Two Cases of Myeloid Sarcoma at Rare Sites: Cytological Manifestations and Diagnostic Experience.

Myeloid sarcoma is a rare extramedullary manifestation of acute myeloid leukemia, and primary involvement of the gastrointestinal tract and uterine cervix is particularly uncommon. Due to its low incidence and nonspecific clinical manifestations, the rates of clinical misdiagnosis and missed diagnosis remain high. Herein, we report two cases of myeloid sarcoma arising at rare sites. Case 1 was a 52-year-old male who presented with acute gastrointestinal obstruction. Intraoperative frozen section favored lymphoma, whereas touch imprint cytology of the tumor cut surface revealed numerous round t

Airborne particle dynamics during draping of a surgical microscope versus an exoscope in spinal surgery: A prospective comparative study.

BACKGROUND: Exoscopes provide ergonomic and visualization advantages over conventional surgical microscopes. Despite their growing adoption, the impact of device draping on transient airborne particulate generation during setup, a critical but understudied contributor to surgical site contamination, remains unclear. METHODS: This prospective observational study compared airborne particle dynamics during draping of a surgical microscope (Pentero) and an exoscope (Orbeye) in ten spinal surgeries (five per group) conducted in a laminar airflow operating room with high-efficiency particulate air

Subhepatic gallbladder: an extremely rare anatomical variant challenging the laparoscopic surgeon.

Gallbladder anatomical variations are common, but positional anomalies such as subhepatic gallbladder are exceedingly rare. Recognition of these variants is crucial for preventing iatrogenic bile duct injury during laparoscopic cholecystectomy. A 35-year-old medically free male presented with a 3-day history of epigastric pain associated with nausea, vomiting, and dark urine. Laboratory investigations revealed elevated liver enzymes and hyperbilirubinemia. Ultrasonography showed gallstones with a dilated common bile duct. Magnetic resonance cholangiopancreatography (MRCP) demonstrated a subhep

Drug-laced vaping-implications and emerging perioperative challenges for the anesthesiologist: a narrative review.

PURPOSE: Drug-laced vaping (DLV) involves vaping devices used to inhale non-nicotine psychoactive substances (e.g., cannabinoids, stimulants, opioids, benzodiazepines [BZDs]), sometimes without the user's awareness. Because exposure is often undisclosed and may not be detected by routine toxicology screening, DLV poses a hidden perioperative risk. We aimed to summarize current evidence, highlight emerging challenges, and outline implications for anesthesiologists. SOURCE: We performed a structured narrative review of MEDLINE, Embase®, Scopus, and the Web of Science™ (inception to November 202

Symptomatic Accessory Ossicle Near the Medial Malleolus Simulating Fracture Non-union: A Case Report and Literature Review.

INTRODUCTION: Accessory ossicles of the ankle and foot are normal anatomical variants that are usually asymptomatic. The os subtibiale is a rare accessory ossicle located just inferior to the medial malleolus, with a reported prevalence of 0.9-1.2% in the general population. When symptomatic, it can mimic fractures or fracture non-union, leading to diagnostic confusion. Accurate diagnosis relies on careful clinical evaluation and appropriate imaging. CASE REPORT: A 20-year-old male presented with persistent pain around the right ankle, 7 months after sustaining a twisting injury while walking

Methicillin-Resistant Staphylococcus aureus Iliac Osteomyelitis with Cortical Breach Mimicking Septic Arthritis of the Hip in a Child: A Diagnostic Pitfall.

INTRODUCTION: Pelvic osteomyelitis is an uncommon but clinically important condition in children, often presenting with symptoms that closely mimic septic arthritis of the hip. Overlapping features, such as fever, severe hip pain, limp, and inability to bear weight, frequently lead to diagnostic delays and, in some cases, to unnecessary hip exploration. Early recognition is essential, particularly with increasingly aggressive presentations associated with methicillin-resistant Staphylococcus aureus (MRSA). CASE REPORT: An 11-year-old girl presented with fever, acute left hip pain, and refusal

Appendi-sception unveiled: a case report of a toddler with concurrent ileocolic intussusception and appendicitis.

BACKGROUND: Ileocecal (ileocolic) intussusception is a time-critical pediatric surgical emergency and a leading cause of acute bowel obstruction in infants and toddlers. Acute appendicitis, although the most common pediatric surgical emergency overall, is uncommon in very young children and is diagnostically challenging at the extremes of age. The coexistence of ileocolic intussusception with acute appendicitis, often termed "appendisception," is rare and frequently recognized during surgery. CASE PRESENTATION: We report a 1-year-9-month-old child who presented with features consistent with i

Management and outcomes of facial nerve injury following rhytidectomy: a systematic review.

BACKGROUND AND AIM: Facial nerve injury is a critical complication of rhytidectomy, affecting patient outcomes and satisfaction. Despite its importance, standardized management strategies remain limited. This systematic review evaluates current evidence on the management, outcomes, and prevention of facial nerve injuries in rhytidectomy, with stratification by injury severity to enhance clinical applicability. METHODS: In this study, PubMed, Embase, and the Cochrane Library were searched from inception to July 2025, identifying 20 studies that met the inclusion criteria. The quality of the st

Mitigating perioperative pressure injuries in microsurgical breast reconstruction.

Pressure injuries (PI) that develop in the operating room (OR) account for just under half of all hospital acquired pressure injuries (HAPI) and contribute significantly to the high cost and patient morbidity of HAPI. Microvascular autologous breast reconstruction poses specific risks to PI development in patients and should be addressed by the reconstructive microsurgeon. Standard risk factors for perioperative PI include patient immobility, absent pain perception, and challenges to maintaining normal body temperature while under general anesthesia for surgery. Specific intraoperative risk fa

Retroaortic Left Renal Vein: A Case Series and Clinical Significance in Renal Surgery.

The retroaortic left renal vein (RLRV) is a rare anatomical variant in which the left renal vein passes posterior to the aorta. Although often asymptomatic, awareness of this anomaly is essential for avoiding vascular injury during retroperitoneal or renal surgery. We report three incidental cases of RLRV identified during imaging studies performed for unrelated renal or urinary conditions. A 53-year-old man presented with colicky left flank pain and multiple left renal calculi on computed tomography (CT). An incidental type I RLRV with orthotopic drainage into the inferior vena cava (IVC) at

A Rare Case of Appendicectomy in Situs Inversus: Challenges and Surgical Approaches.

Left-sided acute appendicitis (LSAA) is an extremely rare cause of acute abdominal pain, and even the most experienced surgeons may encounter it only a few times in their careers. While acute appendicitis remains the most common indication for emergency gastrointestinal (GI) surgery, its diagnosis is typically straightforward. However, in cases of situs inversus totalis (SIT), the atypical anatomical positioning can obscure the clinical picture and lead to diagnostic uncertainty, often mimicking other left-sided pathologies and potentially resulting in delayed treatment and complications. This

Risk factors of perioperative hypoparathyroidism after thyroidectomy: a systematic review and meta-analysis.

BACKGROUND: Perioperative hypoparathyroidism (hypoPT) represents a prevalent complication of thyroid surgery. Reported risk factors remain inconsistent, and identical factors may exert different effects depending on whether hypoPT is defined by serum calcium concentration or parathyroid hormone (PTH) levels. A comprehensive synthesis was undertaken to clarify risk factors for hypoPT under these distinct biochemical definitions. MATERIALS AND METHODS: Three databases (PubMed, Embase, and Scopus) were searched from inception to 2025. Study quality was assessed using the Newcastle-Ottawa scale.

Long-Standing, Giant, Right-Sided Inguinal Hernia Containing the Right Colon and Greater Omentum: A Case Report.

Giant inguinal hernias are rare, long-standing conditions characterized by massive herniation of intra-abdominal organs into the scrotum. Their chronicity, unusual contents, and associated comorbidities make diagnosis and surgical management particularly challenging. We report the case of a 63-year-old man with a right-sided inguinal hernia present for 39 years. His medical history included hypertension, ischemic heart disease, hypercholesterolemia, and diabetes mellitus. Preoperative assessment included recommendations for respiratory rehabilitation to optimize pulmonary function. During surg

A Family Physician's Experience in the Removal of Migrating Subdermal Contraceptive Implants.

Subdermal contraceptive implants are among the most effective and reversible contraceptive methods available today. Implanon®️, approved by the US FDA, is widely used for long-term family planning due to its extended contraceptive effects. Although generally safe, these implants can occasionally lead to rare but serious complications, such as migration into deeper axillary structures. In this case report, we describe the experience of a family physician with a 30-year-old Saudi woman whose Implanon implant migrated approximately 11 cm from the original insertion site over three years. The pati

Acquired Urethral Diverticulum After Holmium Laser Enucleation of the Prostate: A Case Report.

Acquired urethral diverticula (UD) in males is an uncommon entity, and it is rarely reported after an open simple prostatectomy or transurethral resection of the prostate. Here, we report a unique case of a UD presenting after holmium laser enucleation of the prostate (HoLEP) in a 69-year-old male with a prostate of 372 g who had five episodes of urine retention over one year despite combined medical treatment with tamsulosin 0.8 mg and finasteride 5 mg. The patient also has elevated prostate-specific antigen (PSA) with five negative prostate biopsies over the last few years. The procedure las

How obesity affects the disposition of intravenous anesthetics.

PURPOSE OF REVIEW: Understanding the changes in drug disposition of intravenous anesthetics in patients with obesity and administering appropriate doses are critical to avoid intraoperative awareness with recall because of underdosing and over-sedation and delayed emergence due to overdosing. Pharmacokinetic simulation or target-controlled infusion (TCI) using models that have been adapted to patients with obesity are necessary to select appropriate dosing regimens. This review aimed to describe the pharmacokinetic concepts underpinning the use of intravenous anesthetics, including propofol, r…

Litigation in Septorhinoplasty Surgery: A Pan-Specialty Review of National Health Service (the United Kingdom) Data.

Success in septorhinoplasty surgery can be difficult to assess due to a lack of objective and measurable outcomes. If patients' expectations are not met, it places surgeons performing septorhinoplasty at risk of litigation which can be stressful and costly. The National Institute of health (NHS) Resolution is a government-funded organization in the United Kingdom that provides expertise to the NHS on resolving patient concerns. Data were requested from NHS Resolution for claims involving septorhinoplasty surgery over a period of 5 years between April 2015 and April 2020. Rhinoplasty claims per
