The iron status in children with sickle cell anaemia has been studied in 45 patients. The mean haemoglobin level was 7.4 g/dl (s.d. 1.7) and the white cell count more than 11 X 10(9)/l in 35 (78%). Bone marrow examination showed depletion of iron stores of 21 (47%). The total iron binding capacity was elevated in most of the children and serum iron was below the normal mean level for age in 14 (31%) children. Selected parameters related to work tolerance were measured in 31 adult subjects with hemoglobin (Hb) concentration from 2.5 to 14.0 g/100 ml. Work tolerance was closely related to Hb concentration (r = 0.74) regardless of the adequacy of storage iron level. 6 patients with polycythaemia vera who also developed concomitant iron deficiency are reported. When oral iron therapy was given, there was a marked reduction in the previously elevated platelet counts in 4 patients; during 2-9 years of observation of these patients there was a significant inverse relationship between haemoglobin concentration and platelet count. Treatment of severe iron deficiency with iron-poly(sorbitol-gluconic acid) complex (Ferastral) intramuscular 10 ml (iron 500 mg) on alternate days has been shown highly effective and well tolerated. In order to see whether the time of treatment could be shortened, 20 Nigerians with severe iron deficiency (mostly from hookworm infection) were treated with daily intramuscular Ferastral 10 ml until their calculated total requirement of iron was met. The effect of iron deficiency anemia on the development of atherosclerosis was investigated in chicks. The control group of birds were fed a basal diet, the atherosclerosis group received 1% cholesterol. Splenic ultrastructure was studied in experimentally induced severe iron deficiency in rabbits. Fragmentation and phagocytosis of blood cells, particularly red cells and platelets, were observed in the red pulp.
