A Lipid- and Inflammation-Related Metabolite Risk Score Predicts Incident Gout Among Individuals With Hyperuricemia: A Prospective Cohort Study.

BACKGROUND: Although hyperuricemia (HUA) is required for gout development, only a fraction of individuals with elevated serum urate progress to the disease. Metabolomic profiling may help uncover biological mechanisms and improve prediction of gout onset. METHODS: We conducted a prospective metabolomics analysis among 32,563 HUA individuals in the UK Biobank. Baseline plasma metabolites were measured, and differences between individuals who developed gout (n = 2,856) and those who did not were identified. Cox proportional hazards models with false discovery rate correction assessed metabolite…

Polysaccharides from Alpinia oxyphylla fruit prevent hyperuricemia by inhibiting uric acid synthesis, modulating intestinal flora and reducing renal inflammation.

Hyperuricemia (HUA) is one of the most common chronic diseases today, with a prevalence exceeding 14 % in both the United States and China. Current clinical treatments for HUA focus on promoting uric acid (UA) excretion and inhibiting UA production, but often neglect the strain on the liver and kidneys. The fruit of Alpinia oxyphylla (A. oxyphylla) is known to improve renal function, regulate metabolism, and exhibit anti-inflammatory effects; however, its effectiveness and mechanisms in treating HUA are not well understood. In this study, HUA mice induced by potassium oxonate and adenine were …

Identification of four genes as novel susceptibility loci for early-onset type 2 diabetes mellitus, metabolic syndrome, or hyperuricemia.

Given that early-onset type 2 diabetes mellitus (T2DM), metabolic syndrome (MetS), and hyperuricemia have been shown to have strong genetic components, the statistical power of a genetic association study may be increased by focusing on early-onset subjects with these conditions. Although genome-wide association studies have identified various genes and loci significantly associated with T2DM, MetS, and hyperuricemia, genetic variants that contribute to predisposition to these conditions in Japanese subjects remain to be identified definitively. We performed exome-wide association studies (EWA…

Point mutation in D8C domain of Tamm-Horsfall protein/uromodulin in transgenic mice causes progressive renal damage and hyperuricemia.

Hereditary mutations in Tamm-Horsfall protein (THP/uromodulin) gene cause autosomal dominant kidney diseases characterized by juvenile-onset hyperuricemia, gout and progressive kidney failure, although the disease pathogenesis remains unclear. Here we show that targeted expression in transgenic mice of a mutation within the domain of 8 cysteines of THP in kidneys' thick ascending limb (TAL) caused unfolded protein response in younger (1-month old) mice and apoptosis in older (12-month old) mice. While the young mice had urine concentration defects and polyuria, such defects progressively rever…

A Preventable Adverse Reaction: Drug Reaction With Eosinophilia and Systemic Symptoms (DRESS) Syndrome Following Allopurinol Use for Asymptomatic Hyperuricemia.

Drug reaction with eosinophilia and systemic symptoms (DRESS) is a potentially life-threatening drug-induced hypersensitivity reaction. We report the case of a 36-year-old man who developed DRESS syndrome after starting allopurinol for presumed asymptomatic hyperuricemia. He presented with fever and a widespread rash. Symptoms developed within 10 days of starting allopurinol. Labs showed transaminitis, delayed-onset eosinophilia, and elevated inflammatory markers. Biopsy confirmed a drug-induced hypersensitivity pattern. Of note, serum uric acid was normal on admission to our facility. The pat…

GWAS-identified hyperuricemia-associated IGF1R variant rs6598541 has a limited role in urate mediated inflammation in human mononuclear cells.

Gout is a common autoinflammatory joint diseases characterized by deposition of monosodium urate (MSU) crystals which trigger an innate immune response mediated by inflammatory cytokines. IGF1R is one of the loci associated with both urate levels and gout susceptibility in GWAS to date, and IGF-1-IGF-1R signaling is implicated in urate control. We investigate the role of IGF-1/IGF1R signaling in the context of gouty inflammation. Also, we test the gout and urate-associated IGF1R rs6598541 polymorphism for association with the inflammatory capacity of mononuclear cells. For this, freshly isolat
