[Inhibition of prostaglandins in the syndrome of inappropriate secretion of antidiuretic hormone (SIADH)].

The role played by prostaglandins in the secretion of ADH and modulation of its action was investigated by testing the effects of prostaglandin synthesis inhibition in patients with SIADH of cerebral or pulmonary origin. Three patients with SIADH of central origin and 3 with SIADH of peripheral origin were subjected to 4 successive water-loading tests. In the first and fourth tests, the patients absorbed water 20 ml/kg bodyweight; in the second test they absorbed alcohol 2.5 ml/kg followed by water 17.5 ml/kg. The third test was preceded by a 3-day inhibition of prostaglandin synthesis with in

A CASE OF HYPONATREMIA DUE TO PITUITARY METASTASIS OF LUNG CANCER.

CONTEXT: Hyponatremia is a common electrolyte abnormality. OBJECTIVE: We report a patient who presented with hyponatremia and diagnosed as small cell lung cancer metastatic to hypothalamus and pituitary. CASE REPORT: A 68 year old male patient was admitted with fever and cough and pneumonia was considered. Serum sodium level was 113 mmol/L. Syndrome of inappropriate ADH (SIADH) is considered. Thyroid function tests and cortisol levels pointed out a central deficiency in both axes. Pituitary MRI was performed and a hypothalamic and pituitary mass were observed. Prednisolone therapy was starte

Renal function and fluid therapy in high risk infants.

In recent years, the survival rate of high risk infants has markedly increased. The role of such medical management as fluid, electrolyte and nutritional therapy have assumed a greater importance in assuring optimal quality of the survivors. The very low birth weight infants, particularly those with respiratory distress syndrome and perinatal asphyxia, are at highest risk. The inefficient renal function, unique characteristic of body fluid composition and/or presence of severe clinical illness often make the management of fluid and electrolytes in this group of infants difficult. The numerous

Brain opiates and neuroendocrine function.

Opioid peptides are found throughout the central nervous system, and have profound effects on neuroendocrine function. In man, exogenous opiates and opioids elevate circulating prolactin, GH and TSH, and suppress the release of the gonadotrophins and pro-opiocortin-related peptides. However, unlike in other species, there is substantial evidence for a physiological role of endogenous opioids only in the case of the gonadotrophins and ACTH/LPH. Most evidence suggests that LH and FSH are modulated via the hypothalamus or amygdala, where concentrations of opioids and opioid receptors are very hig

Central nervous system involvement in adult T-cell leukemia/lymphoma.

Central nervous system (CNS) involvement was reviewed in 99 patients with adult T-cell leukemia/lymphoma (ATLL). Fifteen episodes of CNS involvement developed in ten of 99 patients (10.1%); nine had leptomeningeal involvement, whereas two developed intracerebral invasion, one developed cord involvement, and one developed both. CNS involvement was more frequent in the lymphoma type than in the other types of ATLL. Nuchal rigidity was not common (33%) and a syndrome of inappropriate secretion of antidiuretic hormone (ADH) occurred in association with CNS involvement (40%). Three episodes of mark

Coexisting paraneoplastic SIAD and tumour-induced osteomalacia in stage IV small cell lung cancer: a diagnostic challenge.

A 70-year-old man with stage IV (extensive-stage) small cell lung carcinoma presented with progressive respiratory symptoms, refractory hypophosphataemia and hyponatraemia. The diagnostic evaluation revealed paraneoplastic arginine vasopressin-mediated syndrome of inappropriate diuresis and fibroblast growth factor 23-mediated renal phosphate wasting, consistent with tumour-induced osteomalacia. This report highlights the diagnostic complexity and reasoning required to identify dual paraneoplastic syndromes in an advanced malignancy, with emphasis on biochemical interpretation and integration

Use of conivaptan for management of hyponatremia following surgery for Cushing's disease.

BACKGROUND: Hyponatremia due to syndrome of inappropriate antidiuretic hormone secretion (SIADH) is a common osmoregulatory complication following surgery for Cushing's disease. Conventional management includes water restriction and sodium repletion, however this regimen does not address the underlying pathophysiology of excessive vasopressin production. Vaptans are arginine vasopressin receptor antagonists shown to be effective in correcting water excess in other disease states of euvolemic and hypervolemic hyponatremia. The use of these agents has not been reported in Cushing's patients. ME

Dysautonomia after COVID-19 infection: A case report.

INTRODUCTION: This case report discusses a case of a patient who experienced acute autonomic dysfunction during the parainfectious phase of COVID-19, attributed to Guillain-Barre Syndrome (GBS). This is the first well-documented case of such an association. CASE PRESENTATION: A 64-year-old woman, previously infected with COVID-19, was admitted to the emergency department due to altered mental status. Brain computed tomography (CT) revealed bilateral occipital diffuse hypodensity. Throughout her hospitalization, she exhibited elevated blood pressure necessitating intravenous treatment. The ini

The frequency and clinical biology of the ectopic hormone syndromes of small cell carcinoma.

Eighty-four patients with small cell carcinoma of the lung were reviewed with respect to the frequency and biologic behavior of patients with a demonstrated ectopic hormone syndrome. This subgroup of 12 patients (14%) was compared with regard to stage of disease, distribution of metastases, response rate, and survival to those patients not demonstrating a clinical syndrome. Stage, distribution, and response rates are comparable in the two groups but differences were observed with regard to sites of metastases and survival. The ectopic hormone syndrome patients had an increased likelihood of li

A case of the syndrome of inappropriate secretion of antidiuretic hormone (SIADH) associated with lupus erythematosus in the central nervous system.

We report on a case of the syndrome of inappropriate secretion of antidiuretic hormone (SIADH) associated with lupus erythematosus in the central nervous system (CNS). A 73-year-old woman with essential hypertension suddenly demonstrated consciousness disturbance. Upon her admission, laboratory data showed significant hyponatremia (114 mEq/L) and a lack of body fluid loss. Diminished free water excretion (urine osmolality 684 mOsm/kg) and normal urine Na excretion (FENa 1.70 %) were consistent with the diagnosis of SIADH, which was confirmed by an inappropriately high concentration of plasma a

Sporadic Creutzfeldt-Jakob disease with MM1-type prion protein and plaques.

The authors report a 75-year-old woman with atypical sporadic Creutzfeldt-Jakob disease (CJD) characterized by MM1-type prion protein (PrP) (methionine homozygosity at codon 129 in the PrP gene and type-1 protease-resistant PrP) and PrP plaques. This patient is the first case of sporadic CJD with plaque-forming MM1-type PrP, suggesting either a shared prion strain with the plaque-forming subset of dural graft-associated CJD or shared host genetic factors that are unrelated to the PrP genotype.

[Effectiveness of the administration of a dietary supplement (oral urea) for the treatment of hyponatremia in SIADH].

Background and objectives: the most common cause of hyponatraemia in hospital settings is the syndrome of inappropriate antidiuretic hormone secretion (SIADH). This study compares the efficacy and safety of urea versus fluid restriction in the treatment of hyponatraemia caused by SIADH. Material and methods: an observational cohort study was conducted with 212 patients suffering from hyponatremia (Na+ < 135 mmol/L) due to SIADH at the Complejo Hospitalario Universitario de Pontevedra between January 2015 and May 2022. Of these, 112 patients received urea (15 g/day) and 100 were treated with fl

High-dose thiotepa may induce inappropriate secretion of antidiuretic hormone syndrome (SIADH) during conditioning treatment for allogeneic stem cell transplantation.

We conducted a retrospective, single-center, observational cohort study in patients with inappropriate secretion of antidiuretic hormone syndrome (SIADH) induced by high-dose thiotepa during conditioning for allogeneic stem cell transplantation (allo-HSCT). Between May 2018 and December 2023, 431 patients underwent allo-HSCT. All patients who developed SIADH had received TBF conditioning, and the incidence of SIADH among TBF-treated patients was 7.3%. Despite its limitations as a retrospective study, this real-life analysis shows that hyponatremia with corresponding serum hypo-osmolality and p
