Fluid loading therapy to prevent spinal hypotension in women undergoing elective caesarean section: Network meta-analysis, trial sequential analysis and meta-regression.

BACKGROUND: Fluid loading is one of the recognised measures to prevent hypotension due to spinal anaesthesia in women scheduled for a caesarean section. OBJECTIVE: We aimed to evaluate the current evidence on fluid loading in the prevention of spinal anaesthesia-induced hypotension. DESIGN: Systematic review and network meta-analysis with trial sequential analysis and meta-regression. DATA SOURCES: Medline, Epub, Embase.com (Embase and Medline), Cochrane Central, Web of Science and Google Scholar were used. ELIGIBILITY CRITERIA: Only randomised controlled trials were used. Patients include

Fluid therapy in patients with pulmonary disease.

Fluid therapy in patients with pulmonary disease is challenging. Although a single set of rules cannot be applied to every patient, the following guidelines can be used when managing patients with pulmonary disease. Euvolemic patients with adequate tissue perfusion should be given sufficient isotonic fluid to balance insensible losses. If severe pulmonary compromise is present, cessation of all fluid therapy may be considered if the patient is able to match its losses by voluntary intake. In hypovolemic or hypotensive patients, small boluses of isotonic crystalloids or colloids should be given

Effect of intraocular irrigating solutions on the viability of cultured retinal vascular endothelial cells.

PURPOSE: To compare the abilities of balanced salt solution, BSS Plus and Hartmann's lactated Ringer's (HLR) solution to maintain the viability of retinal vascular endothelial cells (RVEC) in vitro. METHODS: Cultured retinal vascular endothelial cells were suspended in each irrigating solution for four hours. Viability was determined by trypan blue exclusion at 30 minute intervals. Regression analysis was used to determine the rate of viability loss. Additional studies were performed to determine the effectiveness of lactate in maintaining cell viability. RESULTS: Retinal vascular endothelia

Effects of intraluminal acidification on oesophageal motor activity.

This study assessed the effect of prolonged intraluminal acidification on the motor activity of the entire oesophageal body (under controlled conditions). Intraoesophageal pressures were recorded in 13 endoscopy negative subjects with gastro-oesophageal reflux disease in whom saline, HC1 0.1 N, and saline solutions were infused (1.5 ml/min) blindly in the oesophageal body, 6 cm distal to the upper oesophageal sphincter for three consecutive periods of 45 minutes each. These findings were compared with those of a control group. Intraoesophageal acidification caused an increase in the deglutitio

Effects of hydration on mitral valve prolapse.

We investigated the effect of hydration on mitral valve prolapse (MVP). Ten subjects with documented diagnosis of MVP were studied before and after oral hydration with 1 L of fluid. Increased weight and cardiac output were present after hydration. Results showed that all 10 subjects with diagnosis of MVP before hydration continued to have MVP after hydration; however, subtle changes were detected, especially on auscultation. Seven of 9 subjects (with cardiac examination recorded before and after hydration) had auscultatory findings of MVP before hydration. No detectable auscultatory change aft

Intestinal absorption of peptide enteral formulas in hypoproteinemic (volume expanded) rats: a paired analysis.

Previous studies have confirmed the improved tolerance of a peptide enteral compared to standard enteral alimentation in hypoalbuminemic, critically ill patients. Animal studies, including hypoproteinemic, volume-expanded rats, demonstrated that the protein hydrolysate of a peptide enteral formula was responsible for the enhanced absorption. The purpose of this study was to determine whether the composition of small MW peptides (protein hydrolysate) in two commercially available peptide enteral formulas would affect the rate of intestinal absorption and albumin clearance in intact jejunal loop

Partial metabolic inhibition stimulates adrenal medullary secretion possibly by activating Na-dependent Ca influx mechanism.

The rabbit adrenal gland perfused throughout with a Ca-deficient, Mg-containing medium becomes responsive to a challenge with Ca (2 mM) re-introduction after treatment with cyanide, 2-deoxy-glucose plus glucose deprivation or monoiodoacetate. The results suggest that the partial metabolic inhibition leads to the reduced activity of Na pumping, thereby activating Na-dependent Ca influx mechanism.

Hydroxyethyl starch in priming fluid for cardiopulmonary bypass.

The physiochemical characteristics of hydroxyethyl starch make it suitable for use as a colloidal blood plasma substitute. In high doses, this drug may interfere with blood coagulation. Because of its effectiveness and low cost, we have used hydroxyethyl starch rather than albumin in the priming fluid for cardiopulmonary bypass: 500 ml of 6% hydroxyethyl starch and 2,000 ml of lactated Ringer's solution. To determine if excessive bleeding has been associated with the use of hydroxyethyl starch, we reviewed 760 cardiac operations. The patients were 49.9 +/- 0.5 years old (mean +/- SEM) and weig

Functional renal involvement in normotensive patients with progressive systemic sclerosis: impaired sodium excretion during isotonic saline infusion.

An intravenous infusion of 2,000 ml isotonic saline was performed in 8 normotensive, normoreninemic patients with progressive systemic sclerosis. None of them had clinical evidence of renal disease. Total and proximal fractional excretion of sodium was reduced when compared with that of 8 normal subjects. No correlation was found with para-aminohippurate (PAH) clearance values. Two years after this study was done, 4 patients developed arterial hypertension; interestingly, plasma renin activity remained within the normal range. Reduced sodium excretion is suggested as having a pathogenetic role

Blood substitutes.

Substitutes for whole blood include blood fractions such as plasma, serum albumin and other fluids of various kinds which are not derived from blood but are used as plasma volume expanders; these, include the usual crystaloid intravenous solutions. Since in comparison to blood far more of these later solutions are given intravenously, a thorough knowledge of plasma volume expanders is essential. The first use of such expanders in human patients was by Hogan in 1915. He used colloidal gelatin and noted an improvement in blood pressure in shock. In 1945, Gronwall and Ingelman advocated the use o

Cerebral salt wasting syndrome in a patient who suffered a gunshot traumatic head injury: a case report.

Cerebral salt wasting syndrome is described as a hyponatremic condition in the context of a central nervous system injury. It is caused by a renal loss of sodium, and the primary distinguishing factor among similar conditions, such as the syndrome of inappropriate antidiuretic hormone secretion, is a reduction in the extracellular fluid volume. We describe here the case of a young adult who suffered brain injuries as a result of a gunshot and developed cerebral salt wasting syndrome during his in-hospital stay. Any type of cerebral attack can lead to hyponatraemic syndrome, but the most common

Acute diarrhea: evidence-based management.

OBJECTIVES: To describe the current recommendations on the best management of pediatric patients with acute diarrheal disease. DATA SOURCE: PubMed, Scopus, Google Scholar. DATA SUMMARY: There has been little progress in the use of oral rehydration salts (ORS) in recent decades, despite being widely reported by international guidelines. Several studies have been performed to improve the effectiveness of ORS. Intravenous hydration with isotonic saline solution, quickly infused, should be given in cases of severe dehydration. Nutrition should be ensured after the dehydration resolution, and is

Salt poisoning in beef cattle.

Salt poisoning has been described under various circumstances in adult cattle. Presenting clinical signs in 6 Holstein beef cattle with such poisoning were primarily dysfunction of the central nervous system and included ataxia, opisthotonus, nystagmus, depression, muscle twitching and intermittent convulsions, as well as abdominal pain and polydipsia. Diarrhea occurred in 2, and blindness in 3/6 cattle. Hypernatremia (161.8 - 178.8 mmol/L) and hyperosmolality (331.81 - 366.18 mOsm/L) were present in all animals. To treat the affected cattle, access to fresh water was restricted, vascular volu

Impact of osmolality on burning sensations during and immediately after intramuscular injection of 0.5 ml of vaccine suspensions in healthy adults.

A randomised placebo controlled double-blind cross-over trial was performed on twenty healthy adults to assess the effect of osmolality (300,600,850 and 1100 mOsm) on local tolerance of an intramuscular injection (0.5 ml) of five suspensions containing the same components as the excipients of a combined Diphtheria-Tetanus-acellular Pertussis-inactivated Poliomyelitis-Haemophilus influenzae type b paediatric vaccine (DtacP-IPV-Hib, PENTAVAC). The results did not show any dose-effect relationship between burning or pain sensations and the different osmolalities tested. Although mild and not clin

Gravel obstruction in the abomasum or duodenum of two cows.

Two Holstein cows from dry lot dairies were evaluated because of clinical signs of depression, anorexia, and decreased milk production. Bilateral abdominal distention and decreased fecal output were found on physical examination. Results of serum biochemical analyses indicated that the cows had severe hypochloremic, hypokalemic metabolic alkalosis. On the basis of physical examination and laboratory findings, an obstruction of the abomasum or cranial portion of the small intestine was suspected in these cows and abdominal exploratory surgery was performed. A mass of gravel was found in the pyl

Hypothalamic responsiveness in DOCA hypertensive rats augmented by streptozotocin-induced diabetes.

In rats with deoxycorticosterone acetate (DOCA) salt hypertension, induction of diabetes with streptozotocin did not aggravate the elevation in blood pressure, but pressor and sympathetic nerve responses to electrical stimulation of the ventromedial hypothalamus were enhanced. Along with glycosuria and hyperglycemia, the other effects of streptozotocin-induced diabetes were: reduced body weight, increased fluid intake, and bradycardia. Despite enhanced responsiveness to hypothalamic stimulation, blood pressure increases produced by intravenous injections of norepinephrine, tyramine, or vasopre

Gonadectomy and the development of age-dependent polydipsia and the intake of NaCl solutions in the SWR/J mouse.

Gonadectomies were performed at 25 days of life in SWR/J mice, a strain known to develop age-dependent nephrogenic diabetes insipidus with females drinking far more than males. Effect of gonadectomy was different for each of the 3 nonreproductive behaviors studied. (1) Ovariectomy reduced the age-dependent polydipsia seen in females, but castration was without effect in males. (2) Castration of males resulted in a lower intake of an isotonic NaCl solution, making the castrated males similar to the sham-operated and ovariectomized females which did not differ. (3) For the short-term activity me
