary hyperparathyroidism, affect a large number of patients with chronic kidney disease and are associated with increased relative risk of morbidity and mortality in hemodialysis patients. The traditional therapy of secondary hyperparathyroidism based on vitamin D compounds and calcium-based phosphate binders is often limited by the increase of serum calcium and phosphorus levels limiting the dose that can be given safely, and thus, preventing the attainment of treatment targets. Cinacalcet hydrochloride (Sensipar®, Mimpara®, Parare…

The genetic background of uremic secondary hyperparathyroidism.

Many studies have shown how gene mutations and genetic polymorphisms could influence secondary uremic hyperparathyroidism (HPTH), modulating parathyroid (PT) function and hyperplasia. Parathyroid hormone (PTH) gene expression and hormone secretion is regulated mainly by serum calcium, with a post-transcriptional effect, and by vitamin D with a transcriptional effect. PT cells retain the ability to proliferate and to apoptose. Hyperphosphatemia, hypocalcemia and vitamin D deficiency all stimulate PT cell proliferation. In the early stage of chronic uremia, PT proliferation is polyclonal, as in …

Thyroid evaluation in patients with primary hyperparathyroidism.

We evaluated the efficacy of preoperative high-resolution ultrasonography (US) for diagnosing possible concomitant thyroid disease which affects the surgical management in patients with primary hyperparathyroidism (pHPT). One hundred and nine patients with sporadic pHPT underwent US with or without ultrasound-guided fine-needle aspiration biopsy (US-FNAB). Diagnosis of concomitant thyroid nodules by US and US-FNAB were compared with the histopathological findings. Of the 109 patients, 19 (17.4%) had malignant thyroid nodules, 26 (23.9%) had benign thyroid nodules alone, and 12 (11.0%) had diff…

[Surgical treatment of primary hyperparathyroidism in pregnancy].

Primary hyperparathyroidism during pregnancy is a pathological entity with an unknown incidence that presents certain physiological, therapeutic and prognostic characteristics leading to greater difficulty in diagnosis and a high risk of complications. We present the case of a 16-week pregnant woman who presented with asymptomatic hypercalcemia secondary to a solitary parathyroid adenoma. Since conservative measures produced no improvement, surgery was performed in the second trimester of pregnancy, with satisfactory pre- and postnatal results. Surgery in the second trimester constitutes a saf…

Cinacalcet HCl: a novel therapeutic for hyperparathyroidism.

Hyperparathyroidism (HPT) is a significant clinical concern for patients with a variety of diseases, notably the secondary HPT associated with chronic kidney disease requiring dialysis. Secondary HPT is associated with elevated para-thyroid hormone (PTH) levels, decreased levels of 1,25 dihydroxyvitamin D, and disordered mineral levels (usually high calcium and phosphorus). If not controlled, secondary HPT can result in bone disease, vascular calcification, and ultimately, patient mortality. Established, conventional therapies, such as 1,25dihydroxyvitamin D analogues (vitamin D analogues) and…
